[Bafa in Tianhui medical bamboo slips]. 【八法在天会医疗简牍】。

{"title":"[<i>Bafa</i> in Tianhui medical bamboo slips].","authors":"Xiantong He, Changhua Liu, Feng Gao","doi":"10.13703/j.0255-2930.20241030-k0002","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20241030-k0002","url":null,"abstract":"<p><p>In 2012, a total of eight medical books were compiled from Tianhui medical bamboo slips unearthed in Chengdu, Sichuan, China, which for the first time, recorded <i>bafa</i> (<i>ba</i> technique) and acupuncture technique. <i>Bafa</i> is only found in Tianhui medical bamboo slips. Based on the records in medical bamboo slips, it is believed that the essential theory of <i>bafa</i> refers to an outlook of life for the harmony between form and spirit, meaning \"the heart mind is the monarch of form\". It is the initial view of physique centered on five-<i>zang</i> organs. In clinical application, <i>bafa</i> is used for prevention of diseases in four seasons, specially for <i>wubi</i> (five-<i>bi</i> disorders) and <i>wufeng</i> (five-wind disorders), covering a extensive parts of the body. Compared with the stone technique, \"<i>bafa</i> must work on meridian points, while stone on meridians\". <i>Bafa</i> is often used for deficiency syndrome, while stone technique is for excess syndrome. Hence, these two types of technique should be applied cautiously. [Automatic acquisition and analytic procedure of acupuncture manipulation based on optical navigation]. [基于光学导航的针刺手法自动采集与分析过程]。

An acquisition holder installed at the handle tail of filiform needle was designed to display the movement trajectory of the needle during acupuncture delivery by collecting the movement trajectory of holder. The 3-month old male Bama miniature pig was selected as the experimental subject, and 6 points, \"Bojian\" \"Qiangfeng\" \"Housanli\" \"Xiaokua\" \"Huiyang\" (BL35) and \"Baihui\" (GV20), were selected during acupuncture manipulation. The optical navigation system was used to collect the real-time data, and these data were per-processed and analyzed using mean filtering and Fourier transform. The acupuncture procedure was divided into 3 stages, inserting, lifting-thrusting, and twisting. The results showed that the accuracy was 96.3% at lifting-thrusting stage, and that was 100.0% at twisting stage. The decomposition effect of the entire procedure was satisfactory. This study provides a new approach to the quantitative analysis of acupuncture manipulation. The main symptom was repeated painful penile erections during nighttime sleep. The syndrome was differentiated as \"water failing to nourish wood\" and \"liver <i>qi</i> stagnation\". Acupuncture was applied at Baihui (GV20), Sishencong (EX-HN1), Guanyuan (CV4), and bilateral Neiguan (PC6), Hegu (LI4), Taichong (LR3), Taixi (KI3), Guilai (ST29), Yongquan (KI1), Yanglingquan (GB34), and Sanyinjiao (SP6). The treatment was combined with abdominal vibration therapy. Treatment was given once daily, five times per week. After the first treatment, the severity and duration of nocturnal erection pain were reduced, and sleep quality was improved. After the terminology related to etiology, pathogenesis, accompanying symptoms, acupoints, and treatment methods unified, the frequency statistical analysis and association rule algorithms were applied to analyze the implicit association patterns among various elements of syndrome differentiation, treatment selection, and acupoint selection in ancient prescriptions from multiple dimensions.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;The basic acupoints of high frequency in ancient acupuncture-moxibustion treatment for chest obstruction and heart pain were Daling (PC7), Neiguan (PC6), Taixi (KI3), Taichong (LR3), Shangwan (CV13), Yongquan (KI1), and Xinshu (BL15). The prescription was mostly composed of &lt;i&gt;yuan&lt;/i&gt;-source points. Besides, the combinations of two of five-&lt;i&gt;shu&lt;/i&gt; points, five-&lt;i&gt;shu&lt;/i&gt; points with &lt;i&gt;luo&lt;/i&gt;-connecting points, and &lt;i&gt;yuan&lt;/i&gt;-source points with &lt;i&gt;luo&lt;/i&gt;-connecting points were common. The high-frequency points were from the pericardium meridian of