In Ghana, there is paltry of information on the perspectives of young adults on cervical cancer. Aim: The study explored the perception of young adults between the ages of 17-24 on cervical cancer screening at a hospital in the Sekondi-Takoradi Metropolis of the western region of Ghana. Methods: Qualitative exploratory descriptive method was employed in this study. 13 participants who met the inclusion criteria were purposively selected for the study. Respondents were interviewed face-to-face using a semi structured interview guide designed based on the research objectives. All interviews were audio taped, transcribed verbatim and analysed using thematic content analysis. Results: Younger adults had adequate knowledge on cervical cancer but inadequate information on its screening methods. All young adults in the study had never done screening because they did not see themselves susceptible to the development of cervical cancer and some opined that, cervical cancer is a disease for older and pregnant women. Methods: A descriptive qualitative study was conducted on cancer patients who experience nausea and vomiting due to chemotherapy. Semi-structured interview guide consisting of eight questions was used as a study tool and thematic analysis was done using QDA Miner lite software.Results: Twenty-five participants were included in the final analysis. The median age of the participants was 53 (IQR-15) years and most of the patients were Hindu. All the patients were newly diagnosed. Approximately 80% of patients had received three cycles of chemotherapy by the time of their interviews. Adriamycin, Cyclophosphamide (AC), Docetaxel (T), Paclitaxel and Carboplatin (P+C) were the chemotherapy drugs administered. Our findings suggest that chemotherapy-induced nausea and vomiting has affected many aspects of patient’s lives, especially those related to eating, physical, emotional, and social functioning, but the degree of impact was unique to each patient. Conclusion: The individual experiences of nausea and vomiting during chemotherapy treatment may have a profound effect on treatment compliance and future outcomes. Imatinib therapy resistance in CML patients is important clinical issue. To understand association of kinase domain mutation and promoter hypermethylation of genes with imatinib therapy resistance hold significance in CML patients of North-East India. This study is a hospital based cross sectional study. Methods: A total of Sixty three (n=63) CML patients undergone imatinib mesylate were enrolled for the study. ABL kinase domain T315I mutation was analyzed by Allele specific- PCR (AS-PCR) and confirmed by sequencing. Promoter hypermethylation was analyzed by Methylation specific PCR (MS-PCR). The Chi square test and Fisher exact test used in SPSS ver19. Results: Tyrosine Kinase domain mutation T315I was found in 30.1% (19/63). The promoter hypermethylation of Calcitonin, ATG16L2, TFAP2A, EBF2 gene was detected in 42.9% (n=27), 28.6% (n=18), 38.1% (n=24), 27% (n= 17) CML patients respectively. Median relapse free survival was 21 months and statistically significant for CML patients without T315I mutation compared to patients with T315I mutation who has 12 months median relapse free survival (p=0.005). Median survival was 19 months for patients without EBF2 promoter hypermethylation and also statistically significant compared to CML patients with promoter hypermethylation (12 months) (p=0.026). The covid-19 pandemic had significant ill effects on the health care systems, worldwide. Objective: To retrospectively investigate the management of OPMD patients presented to the oral medicine clinic of the University Dental Teaching Hospital Peradeniya, Sri Lanka before and during the pandemic. Methods: This was a hospital-based cross-sectional study using medical records. The pre-pandemic group included patients diagnosed with OPMD presenting between Jan 2018 - Dec 2019; the pandemic group from Jan 2020 to Dec 2021. Results: There were 321 eligible medical records. Of these, 247 belong to the pre-pandemic group and 74 in the pandemic (3:1). In the pre-pandemic group, 32% of biopsies were performed within the first week, while it was 56% in the pandemic group. We found a significant increase in the loss of follow-up in the pandemic group compared to the pre-pandemic group (p=0.01). The average delay for the 2nd visit was 27 days, for the 3rd visit it was 127 days in the pre-pandemic; and 89 and 152 days respectively in the pandemic group.Conclusions: We recommend following evidence-based practices for the management of OPMD in anticipated pandemics. Strategies to diagnose patients at home must be developed and implemented. There is a higher need for the prevention of habit-related risk factors. Biopsy at the first visit or first week is recommended. Symptoms are nonspecific and may be mistaken for other more benign conditions as otitis, cholesteatoma and polyp. Hence, this condition may present as a diagnostic challenge. Case Discussion: We describe a case series of three patients who presented with external auditory canal (EAC) mass. The most common signs and symptoms are pruritus, decreased hearing and yellowish discharge. They were initially treated with otic antibiotics which afforded no relief. The first case is a 51 year-old female who underwent wide excision of the right ear canal and middle ear mass and right temporal craniotomy. Histopathology showed moderately differentiated squamous cell carcinoma with temporal tumor and subdural extension consistent with metastasis. The second case is a 53 year-old male who refused surgical management. He was non-compliant with work-up and follow-up and later on presented with left facial asymmetry and headache. CT scan showed a left inner and middle ear canal mass with extension to the auditory canal and protrusion into the left cerebellar hemisphere. Punch biopsy of the left EAC mass was done and pathologic report showed well-differentiated squamous cell carcinoma. Lastly, a 54 year old male who underwent Radical mastoidectomy with histopathology reports of a moderately differentiated squamous cell carcinoma. Conclusion: Standard treatment modality is still unclear because of the rarity of this condition. However, surgical resection with negative margins