{"title":"[Textual research on the editions of <i>Zhenjiu Bianyong Tukao</i>].","authors":"Jing Zhao, Shujian Zhang","doi":"10.13703/j.0255-2930.20240508-k0003","DOIUrl":"10.13703/j.0255-2930.20240508-k0003","url":null,"abstract":"<p><p><i>Zhenjiu Bianyong Tukao</i> (<i>Illustration on Acupuncture and Moxibustion</i>) is composed of the empirical prescriptions, dictated by <i>ZHANG Xichun</i>, the medical master of acupuncture in the Qing Dynasty, recorded and drawn in detail by <i>SU Yuanzhen</i>. There are 5 existing editions of this book preserved, and stored in library of Shanghai University of TCM, bookstore of Nanjing Ancient Classics, library of Heilongjiang Provincial Institute of TCM, library of Beijing University of CM, and China Academy of Chinese Medicine Sciences, respectively. Different from the official canonical book, this book provides a simplified and easily understandable contents, including 3 sections, named illustrations, prescriptions, and medicinal properties. In the book, the acupoints were illustrated specially rather than character portrayal, and the empirical treatment for painful symptoms was emphasized. The majority of prescriptions was intended for external application, often accompanied by ritualistic incantations. Medicinal herbs were systematically classified, and the effects were described clearly. Professor <i>LIU</i> believes that constipation in Parkinson's disease involves pathological changes in <i>sanjiao</i> (triple energizers) system, i.e. upper, middle, and lower <i>jiao</i>, and proposes <i>Tiaoshen Tongfu</i> (regulating the mind and <i>fu</i> organs) acupuncture and Chang'an <i>Liu</i>'s <i>tuina</i> manipulation are used to treat Parkinson's disease with constipation based on the \"<i>sanjiao</i> theory\". In acupuncture, the scalp points of the anterior hairline micro-acupuncture system (Touxue, Shangjiao, Zhongjiao and Xiajiao) are selected, complemented by Baihui (GV20), Hegu (LI4), Taichong (LR3), Quchi (LI11), Tianshu (ST25), Dachangshu (BL25), Shangjuxu (ST37) and Zhigou (TE6). {"title":"[Effect of perioperative transcutaneous electrical acupoint stimulation on postoperative fatigue syndrome in elderly patients].","authors":"Jing Cheng, Shiyi Hu, Yuru Fang, Guixia Cao, Tao Jiang, Yiqiao Wang","doi":"10.13703/j.0255-2930.20240814-k0004","DOIUrl":"10.13703/j.0255-2930.20240814-k0004","url":null,"abstract":"<p><strong>Objective: </strong>To observe the effect of perioperative transcutaneous electrical acupoint stimulation (TEAS) on postoperative fatigue syndrome (POFS) in elderly patients undergoing laparoscopic radical gastrectomy.</p><p><strong>Methods: </strong>A total of 80 elderly patients scheduled for laparoscopic radical gastrectomy were randomized into a TEAS group and a sham TEAS group, 40 cases in each one. In the TEAS group, TEAS intervention was applied at bilateral Hegu (LI4), Neiguan (PC6), Zusanli (ST36) and Sanyinjiao (SP6) from 30 min before anesthesia induction until surgery completion, and at 18:00 on 1st, 2nd and 3rd days after surgery, once a day, 30 min a time. In the sham TEAS group, the same acupoints were selected and connected to the electroacupuncture device at the same time, without electrical stimulation. One day before surgery and 1, 3, 7 days after surgery, the 10-item short form of identity consequence fatigue scale (ICFS-10) score was observed, and the POFS incidence rate of 1, 3, 7 days after surgery was assessed in the two groups. One day before surgery, surgery completion, and 1, 3 days after surgery, the serum levels of superoxide dismutase (SOD), β-endorphin (β-EP) were detected; 1 day before surgery and 1, 3, 7 days after surgery, the serum level of tumor necrosis factor-α (TNF-α) was detected in the two groups. The pain visual analog scale (VAS) score was observed at 24, 48 and 72 h after surgery; the intraoperative dosage of propofol and remifentanil, and the incidence rate of postoperative nausea and vomiting, itching, respiratory depression were recorded in the two groups.