hand-&lt;i&gt;jueyin&lt;/i&gt;, conception vessel, kidney meridian of foot-&lt;i&gt;shaoyin&lt;/i&gt;, liver meridian of foot-&lt;i&gt;jueyin&lt;/i&gt;, and bladder meridian of foot-&lt;i&gt;taiyang,&lt;/i&gt; generally distributed on the &lt;i&gt;yin&lt;/i&gt; part of the arm, abdominal region, the &lt;i&gt;yin&lt;/i&gt; part of foot, the back, and the &lt;i&gt;yin&lt;/i&gt; part of the leg. Zhigou (TE6), Zusanli (ST36), Baihui (GV20), and Jiuwei (CV15), as well as the specific acupoint combinations, were used for chest obstruction and heart pain due to &lt;i&gt;qi&lt;/i&gt; stagnation. Moxibustion was more suitable for chest obstruction and heart pain caused by &lt;i&gt;qi&lt;/i&gt; reversion, cold and &lt;i&gt;qi&lt;/i&gt; stagnation. Shaohai (HT3) was invariably selected when hand tremor was accompanied; Zhongchong (PC9) combined with Daling (PC7) was selected specially for feverish sensation in the palms. Moxibustion was exclusively applied to Shangwan (CV13), and Taixi (KI3) was often stimulated with moxibustion. At Neiguan (PC6) and Daling (PC7), moxibustion was delivered in combination with acupuncture (high confidence was presented in acupuncture).&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Conclusion: &lt;/strong&gt;In ancient acupuncture-moxibustion treatment for chest obstruction and heart pain, the points of the pericardium meridian of hand-&lt;i&gt;jueyin&lt;/i&gt; are predominant, coordinated with those of the liver meridian of foot-&lt;i&gt;jueyin&lt;/i&gt;, kidney meridian of foot-&lt;i&gt;shaoyin&lt;/i&gt;, conception vessel, and bladder meridian of foot-&lt;i&gt;taiyang&lt;/i&gt;. The DKD model was established by feeding a high-fat, high-sugar diet combined with intraperitoneal injection of streptozotocin (STZ) in the modeling group. Twenty rats with successful DKD model were randomly divided into a model group (&lt;i&gt;n&lt;/i&gt;=10) and an acupuncture group (&lt;i&gt;n&lt;/i&gt;=10). The acupuncture group received \"spleen and stomach-regulating\" acupuncture at bilateral \"Zusanli\" (ST36), \"Fenglong\" (ST40), \"Yinlingquan\" (SP9), and \"Zhongwan\" (CV12), 30 min per session, once daily, five times per week, for four weeks. The general condition, fasting blood glucose (FBG), 2-hour postprandial glucose (2hPG), serum creatinine (SCr), blood urea nitrogen (BUN), 24-hour urinary protein quantification, and urine albumin-to-creatinine ratio (UACR) were compared before and after the intervention. After intervention, urinary podocyte injury marker SPON2 was measured by ELISA. Podocyte autophagosomes and glomerular basement membrane ultrastructure in renal tissue were observed via transmission electron microscopy. Podocyte apoptosis was assessed by TUNEL staining. The protein expression of microtubule-associated protein 1 light chain 3Ⅱ (LC3-Ⅱ), mTORC1, ULK1, Beclin-1, and p62 in renal tissue was detected by Western blot. LncRNA SOX2OT expression in renal tissue was measured by real-time PCR.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;After the intervention, compared with the control group, the model group exhibited increased food and water intake, increased urine output, weight loss, and loose stools; compared with the model group, the food and water intake, urine volume, and loose stools were improved in the acupuncture group. Compared with the control group, FBG, 2hPG, SCr, BUN, 24-hour urinary protein quantification, UACR, and urinary SPON2 were all higher in the model group (&lt;i&gt;P&lt;/i&gt;&lt;0.01); compared with the model group, the FBG, 2hPG, SCr, BUN, 24-hour urinary protein quantification, UACR, and urinary SPON2 were all lower in the acupuncture group (&lt;i&gt;P&lt;/i&gt;&lt;0.01). Compared with the control group, the model group showed reduced podocyte autophagosomes and thickened glomerular basement membrane; compared with the model group, the acupuncture group had increased podocyte autophagosomes and less thickened basement membrane. Compared with the control group, the podocyte apoptosis index (AI) was higher in the model group (&lt;i&gt;P&lt;/i&gt;&lt;0.01); compared with the model group, the AI was lower in the acupuncture group (&lt;i&gt;P&lt;/i&gt;&lt;0.01). {"title":"[Wrist-ankle acupuncture for functional frequent premature ventricular contractions: a randomized controlled trial].","authors":"Yuxin Huang, Yujiao Sun, Buping Liu, Huanfeng Lin","doi":"10.13703/j.0255-2930.20241029-k0003","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20241029-k0003","url":null,"abstract":"<p><strong>Objective: </strong>To observe the clinical efficacy of wrist-ankle acupuncture in the treatment of functional frequent premature ventricular contractions (PVCs).