{"title":"[<i>Jiuci renmai</i> therapy combined with bladder function training for post-stroke neurogenic bladder: a randomized controlled trial].","authors":"Qiang Huang, Chunning Li, Hongyu Xie, Baoguo Wang, Zhenya Wang, Yi Cao","doi":"10.13703/j.0255-2930.20250302-0001","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20250302-0001","url":null,"abstract":"<p><strong>Objective: </strong>To observe the clinical effect and safety of <i>Jiuci renmai</i> therapy (moxibustion and acupuncture on the conception vessel) combined with bladder function training in treatment of post-stroke neurogenic bladder (PSNB).</p><p><strong>Methods: </strong>Sixty patients with PSNB were randomly divided into an observation group and a control group, 30 cases in each group. On the basis of conventional treatment with western medication, bladder function training was delivered in the control group, once a day for 4 weeks. In the observation group, <i>Jiuci renmai</i> therapy was supplemented besides the regimen as the control group. The main acupoints were Guanyuan (CV4), Zhongji (CV3), Qihai (CV6) and Qugu (CV2); and the supplementary acupoints were Henggu (KI11), Zhongwan (CV12), Xiawan (CV10) and Shuifen (CV9). Warm needling and moxibustion were operated, once every other day, for 4 weeks. Separately, before treatment and in 2 and 4 weeks of treatment, the urodynamic parameters were detected in the two groups, including maximal urine flow rate (Qmax), maximal detrusor pressure (PdetQmax), residual urine volume (RUV), maximal bladder capacity in the filling phase (MCC), and maximal intravesical pressure in the voiding phase (Pvesmax); the voiding parameters (the average daily number of micturition, urinary leakage episodes, and single voiding volume) were recorded; neurogenic bladder symptom score (NBSS), lower urinary tract symptom score (LUTS) and the score of quality of life scale for incontinence of urine (I-QoL) were evaluated, as well as the clinical effect and safety in the two groups.</p><p><strong>Results: </strong>In 2 and 4 weeks of treatment, Qmax, PdetQmax, MCC, Pvesmax, and average daily single voiding volume were increased compared with the levels before treatment in each group (<i>P</i><0.05), and the above indexes in the observation group were higher than those of the control group (<i>P</i><0.05). RUV, the average daily number of micturition, urinary leakage episode, NBSS and LUTS scores of the two groups were reduced in comparison with those before treatment (<i>P</i><0.05 ), and these indexes in the observation group were lower than those of the control group (<i>P</i><0.05). In 4 weeks of treatment, the average urinary leakage episode was reduced largely in comparison with the control group (<i>P</i><0.05); and the improvement in RUV for the patients with retention of urine in the observation group was superior to the control group (<i>P</i><0.05). In 4 weeks of treatment, the score of each dimension in I-QoL and the total score were elevated compared with those before treatment in the two groups (<i>P</i><0.05), and the scores in the observation were higher when compared with the control group (<i>P</i><0.05). The total effective rate in the observation group was 90.0% (27/30) which was higher than 70.0% (21/30) of the control group (<i>P</i><0.05). A systematic search of Chinese and English literature was conducted in CNKI, Wanfang, VIP, SinoMed, PubMed, EMbase, and Cochrane Library for treatment of urticaria with acupuncture and moxibustion, published up to December 31, 2023 since inception of each database. The research status in this field was summarized using an evidence mapping approach, and methodological quality was assessed. A total of 323 randomized controlled trials (RCTs) and 22 systematic reviews were included. The number of studies on acupuncture and moxibustion for urticaria has been increasing, with a significant rise in recent years. In most RCTs, the study scale was small, and the subjects focused on chronic spontaneous urticaria in adolescents and middle-aged adults, aged 14 to 60 years. Regarding the intervention measures, the single therapy of acupuncture and