L’écoconception des soins est un soin qui à qualité, sécurité et pertinence égales, est moins impactant pour l’environnement, c’est donc un levier majeur pour une pratique médicale durable. Cet article explore l’application de l’écoconception des soins en anatomie et cytologie pathologiques (ACP) en France, en réponse aux objectifs de décarbonation du pays.&lt;/p&gt;&lt;/div&gt;&lt;div&gt;&lt;h3&gt;Objectifs&lt;/h3&gt;&lt;p&gt;Après avoir montré que la décarbonation est possible grâce à l’écoconception choisie des soins et pratiques en ACP, nous avons décrit les freins à ces changements, puis les solutions possibles en « vie réelle ».&lt;/p&gt;&lt;/div&gt;&lt;div&gt;&lt;h3&gt;Discussion&lt;/h3&gt;&lt;p&gt;Nous examinons les défis et les solutions pour intégrer les principes de l’écoconception des soins dans la pratique quotidienne de l’ACP, en mettant en évidence l’importance de la pertinence des actes médicaux pour réduire les pratiques inutiles. Nous discutons des freins techniques et humains en ACP, mais aussi des solutions : la sensibilisation des acteurs du laboratoire, des industriels, la recherche et l’innovation, l’implication des sociétés savantes ou encore les initiatives du collectif pour la Transformation écologique en ACP (TEAP). Enfin, nous proposons des leviers financiers pour rendre la sobriété économiquement viable en ACP.&lt;/p&gt;&lt;/div&gt;&lt;div&gt;&lt;h3&gt;Conclusion&lt;/h3&gt;&lt;p&gt;L’écoconception des pratiques en ACP est essentielle pour répondre au défi climatique et assurer la durabilité du système de santé.&lt;/p&gt;&lt;/div&gt;&lt;div&gt;&lt;h3&gt;Introduction&lt;/h3&gt;&lt;p&gt;The healthcare sector is a major contributor to greenhouse gas emissions, accounting for 8 % of annual French emissions. Eco-design in healthcare, which provides care with equal quality, safety, and relevance but with a lower environmental impact, is therefore a crucial lever for sustainable medical practice. This article explores the application of eco-design in anatomical and cytopathological practices (ACP) in France, in response to the country's decarbonization goals.&lt;/p&gt;&lt;/div&gt;&lt;div&gt;&lt;h3&gt;Objectives&lt;/h3&gt;&lt;p&gt;After demonstrating that decarbonization is possible through the chosen eco-design of care and practices in ACP, we describe the barriers to these changes and the potential real-world solutions.&lt;/p&gt;&lt;/div&gt;&lt;div&gt;&lt;h3&gt;Discussion&lt;/h3&gt;&lt;p&gt;We examine the challenges and solutions for integrating eco-design principles into daily ACP practice, highlighting the importance of the relevance of medical procedures to reduce unnecessary practices. We discuss the technical and human barriers in ACP, as well as the solutions: raising awareness among laboratory personnel, industrial stakeholders, research and innovation, the involvement of scientific societies, and initiatives from the collective for Ecological Transformation in ACP (TEAP). L’examen microscopique de l’exérèse ganglionnaire sus-claviculaire retrouvait une infiltration du parenchyme ganglionnaire par une prolifération tumorale indifférenciée. Cette prolifération tumorale exprimait fortement les cytokératines et présentait une perte d’expression de INI1/SMARCB1. Devant cette localisation ganglionnaire d’un carcinome indifférencié INI1-déficient, dans un contexte de lésion rénale chez ce jeune patient avec traits drépanocytaires, le diagnostic retenu a été celui de métastase ganglionnaire d’un carcinome médullaire SMARCB1-déficient du rein (OMS 2022).</p></div><div><p>We report the case of a 14 year-old teenager who has SC hemoglobinosis and presented with a tumor syndrome with a retro-peritoneal mass, a supraclavicular lymph node and a mid-renal lesion. The microscopic examination revealed an undifferentiated tumor proliferation infiltrating the lymph node parenchyma. This tumor proliferation was INI1/SMARCB1-deficient, and expressed cytokeratins. C’est une lésion rare et peu rapportée dans la littérature. Nous rapportons le cas d’une femme âgée de 33<!--> <!-->ans présentant des douleurs lancinantes de la première phalange du deuxième doigt de la main droite, associées à un placard inflammatoire en regard. L’examen histopathologique de la biopsie exérèse chirurgicale de la lésion révélait une prolifération tumorale fusiforme au sein d’un stroma scléro-hyalin et ostéoïde. Dans notre observation, l’apport de l’immunohistochimie et de la biologie moléculaire constitue un élément clé pour poser le diagnostic et éliminer les différents diagnostics différentiels, surtout devant un aspect histopathologique peu spécifique.</p></div><div><p>Fibro-osseous pseudotumor of the digits is a benign tumour closely related to myositis ossificans. It is a rare lesion seldom reported in the literature. We report the case of a 33-year-old woman with lancinating pain in the first phalanx of the second finger of the right hand, associated with inflammation. The histopathological examination of the surgical excision biopsy of the lesion revealed a spindle-shaped proliferation within a sclerosing, hyaline, and osteoid stroma. {"title":"Localisations digestives gastriques et coliques de myélomes ; étude de 3 cas et revue de la littérature","authors":"","doi":"10.1016/j.annpat.2024.06.007","DOIUrl":"10.1016/j.annpat.2024.06.007","url":null,"abstract":"<div><p>Le myélome multiple correspond à une prolifération plasmocytaire maligne de localisation classiquement médullaire et