</p><p><strong>Methods: </strong>A total of 64 patients with functional frequent PVCs were randomly divided into an observation group and a control group, 32 cases in each group. The observation group was treated with wrist-ankle acupuncture at bilateral upper 1 and upper 2 on the wrist. The control group received sham acupuncture at the same points as the observation group. Both groups were treated once every day from Monday to Friday, with the needles retained for 60 min each time, for a total of 4 weeks. The TCM syndrome score, the 24-hour PVC count, and MOS 36-item short form health survey (SF-36) score were compared between the two groups before and after treatment, and the clinical efficacy was evaluated after treatment.</p><p><strong>Results: </strong>After treatment, the TCM syndrome scores and the 24-hour PVC counts in both groups were reduced compared with those before treatment (<i>P</i><0.01), and the above indexes in the observation group were lower than those in the control group (<i>P</i><0.01). After treatment,scores of all SF-36 items in the observation group were increased compared with those before treatment (<i>P</i><0.01); in the control group, the scores of general health (GH), social function (SF) and role-emotional (RE) were increased compared with those before treatment (<i>P</i><0.05). After treatment, scores of all SF-36 items in the observation group were higher than those in the control group (<i>P</i><0.01). The total effective rate in the observation group was 90.6% (29/32), which was higher than 46.9% (15/32) in the control group (<i>P</i><0.01).</p><p><strong>Conclusion: </strong>Wrist-ankle acupuncture has a significant clinical efficacy in the treatment of functional frequent PVCs. {"title":"[Thermo-electroacupuncture at <i>yaosanzhen</i> for chronic lumbar muscle strain of cold dampness: a randomized controlled trial].","authors":"Mengzhong Li, Jianguo Zhang, Wenjie Liang, Wenjie Bai, Xiaoping Lei","doi":"10.13703/j.0255-2930.20240924-k0001","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20240924-k0001","url":null,"abstract":"<p><strong>Objective: </strong>To compare the clinical efficacy between thermo-electroacupuncture at <i>yaosanzhen</i> and oral celecoxib in the treatment of chronic lumbar muscle strain with cold dampness.</p><p><strong>Methods: </strong>A total of 80 patients with chronic lumbar muscle strain of cold dampness were randomly divided into an observation group (40 cases, 1 case dropped out) and a control group (40 cases, 2 cases were excluded). The observation group was treated with thermo-electroacupuncture at <i>yaosanzhen</i> (bilateral Shenshu [BL23], Dachangshu [BL25], Weizhong [BL40]), disperse-dense wave was selected, with a pulse cycle of 0.08 s, current intensity of 1-3 mA, with needles heated to approximately 45 ℃, the duration was 25 min per session, once a day. The control group was given oral celecoxib capsules, once daily, 200 mg each time. Six sessions as one course, with a 1-day interval between courses, 2 courses were required in both groups. The TCM syndrome score, visual analogue scale (VAS) score, Oswestry disability index (ODI) score, and Japanese Orthopedic Association (JOA) score before and after treatment in both groups were compared. The serum levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), thromboxane B2 (TXB2) and C-reactive protein (CRP) were detected using ELISA method before and after treatment in both groups. The clinical efficacy was evaluated in both groups after treatment.