D’autre part, il existe un risque de conversion en une maladie neurodégénérative de type α-synucléinopathie, comme la maladie de Parkinson ou une démence à corps de Lewy. Dans cet article, nous en présenterons les principales caractéristiques, et proposerons également des outils à destination du clinicien, afin d’en favoriser le dépistage et la prise en charge.</div></div><div><div>Within the spectrum of sleep disorders, the diagnosis of rapid eye movement sleep behavior disorder, affecting 1 % of patients, predominantly males, and occurring after the age of 50, is of crucial importance. Firstly, the risk of injury should not be overlooked. Secondly, there is a risk of conversion to a neurodegenerative disease of the α-synucleinopathy type, such as Parkinson's disease or Lewy body dementia. A notable concept related to this matter in clinical psychology and development is ego integrity, which refers to a person's ability to look back on their life with a sense of fulfillment. It is conceived as a form of self-actualization among older adults. Ego integrity has been shown to play a protective role against mental health outcomes like anxiety and depression and to increase psychological well-being. Despite their significance, no research has synthesized the recent findings on self-actualization and ego integrity among older adults, and few interventions are based on this construct.</div></div><div><h3>Methodology</h3><div>This systematic integrative review provides a panorama and an updated perspective on the factors associated with ego integrity and self-actualization among older adults, and paves the way for interventions and therapies that can be implemented in nursing homes. The literature from the last decade was examined across five databases using the PRISMA guidelines, and 34 relevant articles were synthesized.</div></div><div><h3>Findings and clinical implications</h3><div>Five key themes emerged: demographic factors, life review and reminiscence, existential factors, psychological and mental health factors, and spirituality and religiosity. This review indicates that ego integrity plays a protective role against the mental health outcomes of depression, anxiety, and fear of death, and is associated with greater resilience, life satisfaction, and a sense of meaning and purpose. Additionally, higher levels of spirituality, higher levels of social support, and being married seem to contribute to self-actualization among older adults, which, in turn, promotes better mental health. Lastly, the review suggests that reminiscence therapies, psychospiritual and existential interventions, and preventive measures can help develop self-actualization in late adulthood. Consequently, the implementation of these interventions in nursing homes and similar facilities can help older adults achieve better well-being and mental health outcomes.</div></div><div><h3>Conclusion</h3><div>This review highlights the importance of self-actualization in the care of the elderly and links theoretical knowledge to practical implications. It provides insights into ways of promoting psychological well-being and mental health in late adulthood and suggests potential intervention avenues. C’est dans ce contexte que la recherche actuelle fut effectuée. L’étude adopte une approche quasi-expérimentale auprès des personnes âgées. L’objectif consiste à déterminer si l’accompagnement pouvait être associé au dépassement de l’anxiété ainsi qu’à l’amélioration du bonheur chez des personnes en institution, sur une période de 2 mois. Un échantillon de 40 participants âgés entre 65 et 75 ans, en Ehpad et de nationalité libanaise, a été réparti en deux groupes, de 20 chacun. Le premier, groupe expérimental, comprend les personnes bénéficiant d’un programme d’accompagnement et le second, groupe contrôle, compte les personnes non accompagnées. Afin d’évaluer l’anxiété et le bonheur, deux instruments ont été utilisés : l’échelle d’anxiété de Hamilton et le Oxford Happiness Inventory. Les résultats du pré-test révèlent un score d’anxiété élevé lequel se réduit notablement après la phase de l’accompagnement, contrairement au score du bonheur qui, visiblement bas au départ, s’améliore par la suite. De même, la comparaison établie entre les deux groupes, après l’accompagnement, souligne les bénéfices de l’intervention. Les résultats illustrent le rôle de l’accompagnement dans la réduction de l’anxiété et l’amélioration du sentiment de bonheur chez les personnes âgées.</div></div><div><div>With the aging of the population observed in many contemporary societies, the well-being of older people has become a central public health and social policy issue. The current study was conducted in this context. It adopted a quasi-experimental approach to a sample of older adults. The study objective was to determine whether a support intervention of 6 months could help overcome anxiety and improve happiness among older adults. A sample of 40 participants aged from 65 to 75 years in a retirement home and of Lebanese nationality was divided into two groups of 20 each. The experimental group included accompanied older adults, while the control group included individuals who were not accompanied. Two instruments were used to assess anxiety and happiness: the Hamilton Anxiety Scale and the Oxford Happiness Inventory. The results of the pre-test revealed high anxiety scores, which were significantly reduced after the support intervention. The happiness score was clearly low initially and subsequently improved. Likewise, the comparison of the two groups after the support intervention underlines the benefits of the intervention. Many disorders occur with aging. One of the common disorders is the occurrence of dementia and Alzheimer's (AD). The robust link between the hippocampus and the vestibular system suggests a likely relationship between vestibular impairments and the onset of Alzheimer's disease. This article describes the main findings related to the role of the vestibular system in AD and stresses the importance of providing rehabilitation based on the function of the vestibular system in AD. Recent studies have shown that vestibular dysfunction is increasingly common in patients with AD, and patients with AD with vestibular impairment show more visual-spatial cognitive impairment.