This is particularly true of digital hearing instruments, which typically include non-linear amplification in multiple channels. Furthermore, these instruments often incorporate additional non-linear functions such as “noise reduction” and “feedback cancellation”. The output signal produced by a non-linear hearing instrument relates to the characteristics of the input signal in a complex manner. Therefore, the choice of input signal significantly influences the outcome of any acoustic or psychophysical assessment of a non-linear hearing instrument. For this reason, the International Collegium for Rehabilitative Audiology (ICRA) has introduced a collection of noise signals that can be used for hearing aid testing (including real-ear measurements) and psychophysical evaluation. This paper describes the design criteria, the realisation process, and the final selection of nine test signals on a CD. Also, the spectral and temporal characteristics of these signals are documented. The ICRA noises provide a well-specified set of speech-like noises with spectra shaped according to gender and vocal effort, and with different amounts of speech modulation simulating one or more speakers. These noises can be applied as well-specified background noise in psychophysical experiments. They can also serve as test signals for the evaluation of digital hearing aids with noise reduction. It is demonstrated that the ICRA noises show the effectiveness of the noise reduction schemes. Based on these initial measurements, some initial steps are proposed to develop a standard method of technical specification of noise reduction based on the modulation characteristics. For this purpose, the sensitivity of different noise reduction schemes is compared by measurements with ICRA noises with a varying ratio between unmodulated and modulated test signals: a modulated-unmodulated ratio. It can be anticipated that this information is important to understand the differences between the different implementations of noise reduction schemes in different hearing aid models and makes. Los estándares actuales de las especificaciones técnicas de los auxiliares auditivos proporcionan posibilidades limitadas para conocer la influencia de las characterísticas temporales y espectrales de la señal de entrada y estas caracteristicas tienen un efecto significativo en la señal de salida de muchos tipos actuales de auxiliares auditivos. Esto es particularmente cierto en el caso de los instrumentos digitales, que típicamente presentan una amplification no lineal en canales multiples. Incluso, estos instrumentos a menudo incorporan funciones no lineales adicionales como la “reduction de ruido” y la “ca","PeriodicalId":30299,"journal":{"name":"Audiology","volume":"40 1","pages":"148 - 157"},"PeriodicalIF":0.0,"publicationDate":"2001-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.3109/00206090109073110","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69474659","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

To be useful as a clinical tool, a high signal to noise ratio (SNR) is required for a minimal testing time. Four EEG derivations were compared in six relaxed, cooperative awake normally hearing adults to compare the SNRs. Stimuli comprised a 1 kHz carrier wave, amplitude modulated at 39 Hz and 90 Hz. They were presented for about 4.4 min in one ear at a level slightly above the behavioural threshold (10 dB SL). In the 39 Hz condition, the SNRs from the different derivations did not differ significantly. However, in the 90 Hz condition one novel derivation (Cz-inion with ground at Pz) yielded a significantly lower noise level, about 60 per cent of the noise level of the other derivations. The SNR of this derivation was about 50 per cent larger than from other derivations. Such an increase of the SNR implies that testing time is reduced by about 56 per cent as compared to conventional derivations in these subjects. Las respuestas de seguimiento a la modulación de la amplitud (amplitud modulation following responses - AMFR) o los potenciales evocados de estado estable (steady-state evoked potentials -SSEP) pueden ser utilizados para la estimación de umbrales auditivos objetivos y de frecuecia-especifica, en sujetos dormidos y despiertos. Para ser útiles como herramienta clinica, se requiere una tasa señal/ruido alta (signal-to-noise ratio -SNR) por un tiempo minimo de evaluación. Se compararon cuatro derivaciones del EEG en seis adultos normo-oyentes, despiertos, cooperadores y relajados, para cvaluar las tasas de señal/ruido. Los estimulos involucraron una onda portadora dc 1 kHz, con amplitud modulada a 39 Hz y a 90 Hz. Los sujetos fueron estimulados por unos 4.4 minutos en cada oído, a un nivel apenas por encima de su umbral conductual (10 dB SL). En la condició de 39 Hz, los SNR de las difcrentes derivaciones no variaron significativamente. Sin embargo, en la condición de 90 Hz, una derivación novedosa (Cz-Inion con tierra en