</p><p><strong>Results: </strong>After treatment, the TCM syndrome scores, VAS scores, ODI scores and serum levels of TNF-α, IL-6, TXB2, CRP in both groups were reduced compared with those before treatment (<i>P</i><0.01), while the JOA scores were increased (<i>P</i><0.01);the TCM syndrome score, VAS score, ODI score and serum levels of TNF-α, IL-6, TXB2, CRP in the observation group were lower than those in the control group (<i>P</i><0.01, <i>P</i><0.05), and the JOA score was higher than that in the control group (<i>P</i><0.01). The main symptoms of this patient were involuntary and persistent twitching of muscles around the eyes, lips and jaws. The syndrome belongs to hyperactivity of liver <i>yang</i>. The treatment is dispelling wind and dredging collaterals, soothing liver and relieving spasm. Acupuncture treatment was given at bilateral Dadun (LR1), Sanjian (LI3), Shaoshang (LU11), Zulinqi (GB41), Yuyao (EX-HN4), Sibai (ST2), Jiachengjiang (Extra), Fengchi (GB20), Yifeng (TE17), once every other day. After 10 times of treatment, the twitching frequency of facial muscles decreased significantly, and basically did not twitch without emotional fluctuations; the acupuncture treatment was changed to once a week, and the consolidation treatment was 4 times. In the teaching materials of acupuncture and moxibustion, and the national standard, <i>Standardized Manipulations of</i> Acupuncture <i>and Moxibustion</i>-<i>Part 5</i>: <i>Cupping Therapy</i>, this cupping technique is operated by igniting an alcohol-soaked cotton ball held with a forceps, placing it inside the cup, taking it out after \"turning around in several circles\", and placing the cup on the selected area. Based on the clinical experience of chief physician <i>LI Yan</i>, a high-efficient and safe fire twinkling was developed. Clamping the middle part of the cotton ball with a holder, dipping it in 95% ethanol, and squeezing the cotton ball to ensure no ethanol drops left; holding the cup with the dominant hand and covering the ignited cotton ball vertically, removing the cup immediately when the ball touching the cup bottom. In the sham operation group, the spinal cord was exposed and then the incision was sutured without contusion. In the other two groups, SCI models were established using a modified Allen's impact method. On days 1, 3, 7, and 14 after modeling, the electroacupuncture group received electroacupuncture intervention at the T&lt;sub&gt;9&lt;/sub&gt;/T&lt;sub&gt;10&lt;/sub&gt; and T&lt;sub&gt;10&lt;/sub&gt;/T&lt;sub&gt;11&lt;/sub&gt; intervertebral foramen under ultrasound guidance, avoiding spinal cord injury. Stimulation parameters were dense-disperse wave at 2 Hz/100 Hz and 1-2 mA for each session. Following interventions on days 1, 3, 7, and 14, the Basso-Beattie-Bresnahan (BBB) score was assessed; the inclined plane test was used to assess hindlimb grip strength in rats. After the intervention, HE staining was used to observe spinal cord morphology; TUNEL staining was used to detect neuronal apoptosis; ELISA was used to measure the serum levels of interleukin (IL)-6, IL-1β, and tumor necrosis factor-alpha (TNF-α); Western blot was used to analyze the protein expression of Wnt-4, β-catenin, c-Myc, Bax, Bcl-2, and NeuN in spinal tissue; quantitative real-time PCR was used to detect the mRNA expression of Wnt-4, β-catenin, c-Myc, Bax, Bcl-2, and NeuN.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;Compared with the sham operation group, the model group showed significantly reduced BBB scores (&lt;i&gt;P&lt;/i&gt;&lt;0.05), and reduced inclined plane angles (&lt;i&gt;P&lt;/i&gt;&lt;0.05) at all time points. Compared with the model group, the electroacupuncture group exhibited increased BBB scores on days 3, 7, and 14 (&lt;i&gt;P&lt;/i&gt;&lt;0.05), and higher inclined plane angles on days 1, 3, 7, and 14 (&lt;i&gt;P&lt;/i&gt;&lt;0.05). Compared with the sham operation group, the model group showed disorganized spinal cord structure with increased inflammatory cells and necrotic neurons, higher number of apoptotic neurons in spinal tissue (&lt;i&gt;P&lt;/i&gt;&lt;0.05), elevated serum IL-6, IL-1β, and TNF-α levels (&lt;i&gt;P&lt;/i&gt;&lt;0.05), increased protein and mRNA expression of Wnt-4, β-catenin, c-Myc, and Bax (&lt;i&gt;P&lt;/i&gt;&lt;0.05), and decreased protein and mRNA expression of Bcl-2 and NeuN in spinal tissue (&lt;i&gt;P&lt;/i&gt;&lt;0.05).