osseuse. Il peut se manifester secondairement par des atteintes extra-osseuses, mais les atteintes digestives sont rares. Nous rapportons 3 cas de localisations gastrique et coliques de myélome, diagnostiquées chez deux hommes et une femme, âgés de 66, 71 et 77 ans. Les atteintes digestives étaient diagnostiquées 1 à 7 ans après la découverte du myélome. Les patients consultaient pour des épigastralgies, des rectorragies ou un syndrome occlusif. L’endoscopie retrouvait au niveau gastrique des formations tumorales ulcérobourgeonnantes, et au niveau colique des formations tumorales nodulaires pseudopolypoïdes et une plage érythémateuse ulcérée. L’analyse anatomopathologique des biopsies retrouvait, au sein du chorion des muqueuses digestives, une prolifération tumorale d’architecture diffuse, composée de cellules plasmocytoïdes ou plasmablastiques, exprimant à l’étude immunohistochimique les marqueurs plasmocytaires. Les 3 patients décédaient dans les semaines suivant le diagnostic. Le pronostic des localisations digestives de myélome reste très mauvais malgré les nouvelles thérapies. Devant tout symptôme digestif chez les patients porteurs d’un myélome, une endoscopie plus systématique pourrait peut-être permettre un diagnostic plus précoce et la mise en œuvre de thérapies plus efficientes.</p></div><div><p>Multiple myeloma is a malignant plasma cell proliferation located in the bone marrow and bones. It can secondarily manifest with extraosseous involvement, but the gastro-intestinal tract locations are rare. We report 3 cases of gastric and colonic localizations of myeloma in two males and one female, aged 66, 71 and 77<!--> <!-->years. Multiple myeloma had been diagnosed 1 to 7<!--> <!-->years before. Digestive symptoms were epigastric pain, rectal bleeding or an obstructive syndrome. Endoscopy revealed ulcerated and budding tumors in the stomach, and nodular pseudo-polypoid tumor formations or an ulcerated erythematous area in the colon. Histopathological examination of the biopsies showed a diffuse tumor cell proliferation in the lamina propria composed of cells with a plasmacytoid or plasmablastic appearance, expressing plasma cell markers such as CD138 on immunohistochemistry. The 3 patients died in the weeks following the diagnosis. The prognosis of digestive localizations of multiple myeloma remains very poor despite new therapies. {"title":"Impacts environnementaux du numérique et de l’intelligence artificielle, à l’heure de la pathologie digitale","authors":"","doi":"10.1016/j.annpat.2024.05.006","DOIUrl":"10.1016/j.annpat.2024.05.006","url":null,"abstract":"&lt;div&gt;&lt;p&gt;Si la numérisation et l’intelligence artificielle représentent le futur de notre spécialité, l’avenir est également contraint par le réchauffement climatique et le dépassement des limites planétaires, menaçant la santé humaine et le fonctionnement même du système de santé. Le rapport de la Délégation ministérielle du numérique en santé et la planification écologique du système de santé du gouvernement français confirment la nécessité de maîtriser l’impact environnemental du numérique en santé. En effet, en dépit des promesses de dématérialisation, le numérique est une industrie très matérielle, engendrant émissions de gaz à effets de serre, consommation problématique de ressources en eau et minerais et impacts sociaux. Le numérique est impactant de par les phases : (i) de fabrication du matériel ; (ii) des usages ; et (iii) de la fin de vie du matériel, qui lorsqu’il est recyclé, ne peut l’être que de façon très partielle. Il s’agit d’un secteur en pleine croissance, et la numérisation de notre spécialité prend part à cette accélération et à ses impacts. La compréhension des conséquences du numérique et de l’intelligence artificielle, et des phénomènes tels que l’effet rebond, est un préalable indispensable à la mise en œuvre d’une pathologie numérique sobre, responsable et durable. Cette mise au point a pour objectif d’aider les pathologistes à mieux appréhender les impacts environnementaux du numérique. Nous, professionnels de santé, avons cette responsabilité d’allier avancées technologiques et prise en compte de leurs impacts, dans une vision systémique de la santé humaine.&lt;/p&gt;&lt;/div&gt;&lt;div&gt;&lt;p&gt;While digitization and artificial intelligence represent the future of our specialty, future is also constrained by global warming and overstepping of planetary limits, threatening human health and the functioning of the healthcare system. The report by the Délégation ministérielle du numérique en santé and the French government's ecological planning of the healthcare system confirm the need to control the environmental impact of digital technology. Indeed, despite the promises of dematerialization, digital technology is a very material industry, generating greenhouse gas emissions, problematic consumption of water and mineral resources, and social impacts. The digital sector is impacting at every stage: (i) manufacture of equipment; (ii) use; and (iii) end-of-life of equipment, which, when recycled, can only be recycled to a very limited extent. This is a fast-growing sector, and the digitization of our specialty is part of its acceleration and its impact. Understanding the consequences of digitalization and artificial intelligence, and phenomena such as the rebound effect, is an essential prerequisite for the implementation of a sober, responsible, and sustainable digital pathology.