</div></div><div><div>Le vieillissement de la population est une tendance mondiale irréversible et croissante. De nombreux troubles surviennent avec le vieillissement. L’un des troubles courants est la démence et la maladie d’Alzheimer (MA). Le lien entre l’hippocampe et le système vestibulaire suggère une relation probable entre les déficiences vestibulaires et l’apparition de la maladie d’Alzheimer. Cet article présente les données sur le rôle du système vestibulaire dans la MA et souligne l’importance de prescrire une rééducation spécifique du système vestibulaire dans la MA. Ce traitement est indiqué dans les formes modérément sévère à sévère (MMSE<!--> <!--><<!--> <!-->20) de la maladie d’Alzheimer (MA). La mémantine présente une efficacité certes limitée mais qui a été étudiée contre placebo dans des études randomisées contrôlées (ERC) sur 4 critères de jugement cliniques : l’impression clinique de changement, l’autonomie pour les activités de la vie quotidienne, les performances cognitives globales et la présence de troubles psychologiques et du comportement. Chez les patients ayant une MA modérée à sévère et prenant de la mémantine, le déclin cognitif et la perte d’indépendance fonctionnelle sont significativement plus lents que chez les patients sous placebo, avec une proportion plus élevée de patients chez qui l’impression de changement est stable. Un effet positif sur la survie des patients avec la MA traités par mémantine est suggéré dans la littérature récente. De plus, la mémantine est un traitement bien toléré. Dans les ERC, il n’y avait pas de différence de tolérance entre la mémantine et le placebo. La mémantine doit donc être prise en compte dans le traitement de la MA. Les progrès obtenus avec les immunothérapies anti-amyloïdes ne remettent pas en cause cette notion.</div></div><div><div>Memantine is a non-competitive N-methyl-D-aspartate receptor antagonist. This treatment is indicated in moderately severe to severe (MMSE<!--> <!--><<!--> <!-->20) forms of Alzheimer's disease (AD). Memantine has limited efficacy. The efficacy of memantine has mainly been studied against placebo in randomised controlled trials (RCT) on 4 clinical evaluation criteria: clinical impression of change, autonomy for activities of daily living, overall cognitive performance, and the presence of psychological and behavioural disorders. Patients with moderate to severe AD taking memantine have shown delays in cognitive decline and loss of functional independence, and appeared generally stable compared to patients taking placebo. The recent literature suggests a positive effect on survival among AD patients treated with memantine. In addition, memantine is a well-tolerated treatment. In the RCT, there was no difference in tolerance between memantine and placebo. Memantine should therefore be considered in the treatment of AD. Les personnes âgées vivant en établissement ou aidées à domicile courent ainsi le risque de se retrouver réduites au silence, exclues de leur position de sujet et de citoyen par un système qui fait tout pour les « protéger » et préfère penser à leur place plutôt que leur donner la parole. En organisant chaque année un colloque au cours duquel ce sont ces personnes âgées qui parlent et les professionnels qui écoutent, Citoyennage vise à inverser la tendance en leur donnant l’occasion de retrouver une position de sujet libre de s’exprimer et doté d’un pouvoir d’action. Reconnus dans leur altérité et leur statut de citoyen, ils peuvent aussi bien agir sur leur quotidien que retrouver leur place légitime d’interlocuteurs des professionnels et des pouvoirs publics. Nous verrons les effets de cette démarche sur les participants, à la fois sur le plan narcissique et relationnel et comment elle contribue à la relance des investissements. Nous montrerons également son impact sur les pratiques professionnelles, le projet d’établissement ou de service et les représentations sociales du grand âge et discuterons des freins possibles qu’elle peut rencontrer.</p></div><div><p>The authors propose to show how the approach of the association <em>Citoyennage</em> is a response to the deleterious effects on elderly subjects of an ageist society that disqualifies those who remind them that human beings are not immortal nor eternally young. Elderly people living in institutions or assisted at home thus run the risk of finding themselves reduced to silence, excluded from their position as subjects and citizens by a system that does everything to “protect” them and prefers to think for them rather than giving them the floor. By organizing a symposium each year during which elderly people hold the floor and professionals listen, <em>Citoyennage</em> aims to reverse the trend by giving elderly people the opportunity to find their position as subjects free to express themselves and endowed with a power for action. Recognized in their otherness and their status as citizens, they can both act on their daily lives and regain their legitimate place as protagonists with professionals and public authorities. We explore the effects of this approach on participants in both the narcissistic and the relational sphere, and how this contributes to renewed investment. La dépression chez les personnes âgées est le plus souvent le fait de formes tardives survenant chez une personne sans antécédent dépressif. Leur pronostic cognitif diffère des dépressions d’un adulte vieillissant ayant une longue histoire de dépression. Dans