moxibustion was predominant such as acupoint injection, acupoint embedding thread, and filiform needling. In acupuncture with filiform needles, the commonly used acupoints were Quchi (LI11), Xuehai (SP10), Sanyinjiao (SP6), Zusanli (ST36) and Hegu (LI4). The main outcome measures referred to effectiveness rate, score of disease severity, recurrence rate, laboratory indexes, and score of quality of life; and the short-term effect was evaluated specifically. The overall methodological quality of the included studies was relatively low. {"title":"[Herbal cake-separated moxibustion at <i>Baliao</i> acupoints for erectile dysfunction with kidney deficiency and blood stasis: a randomized controlled trial].","authors":"Junge DU, Pengchao Li, Zixue Sun","doi":"10.13703/j.0255-2930.20240924-0001","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20240924-0001","url":null,"abstract":"<p><strong>Objective: </strong>To compare the clinical efficacy of herbal cake-separated moxibustion at <i>Baliao</i> acupoints combined with western medication versus western medication alone in treating erectile dysfunction (ED) with kidney deficiency and blood stasis.</p><p><strong>Methods: </strong>A total of 108 ED patients were randomly divided into a combination group (54 cases, 2 cases dropped out) and a western medication group (54 cases, 1 case was eliminated, 2 cases dropped out). The western medication group received oral tadalafil tablets 5 mg a time, once daily. The combination group received additional herbal cake-separated moxibustion at <i>Baliao</i> acupoints (Shangliao [BL31], Ciliao [BL32], Zhongliao [BL33], Xialiao [BL34]) twice weekly (administered on Tuesdays and Fridays). Both groups were treated for 4 weeks. The 5-question international index of erectile function (IIEF-5) score, erectile hardness assessment (EHS) score, TCM syndrome score, serum sex hormone levels (follicle-stimulating hormone [FSH], luteinizing hormone [LH], prolactin [PRL], testosterone [T]), penile hemodynamic parameters [peak systolic velocity (PSV) of penile cavernosal artery, end-diastolic velocity (EDV), resistance index (RI)] were compared before and after treatment between the two groups, and the safety and clinical efficacy were evaluated.</p><p><strong>Results: </strong>After treatment, the IIEF-5 scores, EHS scores, serum T levels, PSV and RI of penile cavernosal artery were increased compared with those before treatment in the two groups (<i>P</i><0.05). Except for RI, the combination group exhibited significantly higher improvements in the above indexes than those in the western medication group (<i>P</i><0.05). After treatment, the TCM syndrome scores, the serum FSH, LH levels, and EDV in the two groups and the serum PRL level in the combination group were decreased compared with those before treatment (<i>P</i><0.05), while the above indexes in the combination group were lower than those in the western medication group (<i>P</i><0.05). The total effective rate of the combination group was 88.5% (46/52), which was superior to 72.5% (37/51) in the western medication group (<i>P</i><0.05). {"title":"[Research on machine-learning quantitative evaluative model of manual acupuncture manipulation based on three-dimensional motion tracking technology].","authors":"Jiayao Wan, Binggan Wang, Tianai Huang, Fan Wang, Wenchao Tang","doi":"10.13703/j.0255-2930.20241209-0002","DOIUrl":"10.13703/j.0255-2930.20241209-0002","url":null,"abstract":"<p><strong>Objective: </strong>To develop an objective quantitative evaluative model of manual acupuncture manipulation (MAM) using three-dimensional motion tracking technology and machine learning, so as to provide a new approach to the study on acupuncture and moxibustion education and manipulation standardization.</p><p><strong>Methods: </strong>A total of 120 undergraduate students in the major of acupuncture-moxibustion and <i>tuina</i> were recruited. The Simi Motion Ver.8.5 motion tracking system was used to collect the data of three types of MAM, balanced reinforcing and reducing by twisting, reinforcing technique by twisting and reducing technique by twisting. Eight quantitative parameters covering movement performance and stability were established. With 5 types of machine learning algorithms (logistic regression, random forest, support vector machine, K-nearest neighbor, and decision tree) adopted, the evaluative model was constructed, and the feature importance analyzed.