It represents no more than 4.8% of breast tumors and it rarely reaches a significant size to be categorized as a Giant Hamartoma. We present a case of a 31-year-old woman, who presented an asymmetrical breast growth. At physical examination with left macromastia, tumor of 20 x 10 cm. Resection and reconstruction with Wise pattern is performed in the breast without complications. In this study, the physiotherapeutic approach undertaken in the baby, promoting proper breast drainage for managing mastitis and breast pain during lactation, is described.</p></div><div><h3>Materials and methods</h3><p>A pilot study was conducted with a sample of breastfeeding women who presented mastitis and breast pain. An assessment and treatment were carried out through physiotherapy in the baby at the musculoskeletal level of the cervical, cranial, oral, and temporomandibular joint regions. Specific physiotherapeutic techniques were applied to the baby to improve breastfeeding biomechanics and thereby promote proper milk drainage.</p></div><div><h3>Results</h3><p>Improvements in mastitis symptoms and breast pain were observed after physiotherapeutic intervention. Out of the total sample (30 cases), 17 (56.7%) reported pain during breastfeeding at the time of assessment. Additionally, in these cases of pain, mastitis was present in 35.3%. After physiotherapy treatment, these symptoms disappeared in the mother, being significant with a p-value &lt;<!--> <!-->0.001.</p></div><div><h3>Discussion</h3><p>This study examines how physiotherapy, by improving biomechanics and baby function during breastfeeding, can facilitate proper breast drainage and effectively help resolve mastitis and breast pain. Axillary polymastia is the most common form of presentation (71%) and can be affected by diseases of the breast tissue: benign and malignant diseases. Phyllodes tumors are unusual fibroepithelial tumors in the mammary gland (1% of neoplasms). Their presentation in ectopic tissue is extremely rare, with only 10 cases having been reported in the literature. In breast cancer, many women experience therapeutic failure and unfavorable clinical outcomes due to mechanisms related to chemoresistance acquisition, which may include oxidative stress. In this study, we investigated the systemic oxidative stress profile of women diagnosed with chemoresistant breast cancer and evaluated the correlation of this profile with clinicopathological features.</p></div><div><h3>Methods</h3><p>The oxidative stress levels were determined based on lipid peroxidation and nitric oxide metabolite (NOx) measurements. Chemoresistance was determined based on the Response Evaluation Criteria in Solid Tumors guidelines, and patients were categorized as responsive (complete response) or chemoresistant (partial or no response).</p></div><div><h3>Results</h3><p>Reduced lipid peroxide levels were observed independent of the pattern of chemotherapy response, without NOx variation. The type of drug schedule did not interfere with oxidative stress levels in the responsive patients. However, lipid peroxide levels were reduced in patients in the chemoresistant group receiving the combination of adryamicin<!--> <!-->+<!--> <!-->ciclofosfamide<!--> <!-->+<!--> <!-->Taxol. It is a rare entity, not yet fully characterized in terms of its clinical behavior and evolution.</p><p>Its differential diagnosis includes papillary thyroid carcinoma as well as other types of papillary carcinomas of the breast.</p><p>We review the literature and present a case with three years of follow-up and with special attention to its clinical, morphologic, immunohistochemical, genetic and follow-up features. Surgery, radiotherapy and chemotherapy are the only effective therapies, since it has no target treatments. In TNBC, AR modulation has been demonstrated to inhibit cell development and increase apoptosis in vitro and in vivo.</p></div><div><h3>Objective</h3><p>To assess whether AR in TNBC is associated with a better prognosis.</p></div><div><h3>Material and methods</h3><p>This study is a retrospective description of a cohort of 163 TNBC patients, who underwent treatment from 2003 to 2017. Immunohistochemical determinations were examined in the surgical specimen for AR, E-cadherina and GATA3.</p></div><div><h3>Results</h3><p>AR positive TNBC are more differentiated and less proliferative tumors than AR negative (p &lt; 0.05). However, this fact has not been significantly associated with better overall survival nor disease free survival. Loss of expression of GATA3 and E-cadherina has not been related to worse overall survival in our paper.</p></div><div><h3>Conclusions</h3><p>Although no difference in TNBC survival related to AR expression has been observed, determination of AR may be useful in TNBC due to possibility of modulating AR as a target therapy. At the time of initial diagnosis, 5%–6% already presented metastasis. In 2007, The National Institute for Health and Care Excellence published a set of recommendations about low-value practices commonly known as “do-not-do” (DND), which lead to initiatives to decrease the overuse of health resources. The aim of our study was to collect, based on an expert consensus, DND recommendations for metastatic breast cancer and assess frequency of adherence in the real world.</p></div><div><h3>Methods</h3><p>Mixed analysis based on a literature review that combined a qualitative analysis (a panel of 10 experts reached an agreement on a list of criteria by consensus building) and a quantitative analysis (observational retrospective study assessing codified clinical information from electronic health records [EHRs] about the DND recommendations in 4 Spanish university hospitals).</p></div><div><h3>Results</h3><p>Consensus was reached on 12 recommendations based on the review of 826 EHR from the participating hospitals. The review revealed a 1.3%–28.1% range for improvement in the adherence to the recommendations. In cases where positive nodes are negative after chemotherapy, patients may be selected for targeted axillary dissection. This technique consists of marking the positive node and then administering neoadjuvant systemic management. If a complete clinical and imaging response is achieved, patients are taken to resection of the marked node and sentinel nodes. This procedure allows good locoregional control of the disease, avoids futile axillary emptying and reduces the percentage of post-surgical complications such as lymphedema. The objective of our review was to describe the agreement between the sentinel node and the clipped node; for this we followed the PRISMA guide and performed a literature search in Pubmed, Embase, LILACS and WoS databases. We found that the agreement between the sentinel node and the clipped node is 70.9%. Performing Targeted Axillary Dissection, even in patients where the clipped node is not found after neoadjuvant treatment, as well as in centers where axillary node marking is not available, is a feasible option taking into account the high concordance between the clipped node and the sentinel nodes. Finally, 53 articles were analyzed, after applying the inclusion and exclusion criteria.</p></div><div><h3>Results</h3><p>All women should undergo a risk assessment starting at 25 years of age. In the high-risk group, the screening strategy should be early, multimodal and annual. Ample evidence in the literature that MRI is the most sensitive imaging method for the diagnostic of breast cancer.</p></div><div><h3>Conclusion</h3><p>Breast MRI will be the method of choice along with mammography to perform annual screening in this risk group. Genomic risk was established using the 70-gene gene expression platform (MammaPrint®). The result obtained with the genomic risk assessment has been considered “gold standard”.</p></div><div><h3>Results</h3><p>126 cases (56.25%) showed a “low” risk genomic profile and 98 cases (43.75%) showed a “high” risk. Among those with low genomic risk, 26.19% of tumors classified “T2”, “T3” and “T4” were observed. On the contrary, among those with “high” risk, 67.35% of tumors classified as “T1” were observed. Regarding the “N” status, among the “low” risk tumors, 26.98% of cases classified as “N1”, “N2” and “N3” were observed. On the other hand, among those with “high” genomic risk, 77.14% of cases classified as “N0” were observed. Among a total of 131 “T1N0” tumors, 57 (43.51%) showed “high” genomic risk. No significant differences have been observed in terms of risk assessment of metastatic disease (high vs. low) between the various “TN” categories studied.</p></div><div><h3>Conclusions</h3><p>The evaluation of the risk of metastatic disease using the “TN” system is not accurate. The detection of early tumors does not necessarily mean a good prognosis.