Pz) rindió un nivel de ruido significativamente más bajo, cerca del 60% del nivel de ruido de las otras derivaciones. EL SNR de esta derivación fue alrededor de un 50 por ciento más grande que el de otras derivaciones. Tal incremento del SNR implica que el tiempo de evaluación se reduce en un 56 por ciento, en comparación con las derivaciones convencionales en estos sujetos.","PeriodicalId":30299,"journal":{"name":"Audiology","volume":"40 1","pages":"202 - 207"},"PeriodicalIF":0.0,"publicationDate":"2001-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.3109/00206090109073115","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69474852","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The cisplatin was administered as a 16 mg/kg, dose introduced by a slow 30-min intraperitoneal infusion. Data from three DP-gram protocols, DPOAE input-output responses at 8 kHz, and auditory brainstem responses (ABRs) at 8, 12 and 16 kHz were collected before and 72 h after treatment. The post-treatment ABRs at 16 kHz showed the greatest mean threshold shift of 33.6 dB. The post-treatment DP-gram data showed significant reduction of the signal to noise ratios in the majority of the frequencies tested, across all tested protocols. The data suggest that the most sensitive DPOAE procedure for the early detection of the cisplatin-induced ototoxic damage is the DPOAE I/O protocol. Morphological analyses indicated that the inner hair cells remained intact, while several types of alterations were observed in the arrangement of the stereocilia in the outer hair cells.","PeriodicalId":30299,"journal":{"name":"Audiology","volume":"40 1","pages":"253 - 264"},"PeriodicalIF":0.0,"publicationDate":"2001-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.3109/00206090109073119","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69475168","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We utilized an immortalized cell line (OC-k3 cells) derived from the organ of Corti of transgenic mice in order to perform in-depth biochemical studies with no limitations on sample size and number. We probed these cells with cisplatin and gentamicin, two drugs which display in vivo undesired ototoxic side-effects. We investigated cell viability, reactive oxygen species (ROS) production and glutathione (GSH) levels and tested the effects of different concentrations of cisplatin and gentamicin from 0 to 48 h. Results show that cells undergo a dose-and treatment-time-dependent apoptosis characterized by nuclear fragmentation, integrity of the cell membrane and mitochondria, and absence of DNA endonuclease activity. During the early part of treatment, ROS production increases and intracellular GSH decreases, probably due to the activation of protein kinase Ca. Use of antioxidants such as acetylcysteine, GSH and vitamin C rescues cells from apoptosis almost completely. Overall, these data indicate that ROS generation might play a central role in inducing inner ear cell apoptosis and may have an additive role in the ageing process.","PeriodicalId":30299,"journal":{"name":"Audiology","volume":"40 1","pages":"327 - 335"},"PeriodicalIF":0.0,"publicationDate":"2001-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.3109/00206090109073130","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69476336","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The amount of smoothed phonemic compression in a high-frequency channel was varied using wide-band control. Two alternative systems were tested to compensate for upward spread of masking (USOM) and to reduce modulations in the high-frequency channel effectively. Consonant-vowel-consonant tests were conducted in a group of 14 subjects using eight different speech-processing settings. Speech intelligibility improved significantly with compression, mainly due to positive effects on the initial-consonant score. Surprisingly, listeners with a smaller residual dynamic range tended to profit less from compression. Compensation for USOM gave an additional improvement of vowel intelligibility. In background noise, consistently negative effects of speech processing were found. The combined use of phonemic compression and USOM compensation is promising in conditions without background noise.","PeriodicalId":30299,"journal":{"name":"Audiology","volume":"40 1","pages":"10 - 25"},"PeriodicalIF":0.0,"publicationDate":"2001-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.3109/00206090109073096","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69473459","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In the Veneto region (Italy), a representative sample of 2700 independently living individuals of 65 years of age and older was selected for the study. All participants were administered a comprehensive questionnaire and a brief examination at their home, including a general physical examination, speech audiometry, Sanders' Speech Disability test, part I and 111, Mini Mental State