</p><p><strong>Results: </strong>In the evaluation of different types of MAM, the support vector machine presented the best for the effects of the balanced reinforcing and reducing by twisting, and the reducing by twisting (accuracy rates were both 0.88); and the logistic regression algorithm showed the optimal performance in evaluating the reinforcing by twisting (1.00 of accuracy rate). Feature importance analysis revealed that twisting velocity was the dominant parameter for evaluating the balanced reinforcing-reducing manipulation. Regarding the syndrome/pattern differentiation of TCM, a four-approach framework is established, identifying the nature of illness, analyzing the syndrome/pattern and pathogenesis, determining the stage of illness, and identifying body constitution. In treatment, the principle of treatment is determined in line with syndrome/pattern differentiation, so as to ensure the therapeutic effect by means of \"four dimensions\". Professor <i>ZHANG</i> optimizes the methods of acupoint selection and provides the comprehensive prescriptions, \"basic prescription, prescription based on disease differentiation, and supplementary prescription\". {"title":"[Inhibitory effect of electroacupuncture on microglial activation via Notch1/Hes1 pathway in Parkinson's disease mice].","authors":"Jinxu Jiang, Yang Liu, Huijie Fan, Tiansheng Zhang, Liran Wang, Lei Xu, Lixia Yang, Yunfei Song, Cungen Ma, Chongyao Hao, Zhi Chai","doi":"10.13703/j.0255-2930.20240528-0002","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20240528-0002","url":null,"abstract":"<p><strong>Objective: </strong>To observe the effects of electroacupuncture (EA) on improving motor function and regulating microglial activation based on Notch receptor 1 (Notch1)/Hes family bHLH transcription factor 1 (Hes1) pathway in mice with Parkinson's disease (PD).</p><p><strong>Methods: </strong>Thirty-six male C57BL/6 mice were randomly divided into a control group, a model group and an EA group, 12 mice in each group. PD model was established by intraperitoneal injection of 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) for 7 consecutive days in the model group and the EA group. From the 1st day of modeling, EA was applied at \"Baihui\" (GV20) and bilateral \"Shenshu\" (BL23) in the EA group, with continuous wave, in frequency of 2 Hz and current of 2 mA, 15 min a time, once a day for 14 days continuously. The behavioral performance was evaluated by gait test, pole climbing test and hanging test, the number of positive cells of tyrosine hydroxylase (TH) and the co-expression positive cells of Notch1/ionized calcium binding adaptor molecule 1 (Iba-1) in the substantia nigra of midbrain was assessed by immunofluorescence, the protein expression of TH, α-synuclein (α-syn), Notch1, Hes1, Iba-1, inducible nitric oxide synthase (iNOS), Arginase-1 (ARG1), tumor necrosis factor-α (TNF-α), interleukin (IL)-1β, IL-6 and IL-10 was detected by Western blot, the mRNA expression of Notch1 and Hes1 was detected by real-time PCR.</p><p><strong>Results: </strong>Compared with the control group, in the model group, the stride frequency was accelerated (<i>P</i><0.001) and the stride length was shortened (<i>P</i><0.001) for the four limbs, the pole climbing test time was prolonged (<i>P</i><0.01) and the grip level was reduced (<i>P</i><0.01); in the substantia nigra of midbrain, the number of positive cells of TH was decreased (<i>P</i><0.001), the number of co-expression positive cells of Notch1/Iba-1 was increased (<i>P</i><0.001), the protein expression of α-syn, Notch1, Hes1, Iba-1, iNOS, TNF-α, IL-1βand IL-6 was increased (<i>P</i><0.01, <i>P</i><0.05, <i>P</i><0.001), the protein expression of TH, ARG1 and IL-10 was decreased (<i>P</i><0.01, <i>P</i><0.001), the mRNA expression of Notch1 and Hes1 was increased (<i>P</i><0.01).