</p><p><strong>Results: </strong>In the TEAS group, on 1, 3, 7 days after surgery, except for the scores of item 8-10, the item scores and the total scores of ICFS-10 were lower than those in the sham TEAS group (<i>P</i><0.001); on 3 and 7 days after surgery, the POFS incidence rates were lower than those in the sham TEAS group (<i>P</i><0.05). In the TEAS group, on 1 and 3 days after surgery, the serum levels of SOD were higher than those in the sham TEAS group (<i>P</i><0.05, <i>P</i><0.01); at surgery completion, and on 1, 3 days after surgery, the serum levels of β-EP were higher than those in the sham TEAS group (<i>P</i><0.001, <i>P</i><0.01); on 1, 3, 7 days after surgery, the serum levels of TNF-α were lower than those in the sham TEAS group (<i>P</i><0.01, <i>P</i><0.001). As a result, 21 RCTs were included. The problems identified included conceptual misunderstandings regarding acupuncture direction, incomplete selection strategies, confounding research factors, and inaccuracies in reporting. Based on the findings, four strategic approaches for enhancing therapeutic efficacy through acupuncture direction were summarized: aligning needle direction with the meridian pathway, directing the needle toward the lesion site, orienting the needle toward adjacent acupoints, and targeting special anatomical structures. Two additional strategies were proposed for optimizing the procedure: simplifying acupuncture operations and directing the needle toward safe anatomical sites. Recommendations were made to improve the rationality of research factor settings and the completeness of acupuncture operation reporting. This paper systematically reviews the relevant materials on moxibustion in the <i>Fragment</i> and summarizes the findings as follows: Tibetan fire moxibustion mainly uses mugwort as the material, and terms like \"fine mugwort\", \"broad bean\" and \"sheep dung pellet\" refer to the size of the moxa cone. The number of moxa cones used is predominantly odd numbers, usually ranging from 5 to 21. The main indications for fire moxibustion cover internal medicine, external medicine, gynecology, pediatrics, and various pain syndromes. The therapy advocates for treating acute conditions and heat syndromes with moxibustion. The manuscript also records detailed contraindications, including time-based and seasonal taboos. Moxibustion is applied to both local and distal acupoints, reflecting the therapeutic concept of treating both proximal and distal regions. Furthermore, it documents simple and practical acupoint localization methods such as surface anatomical markers, proportional bone measurement, finger measurement, and hand-span measurement. Compared with contemporaneous Chinese medical moxibustion techniques, the moxibustion methods recorded in this <i>Fragment</i> are rich in content and present unique Tibetan theoretical characteristics. Based on the requirements of open two-way qualitative response sequential trial, the sample size was not set in advance, and the matched pair design was used for random division into a moxibustion group and a health education group. The patients of the two groups were treated in pairs, and the analysis was immediately performed after the end of treatment for each pair, and the sequential trial analysis (STA) was diagrammed. The health education group provided the health education for prehypertension to the patients according to <i>2018 Revised Chinese Hypertension Prevention and Treatment Guidelines</i>. In the moxibustion group, beside the health education for prehypertension, the non-suppurative moxibustion was delivered at Baihui (GV20), Guanyuan (CV4) and bilateral Neiguan (PC6) and Zusanli (ST36), once every two days, 3 treatments per week and for 4 consecutive weeks. Before treatment, in 2 and 4 weeks of treatment, as well as in 1-month follow-up after treatment, BP was measured in each group, separately. Before treatment and in 4 weeks of treatment, the observation was performed in the score of the modified Kupperman scale, and Pittsburgh sleep quality index (PSQI) score, the indexes of cardiovascular function (stroke volume [SV], cardiac output per minute [CO], stroke index [SI], cardiac index [CI], left ventricular effective pumping force [VPE], left ventricular energy efficiency [EWK], arterial compliance [AC], left ventricular ejection resistance [VER], blood viscosity (N), and microcirculation half update rate [MHR]), and heart rate variability (HRV) indexes (low frequency [LF], high frequency [HF], LF/HF, standard deviation of NN interval [SDNN], root mean square of successive differences in adjacent NN intervals [RMSSD]).