The following data were collected: age, age at menopause, smoking status, </span>body mass index, adiposity, and physical activity. Plasma levels of follicle-stimulating hormone, estradiol, testosterone, </span>dehydroepiandrosterone sulfate (DHEAS), ∆</span> <span><span>4 androstenedione<span>, cortisol, insulin-like growth factor-1 (IGF-1), and 25-hydroxyvitamin D were evaluated. Directed acyclic graph was used for the selection of potential confounding variables, and the </span></span>linear regression was adjusted for confounders to study the association between endogenous hormones and mammographic parameters. Results are reported as β-coefficients (β) and 95% confidence interval (95% CI).</span></p></div><div><h3>Results</h3><p>Multivariable linear regression analysis adjusted for confounding variables showed that cortisol (β = 0.20; 95% CI: 0.02; 0.37), and ∆<!--> <!-->4 androstenedione (β = −1.90; 95% CI: −<!--> <!-->3.30, −<!--> <!-->0.39) were significantly associated with breast density. IGF-1 (β = −0.01; 95% CI: −<!--> <!-->0.20, −<!--> <!-->0.01) was the only hormone with significant association with glandular volume. Besides being uncommon, most cases of secondary bladder involvement occur as a result of </span></span>disseminated disease, being the solitary bladder metastasis sporadically reported.</p><p>This article presents the case of a woman with a history of invasive lobular breast carcinoma<span> who, 18 months after diagnosis, presents herself with complaints of urinary urgency<span> and urinary incontinence. Surgical management was effective, and follow-up shows no signs of recurrence. The lesions' probability of malignancy based on AI and BI-RADS categories of the lesion based on initial US interpretations were recorded. Categories obtained by both systems were divided into 2 groups as unsuspicious and suspicious in terms of malignancy and compared statistically. Reliability of AI was also evaluated.</p></div><div><h3>Results</h3><p>No statistically significant difference was found in the lesions' likelihood of malignancy based on the AI and initial US interpretations (<em>P</em> = .512). Additionally, a positive and substantial association (τ-b = 0.458, <em>P</em> < .001) between the levels of suspicion by AI and the initial US interpretation reports was discovered, as per Kendall-b correlation analysis. With a Cronbach alpha correlation coefficient of 0.727, the reliability was high for AI.</p></div><div><h3>Conclusions</h3><p>Benign inflammatory breast lesions may show suspicious appearances in terms of malignancy with US and AI. Artificial intelligence produces results comparable to radiologists' US reports for benign inflammatory diseases. However, there are still certain scenarios in which mastectomy is advocated. The concept of oncoplastic surgery has emerged to improve the defects resulting from conservative surgery, in which plastic surgery techniques are used and which could also have added value in reducing the indications for mastectomy. In this context, the aim of this review is to analyse the current scientific evidence on extreme oncoplastic surgery in locally advanced breast cancers.</p></div><div><h3>Materials and methods</h3><p>We undertook a search of the literature published in the Medline database using the terms \"extreme oncoplasty\" OR \"extreme oncoplastic\", and found a total of 17 publications. Finally, 6 articles were analysed, after applying the inclusion and exclusion criteria.</p></div><div><h3>Results</h3><p>The re-excision rate after extreme oncoplasty ranged from 0%-37.8% with a conversion rate to mastectomy of 0%–13.5%. The highest rate of minor complications was 24% and of major complications was 10%, with no delay in the initiation of adjuvant treatment. Satisfaction scores of more than 75% were obtained. These results compare favourably with conventional oncoplastic surgery and mastectomy.</p></div><div><h3>Conclusions</h3><p>Extreme oncoplasty may be a conservative alternative to mastectomy with reconstruction in a selected group of patients with local recurrence of less than 10% at 10 years, with a low complication rate and good patient satisfaction. The aim of our work is to identify the failure factors (FF) of the SLN biopsy in tumors larger than 4 cm.</span></p></div><div><h3>Material and methods</h3><p>This is a prospective series of patients with breast tumors larger than 4 cm without clinical axillary involvement. The colorimetric method was used in 52% of cases and the dual method in 48% of cases.</p></div><div><h3>Results</h3><p>The failure rate (FR) is 13%. The FF with a statistically significant difference are axillary invasion (<em>P</em>: .03; OR: 6.58) and the presence of lympho-vascular emboli (LVE) (<em>P</em><span> < .05; OR: 4.95). It should be noted that lympho-vascular emboli only concern those found in the peri-tumoral area. On the other hand, age over 60 years, Body mass index (BMI) over 30 and Ki 67 over 14 are not determining factors.</span></p></div><div><h3>Discussion</h3><p>Sener finds a more important FR when more than 10 lymph nodes of the axillary dissection are reached compared to the axillary hollows free from any neoplastic infiltration (<em>P</em>: .002; OR: 9.19). The FR for Gimbergues was statistically correlated with lymph node status (0% for pN0 tumors, versus 55% for pN1–N2 tumors). Etiology is still unclear, and an autoimmune response related to <em>Corynebacterium</em> is postulated. There are no diagnostic guidelines or therapeutic algorithms. In our study we analyzed the clinical presentations, images, cultures, and pathological findings together with the treatments, results, and evolution.</p></div><div><h3>Material and methods</h3><p>We carried out a descriptive, retrospective, and observational study of patients diagnosed with granulomatous mastitis treated in our hospital in the period 2017-2022.</p></div><div><h3>Results</h3><p>51 women. Average age 38.1 years. Pregnancies and Lactation 88.2%. Clinic: Abscessed and/or fistulized palpable mass 74.5%, painful palpable mass 13.8%. Asymptomatic mass 11.7%. Erythema Nodosum 11.7% BIRADS images 4/5: 82.3%. Average size: 3.54 cm. Pathology: Cystic vacuoles associated with <em>Corynebacterium</em> in 24 biopsies (47.1%). Bacteriology: positive cultures for <em>Corynebacterium</em> in 13 of 47 samples (27.6%). Treatment: antibiotic therapy 92.1%, immunosuppression 78.4%, surgical treatment 60.7%. Mean time of immunosuppression with corticosteroids 7.09 months and with methotrexate 9.27 months. Complications: 9.8% Aesthetic sequelae: 39.2% Disease-free time: 14.04 months.