ces deux formes cliniques, deux tiers des malades âgés restent déprimés un an plus tard malgré le traitement. La résistance au traitement de la dépression de la personne âgée pose souvent le problème de l’observance thérapeutique. Les formes résistantes aux antidépresseurs peuvent être liées à des facteurs hormonaux, environnementaux ou traumatiques. Elles peuvent encore avoir des causes médicales en raison de la polypathologie fréquente chez les personnes âgées ou en raison d’une longue histoire psychiatrique. Les dépressions tardives résistantes aux traitements peuvent être liées aux morbidités associées, à leurs traitements, voire rendre visible une maladie plus grave sous-jacente en particulier neurocognitive non encore révélée. Les traitements pour la dépression résistante incluent des ajustements de posologie, des changements de médicaments et des soins psychothérapiques. Des évaluations régulières sont essentielles ainsi qu’une alliance thérapeutique pour tenter d’améliorer la qualité de vie pour les patients dépressifs. L’orientation vers un psychiatre est indiquée pour des interventions médicales complexes en raison des difficultés de prise en charge.</p></div><div><p>Resistant depression in the elderly is defined as the failure of at least two antidepressants used successively at the right dosage during a single depressive episode, lasting at least 8 weeks. Depression in the elderly is most often the result of late forms occurring in a person without a history of depression. Their cognitive prognosis differs from depressions in an aging adult with a long history of depression. In these two clinical forms, two thirds of elderly patients remain depressed one year later despite treatment. Resistance to treatment for depression in the elderly often raises the problem of therapeutic compliance. Antidepressant-resistant forms can be linked to hormonal, environmental or traumatic factors. They may also have medical causes resulting from the polypathology that is common in the elderly, or from a prolonged psychiatric history. Treatment-resistant, late-onset depression can be related to associated morbidities, their treatments, or it can even be a sign of a serious underlying condition, particularly neurocognitive disease that has not yet been detected. À partir de la description d’une situation clinique, nous avons voulu présenter le cheminement complet d’une visite : de sa préparation aux échanges avec les patients rencontrés, jusqu’à l’interprétation des faits et lieux observés et,enfin,à l’élaboration d’une réflexion qui guidera la rédaction du compte rendu. Nous avons utilisé des éléments du symbolisme de la maison pour montrer, à quel point, ces visites peuvent être délicates mais essentielles pour comprendre la singularité de la situation médico-psychosociale de chaque patient. Enfin, nous avons voulu développer une réflexion éthique sur ces interventions et ses conséquences dans le « chez-soi » des personnes concernées.</p></div><div><p>Visits by inpatient geriatric consultation teams help to improve the care of the elderly by observing them in their living environment. Starting with a description of a clinical situation, we set out to present the complete process of a visit of this nature, from its preparation to exchanges with the patients encountered, the interpretation of the realities and settings observed and the elaboration of reflection to guide the drafting of a report. We use the symbolism of the house/home to illustrate just how challenging these visits can be, but how fundamental they are to understanding each patient's unique medico-psycho-social situation. {"title":"Prevalence of depression among the elderly in Morocco","authors":"","doi":"10.1016/j.npg.2024.02.006","DOIUrl":"10.1016/j.npg.2024.02.006","url":null,"abstract":"<div><p>Our aim was to estimate the prevalence of depression among the elderly in Morocco and to study the associated risk factors.</p><p>This was a cross-sectional survey of people aged 65 and over attending health centres in the province of Essaouira. We used the short version of the Geriatric Depression Scale (GDS-15) to study the prevalence of depressive symptoms.</p><p>The study included 509 patients, the majority of whom were men (54.8%). The mean age was 71.1 years (± 6.8); 55% of participants were chronically ill. The GDS-15 score ranged from 2 to 12, with an average of 5.4 (± 3.2). Among our participants, 35.4% had depressive symptoms. There was a significant correlation between the presence of depressive symptoms and marriage, chronic illness and advanced age.</p><p>Our study has shown that depressive symptoms are very common among the elderly in Morocco. A screening strategy and a strengthening of mental health care provision are strongly recommended to ensure that Moroccan elderly people age actively and in good health.</p></div><div><p>Notre objectif était d’estimer la prévalence de la dépression chez les personnes âgées au Maroc et d’étudier les facteurs de risque qui sont associés.</p><p>Il s’agissait d’une enquête transversale menée auprès des personnes âgées de 65 ans et plus fréquentant les centres de santé de la province d’Essaouira au Maroc. Nous avons utilisé la version abrégée de l’échelle de dépression gériatrique (GDS-15) pour étudier la prévalence des symptômes dépressifs.</p><p>L’étude a inclus 509 personnes, majoritairement des hommes (54,8%). L’âge moyen était de 71,1 ans (± 6,8); 55% des participants étaient atteints de maladies chroniques. Le score GDS-15 variait de 2 à 12 avec une moyenne de 5,4 (± 3,2). Parmi nos participants, 35,4% avaient des symptômes dépressifs. Une corrélation significative était mise en évidence entre la présence de symptômes dépressifs et le mariage, les maladies chroniques et l’âge avancé.</p><p>Notre étude a montré que les symptômes dépressifs sont très fréquents chez les personnes âgées au Maroc.