</p><p><strong>Results: </strong>Based on sequential test line, when the trial was performed to the 17th pair, and the test line touched the upper limit <i>U</i>, meaning the results of the moxibustion group was superior to the health education group, thus, the trial stopped immediately. In 2 and 4 weeks of treatment and in follow-up, the systolic blood pressure (SBP) and diastolic blood pressure (DBP) were lower when compared with those before treatment in the moxibustion group (<i>P</i><0.05); SBP at each time point and DBP in 2 and 4 weeks of treatment in the moxibustion group were lower than those of the health education group (<i>P</i><0.05). The medication group was treated with levocarnitine oral solution orally, 10 mL once, 3 times a day. On the basis of the treatment in the medication group, warming acupuncture was applied at Baihui (GV20), Guanyuan (CV4) and Mingmen (GV4) in the combination group, once every other day, 3 times a week. Both groups were treated for 12 weeks. Before and after treatment, the TCM syndrome score was observed, the semen routine indexes (the sperm concentration, progressive [PR] sperm motility, PR + non-progressive [NP] sperm motility and sperm malformation rate), the serum sex hormones indexes (follicle-stimulating hormone [FSH], luteinizing hormone [LH], testosterone [T] and estradiol [E<sub>2</sub>]), as well as the IL-6 and IL-10 levels in seminal plasma were detected, and the clinical efficacy was evaluated after treatment in the two groups.</p><p><strong>Results: </strong>After treatment, except for the hyposexuality score in the medication group, the each item scores and total scores of TCM syndrome were decreased compared with those before treatment (<i>P</i><0.01, <i>P</i><0.05), the sperm malformation rates, serum FSH and LH levels, IL-6 levels in the seminal plasma were decreased compared with those before treatment (<i>P</i><0.01, <i>P</i><0.05), the PR sperm motility, PR + NP sperm motility, serum T levels, IL-10 levels in the seminal plasma were increased compared with those before treatment (<i>P</i><0.01, <i>P</i><0.05) in the two groups; the sperm concentration was increased compared with that before treatment in the combination group (<i>P</i><0.01). After treatment, compared with the medication group, except for the hyposexuality and frequent nocturia scores, the each item scores and total score of TCM syndrome were lower (<i>P</i><0.01, <i>P</i><0.05); the sperm concentration, PR sperm motility and PR + NP sperm motility, serum T level, IL-10 level in the seminal plasma were higher (<i>P</i><0.01, <i>P</i><0.05); sperm malformation rate, serum FSH and LH levels, IL-6 level in the seminal plasma were lower (<i>P</i><0.01, <i>P</i><0.05) in the combination group. {"title":"[Effect of <i>Tongdu Tiaoshen</i> acupuncture on hippocampal neuronal ferroptosis in depression rats based on SLC7A11/GPX4 pathway].","authors":"Tingting Qian, Ling Zou, Zhi Gao, Yu Wu, Yanbiao Zhao, Nan Li, Hui Liu, Meixiang Sun, Peiyang Sun","doi":"10.13703/j.0255-2930.20240917-k0001","DOIUrl":"10.13703/j.0255-2930.20240917-k0001","url":null,"abstract":"<p><strong>Objective: </strong>To observe the effects of <i>Tongdu Tiaoshen</i> acupuncture (acupuncture for unblocking the obstruction in the governor vessel and regulating the spirit) on the depression-like behavior and the hippocampal neuronal ferroptosis mediated by solute carrier family 7 member 11 (SLC7A11)/glutathione peroxidase 4 (GPX4) pathway in depression rats, and explore the mechanism of this therapy for depression.</p><p><strong>Methods: </strong>Of 30 male SD rats of SPF grade, 24 rats were selected. According to the random number table, they were divided into a normal group (<i>n</i>=8) and a modeling group (<i>n</i>=16). The rats in the modeling group were subjected to chronic unpredictable mild stress (CUMS) for 28 consecutive days to establish depression model. After modeling, 16 successfully-modeled rats were randomly divided into a model group and an acupuncture group, 8 rats in each one. In the acupuncture group, <i>Tongdu Tiaoshen</i> acupuncture was applied to \"Dazhui\"(GV14), \"Shuigou\" (GV26), \"Baihui\" (GV20) and \"Shenting\" (GV24). This intervention measure was deliveredonce a day, continuously for 6 days. The intervention discontinued on day 7, and was completed in 4 weeks. Before and after modeling, and after intervention completion, the behavioristics detection was performed using sucrose preference experiment and open field experiment. After intervention, using hematoxylin-eosin (HE) and Nissl staining, the morphology of hippocampal neurons was observed; with Western blot method, the protein expression of GPX4, SLC7A11, Ferritin and acyl-CoA synthetase long-chain family 4 (ACSL4) in hippocampal tissues was detected; with the real-time fluorescence quantitative PCR adopted, the mRNA expression of GPX4, SLC7A11, Ferritin and ACSL4 was detected; and using colorimetry, the hippocampal iron content was determined.