Ten adults of both sexes, between 23 and 54 years old, who had lost a family member as a result of COVID-19, during the pandemic period, participated in the study. A semi-structured interview was administered to them.</div></div><div><h3>Results</h3><div>The results showed that adults experiencing bereavement due to the loss of a family member in the pandemic scenario perceive their grief at different times. First, there is the impossibility of seeing their loved one due to the biosecurity measures that prevent them from being with their relative, which entails a moment of shock and disbelief; then there is the release of repressed emotions, where dysfunctional thoughts and emotions occur. Later, there is the negotiation of the emotional burden, a period in which there is a reconciliation with the unbridled emotions; followed by the feeling of hopelessness, in which the person experiences deep sadness and lack of meaning in life. This study aimed at evaluating the effectiveness of paliperidone palmitate once monthly (PP1M) on treatment adherence and relapse in the schizophrenia population followed in a government-funded mental health care facility in Colombia.</div></div><div><h3>Methods</h3><div>A mirror-image study was conducted. Adult schizophrenia patients treated with oral antipsychotics who subsequently received ≥2 PP1M injections between Jan. 1<sup>st</sup>, 2015 and Oct. 31<sup>st</sup>, 2018 were included. The study consisted of two retrospective phases: 12 months before and after the first PP1M injection. Outcomes were treatment adherence (proportion of days covered ≥80%), hospitalized relapse, hospital length of stay, and non-hospitalised relapse. Effect of PP1M on outcomes was assessed through multivariable conditional Poisson regression.</div></div><div><h3>Results</h3><div>123 patients were eligible (mean age, 30.3 years; 79.7% males). Adherence was 23.6% in the pre-phase and 89.4% in the post-phase (RR<!--> <!-->=<!--> <!-->3.77; 95%CI, 2.75-5.17). The proportion of patients with hospitalised relapse decreased from 46.3% to 35.0% (RR<!--> <!-->=<!--> <!-->0.76; 95%CI, 0.59-0.99). In the 75 (61.0%) patients who continued PP1M throughout post-phase, beneficial effect on hospitalised relapse was stronger (RR<!--> <!-->=<!--> <!-->0.64; 95%CI, 0.42-0.98). The proportion of patients with non-hospitalised relapse symptoms increased from 6.5% to 18.7% (RR<!--> <!-->=<!--> <!-->2.27; 95%CI, 1.11-4.64).</div></div><div><h3>Conclusions</h3><div>PP1M initiation led to a dramatic improvement in treatment adherence and a decrease in hospitalised relapse. Observed increase in non-hospitalised relapse may be explained by a decrease in severity. Limitations are absence of a parallel comparison group and a generalisability limited to the population treated at this facility. {"title":"Características demográficas, clínicas y del uso de recursos hospitalarios de los pacientes pediátricos con trastorno de síntomas somáticos y trastornos relacionados, hospitalizados en un centro de referencia de Medellín","authors":"Diana Judith Beltrán , Juan Pablo Zapata-Ospina , Paula Andrea Duque","doi":"10.1016/j.rcp.2022.09.005","DOIUrl":"10.1016/j.rcp.2022.09.005","url":null,"abstract":"<div><h3>Background</h3><div>Somatic symptom and related disorders (SSRD) constitute a clinical challenge in pediatric psychiatry due to the difficulties in their conceptualization, the complexity of their clinical presentation, which also depends on the cultural context, repeated consultations to health services and the predominance of studies in adults.</div></div><div><h3>Objective</h3><div>To describe the characteristics of the pediatric inpatients with SSRD in a reference hospital.</div></div><div><h3>Design and methods</h3><div>Descriptive cross-sectional study of demographic, clinical variables, and the use of institutional resources, based on the review of medical records of hospitalized patients under 18 years of age with a diagnosis of SSRD evaluated by psychiatry or pediatric psychiatry between 2018 and 2021 at reference pediatric hospital in Medellín, Colombia.</div></div><div><h3>Results</h3><div>The clinical records of 83 patients were included. Most patients were female (72%), with a median age of 12 years, anxiety and depression were the most frequent comorbidities (83% and 75%) and also the most frequent disorders in mothers (25% and 26%). These stigmatising depictions contribute to the development and persistence of negative attitudes towards people with psychiatric conditions. The objective was to examine, through popular music, the subcultural representations of crime and violence in the context of mental disorders, focusing on depictions of victims and offenders.</div></div><div><h3>Methods</h3><div>Strategy of analysis: Content analysis of Spanish punk lyrics (1981-2010) with references to violent and criminal behaviour associated with mental disorders.</div></div><div><h3>Results</h3><div><span>257 Spanish punk bands were identified. The discographies included 7,777 songs, of which 190 were related to aggression, violence, or crime. A predilection for violent crimes and descriptions of the perpetrator as “mentally disturbed” was observed. Although they were present, psychotic </span>symptoms were not the main psychiatric symptoms associated with violent crime, but instead it was substance use, antisocial personality traits and paraphilic behaviour. There was less attention paid to victims than to perpetrators.</div></div><div><h3>Conclusions</h3><div>The relationships between mental disorders and criminality/violence are overemphasised in the analysed subculture. We analyzed 50 medical records of patients who presented delirium and 50 who did not present delirium in a COVID hospitalization area during the period from March 2020 to March 2021. The odd ratio of sociodemographic and clinical factors for presenting delirium was evaluated. The logistic regression statistical test was performed using STATA 17.0 software. The protocol was approved by the Research Ethics Committee of the Villa El Salvador Emergency Hospital.</div></div><div><h3>Results</h3><div>Ventilatory support (<em>P</em>=.04), especially the use of a reservoir mask (OR: .34; 95%CI: 0.12–0.88) and the presence of systemic complications (<em>P</em>=.03) such as metabolic acidosis (OR: 12.9; 95%CI: 1.45-115.58) were statistically significant variables associated with a higher risk of presenting delirium.</div></div><div><h3>Conclusions</h3><div>The type of ventilatory support and the presence of systemic complications were significant risk factors for delirium in patients hospitalized in a COVID area.