followed by concurrent chemotherapy and radiation is the most commonly performed approach1. Currently, partial cystectomy has experienced a resurgence as a less morbid and oncologically effective treatment. Here, we appropriately diagnosed and surgically treated 5 patients with partial cystectomy without frozen section and reviewed the outcome of this procedure. Case presentation: The diagnosis was made based on the history of illness, physical examination, CT-Scan, and confirmed by histopathological examination. A partial cystectomy was performed in all cases, which include laparoscopic partial cystectomy. Based on histological examination, we found that 4 cases in this study were adenocarcinoma bladder and 1 case was urothelial carcinoma cells with sarcomatoid variant. Our surgical technique started with doing a cystoscopy first. We performed excision of the tumor with lateral margin 1cm around the tumor. After resection of the tumor, we closed the bladder and evaluated the capacity of the bladder, which was 200cc minimal capacity. Then we closed the bladder with the double layer technique and closed the muscle, fascia, and skin, respectively. Follow-up was performed 6 and 12 months after surgery to observe progression or recurrence of the tumor. There was no postoperative complication in our case. All of our cases had no complaints and no sign of progression, recurrence, and metastatic sign. Conclusion: A frozen section has not been proven to be mandatory in partial cystectomy and the results had no significant effect in surgical margins. Majority of oral cancer cases reported in Pakistan are metastatic to Head and Neck and are with morphology of squamous cell carcinoma. Locally advance, un-resectable tumors are treated with systemic therapies. Commonly prescribe systemic therapies includes chemotherapy and targeted therapy. However, chemotherapy is the most prescribed systemic therapy because of advance presentation of cases with the morphology of Squamous cell carcinoma. Cisplatin and Carboplatin are drug of choice in HNC treatment. Chemotherapy treatment result in many toxicities such as nausea-vomiting, nephrotoxicity, cystitis, diarrhea, mucositis, ototoxicity, myelosuppresion, allergic reaction and others. Dy","doi":"10.31557/apjcc.2023.8.1.3-9","DOIUrl":"https://doi.org/10.31557/apjcc.2023.8.1.3-9","url":null,"abstract":"Background: Neoadjuvant therapy has given important prognostic and predictive information based on pathologic response as seen on studies showing that pathologic complete response (pCR) is associated with favorable disease-free and overall survival in some molecular subtypes. The main objective is to determine the pCR rate among patients with locally advanced breast cancer (LABC) in our institution. Method: A total of 259 patients with LABC who received neoadjuvant therapy and underwent definitive surgery at St. Luke’s Medical Center Quezon City from 2007 to 2017 were retrospectively analyzed through chart review. Results: Patients’ median age at diagnosis was 51 years old. 57% were premenopausal and 42% were postmenopausal. The most common histology was Invasive Ductal at 92%. The most common subtype was Luminal/HER2 negative (52%). Majority of the patients have Stage III disease. The most common chemotherapy regimen used was sequential Anthracycline and Taxane (AC/EC/FEC then Taxane with or without Trastuzumab and Docetaxel with or without Trastuzumab followed by FEC) at 31%, Anthracycline-based (Doxorubicin or Epirubicin with Cyclophosphamide/5FU -EC/ AC/FAC) at 28%, and concurrent Anthracycline-based and Taxane (TAC/AT/ET) at 27%. Of the 95 patients with Her 2 positive disease, 21 (22%) received anti-Her2 blockade as part of their neoadjuvant regimen. The pCR, defined as absence of invasive residual cancer in the breast and axillary lymph nodes, with or without ductal carcinoma-in-situ (DCIS) was seen in 46 patients (18%). Among the patients who achieved pCR, the most common subtype is Luminal/HER2 negative at 17% and the most common regimen used was Docetaxel followed by FEC (5FU+EC) at39%. Conclusion: Among 259 patients, pCR is achieved in 18% of patients using standard chemotherapy. It has a great impact on global health. Although, current treatment modalities improve patients` survival rates, survivors suffer from long term treatment related morbidity. Major advances have changed molecular understanding of medulloblastoma with the emergence of the molecular classification of medulloblastoma that has been introduced in WHO classification of CNS tumors. Non-WNT/non-SHH molecular subgroup is the most common molecular subgroup representing more than 60% of medulloblastoma cases. The present study aims to describe the clinical, pathological, and survival characteristics of pediatric patients with non-WNT/non-SHH MB. Results: A total of 36 non-WNT/non-SHH MB cases were detected. The age of the patients ranged between 2 to 18 years. 14 patients (39%) were standard risk while 22 patients (61%) were high risk. Microscopic evaluation showed that 34 cases (94.4%) were of classic histology, while 2 cases (5.6%) were of LC/A histology. The 5-year overall survival of the 36 non-WNT/non-SHH cases detected was 55% and the 5-year event free survival was 40%. It can be asymptomatic or present with abdominal pain, and surgical resection is the only curative treatment. Methods: A retrospective study was conducted on patients with biopsy proven Solid Pseudopapillary Neoplasm (SPN) of the pancreas between January 2016 and June 2021. The study included 14 cases, including 7 resection specimens and 7 biopsies, and was conducted at a tertiary referral center in PakistanResults: All 14 patients were female and ranged in age from 15 to 48 years old, with a mean age of 24. The most common symptom was abdominal pain, and the head of the pancreas was the most common location of the tumor. Tumor size ranged from 5 cm to 18 cm, and one patient had lymph node involvement and two patients had hepatic metastasis. Most patients received surgical treatment, and follow-up information was available for 8 out of the 14 patients, who were all alive with no recurrence or metastasis. Conclusion: Due to its rarity, non-specific clinical presentation, and imaging findings, it can be challenging to diagnose.
