Unfortunately, many organizations have vacant CNS positions due to a nationwide CNS shortage. The CNS Academy is a novel internship program launched at a large health care system to recruit nurses to pursue CNS education while providing hands-on experience as a CNS intern. This “grow your own” approach provides on-the-job opportunities to gain CNS competencies and ease role transition upon graduation. Using a qualitative descriptive design, we conducted surveys to assess nurse manager perceptions of existing staffing tools, access to tools, and knowledge of their department’s unit of service, labor standard, and budgeted nonproductive time. The purpose was to determine the specific gaps in nurse manager knowledge related to determining staffing needs. The results also support the important influence FNMs have on DCNs and the health of the work unit. Substantial evidence demonstrates that nurse residency programs improve nurse outcomes such as retention, satisfaction, and competence; however, little is known or studied about the impact on patient outcomes related to the quality and safety of nursing care. Safe staffing in an era of nurse shortages must be achieved by a radical change to the current models of care. Clinical organizations must innovate by implementing and testing new models of care while partnering with their local academic colleagues to develop a new body of evidence. There is a need to educate all stakeholders, including legislators, that mandated nurse-to-patient ratios are not the best solution to ensure that patients have access to excellent care and that nurses are able to practice in a professionally fulfilling environment. Of paramount importance to planning a preferred future for both those who receive and deliver care will be the engagement of nurses, advanced practitioners, educators, and researchers.
