Nodal irradiation, including axillary, supraclavicular nodes, and internal mammary chain, is as important as the primary tumor. There is a broad consensus on the need for breast volume irradiation after conservative surgery and chest wall after mastectomy. However, regarding the nodal irradiation, there has been a continuing controversy for some years, which has increased since the abandonment of axillary lymphadenectomy in cases of low burden disease at this level. There are several studies suggesting the need or not for nodal irradiation, although many of them have the problem of not describing the received radiation dose at these levels, and so decision-making is complex. Once it has been decided that the patient is a candidate for irradiation, the second question refers to the lymph node volumes that must be treated. Finally it is necessary to determine which therapeutic scheme must be administered. In these two points, despite the existence of a higher level of evidence as well there is also controversy. Radiotherapy in breast cancer has shown benefits at all stages and in all age groups, with minimal morbidity and excellent tolerance. Radiotherapy treatment has evolved, the volume of treatment has been reduced in low-risk subgroups (partial breast irradiation) and hypofractionated treatments, standard today, have demostrated disease control rates comparable to previous ones with a clear improvement in the quality of life of the patients. Endocrine therapy (ET) in breast cancer impacts local and distant recurrence, breast cancer-specific mortality and overall survival, but it associates significant side effects in the short, medium and long term and there are many women who have difficulty to tolerating this treatment. In this review, we try to answer questions such as: Is the omission of radiotherapy an option? And the omission of ET? Tumor resection surgery constitutes the main treatment, but paradoxically, the surgical manipulation and perioperative </span>immunosuppression may predispose to </span>cancer dissemination. Locoregional anesthetic techniques would avoid this immunosuppression, thus improving the oncologic outcomes of surgery. This study aimed to evaluate the prognostic influence of locoregional anesthesia on breast cancer dissemination and recurrence after surgery.</p></div><div><h3>Methods</h3><p><span>A retrospective cohort study was performed on 165 centrolobulillar breast cancer patients, scheduled for non-reconstructive breast oncologic surgery between 2012 and 2015. These patients were treated with conservative surgery under </span>general anesthesia (control group, n = 81) or combined anesthesia with a locoregional block (n = 84). Data were collected on age, tumor type (size, stage, lymph node infiltration), immunohistochemical factors (hormone receptors), procedure (duration, technique), anesthesia (general anesthesia or associated with regional blockade), complications, survival, and recurrence.</p></div><div><h3>Results</h3><p>Statistical analysis demonstrated no significative differences in age, weight, sex, ASA<span> status, and surgical technique and duration. Tumor recurrence was recorded in 6 patients (4 in the general group and 2 in the locoregional group) 1 year after surgery, and 6 (4 in the general group and 2 in the locoregional group) 5 years after. The most common treatment for these patients is systemic therapy (chemotherapy, hormonal therapy, targeted therapies, and combinations of these). The subgroup of patients with oligometastatic disease and low systemic disease burden may benefit more from adding a locoregional treatment. This literature review aims to analyze the benefits and disadvantages of locoregional treatment in patients with de-novo oligometastatic breast cancer. Retrospective series report that this subgroup of patients may benefit from locoregional treatment, despite presenting with metastatic disease, by improving overall survival and local control. Additionally, several retrospective series have shown that locoregional treatment with surgery, surgery and adjuvant radiotherapy, or exclusive radiotherapy present similar results in local control. Therefore, exclusive radiotherapy may be a viable alternative in these patients. In recent years, attempts have been made to demonstrate the trend of retrospective series in prospective trials. These studies have shown great heterogeneity. Some with positive results in improving overall survival and others with negative results, but not statistically significant. The main limitation of retrospective series is the selection bias, which could explain the differences in the results obtained.