</p><p><strong>Results: </strong>After modeling, the sucrose preference rates, the total distance of movement, the standing times and the boxes of horizontal crossing in the model group and the acupuncture group were lower than those in the normal group (<i>P</i><0.01). After the intervention, the sucrose preference rates, the total distance of movement, the standing times and the boxes of horizontal crossing in the acupuncture group were higher than those in the model group (<i>P</i><0.01, <i>P</i><0.05). Compared with the normal group, the number of necrotic cells increased and the number of Nissl bodies decreased in the model group; and when compared with the model group, the neuronal pyknosis and necrosis were ameliorated, the cells were arranged more regularly, the neuronal structure was clear, the matrix was dense, the blood vessels were enriched and the number of Nissl bodies increased in the acupuncture group. Regarding the pathogenesis of this disease, Professor <i>WANG Jihong</i> emphasizes a \"trinity\" approach, \"the mind disturbed, <i>qi</i> and blood unbalanced, and <i>yuan</i> (vital essence) not consolidated\". In treatment, \"the unblocking, conducting and nourishing are taken as the fundamental\" to \"prevent from over-dredging, balancing <i>yin</i> and <i>yang</i>, and avoiding over-replenishing\". In clinical practice, the combined measure is adopted with acupuncture, <i>tuina</i> and herbal medicine. In acupuncture, the acupoints are selected according to the front-back combination of the points of the conception vessel and the governor vessel (basic prescription: Yintang [GV24<sup>+</sup>], Baihui [GV20] and Niesanzhen; prescription for promoting the governor vessel: Dazhui (GV14), Jinsuo [GV8], Xinshu [BL15], Pishu [BL20] and etc.; and the prescription for consolidating the vital essence: Danzhong (CV17), Zhongwan [CV12], Tianshu [ST25], Qihai [CV6]) and etc., as well as those on the four limbs for \"opening, closing and pivoting of <i>yin</i> and <i>yang</i>\" (on the upper limbs of the affected side: Jiquan [HT1], Jianyu [LI15], Quchi [LI11], Quze [PC3] and etc.; on the lower limbs of the affected side: Huantiao [GB30], Liangqiu [ST34], Xuehai [SP10], Yanglingquan [GB34]) and etc. The above points co-act on regulating the conception vessel and the governor vessel, and balancing <i>yin</i> and <i>yang</i>. <i>Tuina</i> is delivered on the governor vessel, the bladder meridian of foot-<i>taiyang</i> and wind points on the area between the head and nape, so as to promote the governor vessel, benefit the orifices, strengthen the spine and relax tendons. {"title":"[Chicken-claw needling with blade needle for 30 cases of postherpetic neuralgia].","authors":"Yibin Wu, Yingying Zhang, Weiqi Feng","doi":"10.13703/j.0255-2930.20241020-0001","DOIUrl":"10.13703/j.0255-2930.20241020-0001","url":null,"abstract":"<p><strong>Objective: </strong>To observe the clinical efficacy of chicken-claw needling with blade needle for postherpetic neuralgia (PHN).</p><p><strong>Methods: </strong>A total of 30 patients with PHN were treated with chicken-claw needling with blade needle, the most painful area, costal angle on the same side with the affected intercostal nerve and the midpoint of the line connecting the first two points along the intercostal nerve were selected as treatment points, without needle retained, once a week, a total of 2 times were required. Before and after treatment, the scores of pain visual analogue scale (VAS) and dermatology life quality index (DLQI) were observed, and the clinical efficacy was evaluated after treatment. The recurrence rate of neuralgia was evaluated on the 60th day after treatment.</p><p><strong>Results: </strong>After treatment, the scores of VAS and DLQI were decreased compared with those before treatment (<i>P</i><0.05). Of 30 patients, 4 cases were cured, 20 cases showed markedly efficacy, 4 cases were improved, 2 cases were ineffective, the total effective rate was 93.3% (28/30).