Examination, CES-D scale for depression, visual acuity, self-reported diseases and physical function. Auditory function was worst in the older individuals: auditory performance was within acceptable limits up to the 75–79 age group, while it rapidly deteriorates in the older groups. This trend is consistent with self-reported auditory disability (Sanders' test). A detailed analysis of the type of errors made in the speech audiometry was conducted for each subject. Speech audiometry is a good indicator of real hearing difficulties faced by the elderly, and it might be preferred to pure-tone audiometry, since hearing deficits with age are not always limited to an increased detection threshold, but include other aspects of hearing such as distortion of sounds, comprehension of speech and noise discrimination.","PeriodicalId":30299,"journal":{"name":"Audiology","volume":"40 1","pages":"285 - 291"},"PeriodicalIF":0.0,"publicationDate":"2001-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.3109/00206090109073124","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69476151","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Subject selection was based on the hearing levels from 0.5 to 4 kHz being better than 25 dB HL, normal tympanograms and stapedial reflexes, presence of TEOAEs in at least one ear and no history of otological disease, noise exposure, ototoxic drugs, metabolic disease associated with hearing loss or a family history of hearing loss. The ear with better audiological thresholds was selected as the test ear. If there was no difference between the ears, the ear with stronger TEOAEs was selected. Subjects were divided into five age groups: 20–34 years (n=12, mean age 23.7). 35–44 (n=11, mean age 39.7), 45–54 (n=8, mean age 48.1), 55–64 (n=10, mean age 60), 65–78 (n=11, mean age 71). TEOAEs were never absent in the first two groups, but they were absent in two ears in the 45–54 group, and in one ear of the 55–64 and 65–78 groups. Mean TEOAE amplitude decreased with age, but the differences between the five groups were not significant. Contralateral white noise suppressed emission amplitude, but the amount of suppression was not significantly different between the five groups. A linear regression analysis showed a significant correlation between age and hearing levels, and a negative correlation between age and TEOAE amplitude. In addition, a significant negative correlation between hearing threshold and TEOAE amplitude was evident. Cochlear monitoring aimed to compare the shifts in round-window compound action potentials (CAP) thresholds to those of 2fl-f2 distortion-product otoacoustic emissions (DPOAE, frequency of stimuli fl and f2). Both responses were evaluated every 1/10th octave between 6 and 12 kHz for CAP thresholds, and from 4 to about 14 kHz for DPOAEs in response to 50- and 70-dB SPL stimuli. The auditory fatigue turned out to be sufficiently mild that DPOAEs remained present, so that their microstructure could be followed up while the stimulus frequency ratio f2/fl was swept from 1.06 to 1.30 (fixed f2) so as to derive DPOAE level profiles against f2/fl and group latencies. CAP thresholds decreased by about 10 dB above 7.2 kHz, whereas DPOAE amplitudes decreased at most f2 frequencies from 6.6 kHz to 15.2 kHz, with the range of decrease being slightly narrower at higher stimulus intensities. While the mean DPOAE shift after 1 hour was around 5 dB irrespective of stimulus intensity, it tended to increase slightly after 2 hours despite stable CAP thresholds. DPOAE profiles against f2/f1 were slightly modified by the auditory fatigue, so that the maximum tended to be reached at lower ratio. No significant variation of DPOAE latencies was found after acoustic overstimulation. These experiments show that complex DPOAE changes were induced by auditory fatigue and their relationship to CAP threshold changes does not seem to be straightforward. Nonetheless, fine DPOAE recordings might be useful to detect early changes in cochlear mechanics. Los cobayos fueron expuestos a un tono de 6kHz a 80dBSPL por 30 minutos para inducirles fatiga auditiva moderada reversible en las 4 horas siguientes. Se realizó un monitoreo coclear para comparar las caídas en el umbral del componente de la ventana oval del potencial de actión (CAP) con aquellos de los productos de distorsión de las emisiones otoacusticas (DPOAE) 2fl - f2. Ambas respuestas se evaluaron cada 1/10 de octava entre 6 y 12kHz para los umbrales CAP y de 4 hasta casi 14kHz para DPOAE en respuesta a estimulos de 50 a 70dBSPL. La fatiga auditiva resultó ser lo suficientemente moderada para que los DPOAEs permanecieran presentes y para que su micro-estructura pudiera mantenerse mientras que el rango de frecuencia f2-f1 se trasladó de 1.06 a 1.30 (f2 fija) para derivar los niveles de DPOAE en contraste con f2/f1 y las latencias grupales. Los umbrales de CAP disminuyeron en 10dB por debajo de los 7.2kHz mientras que la amplitud de los DPOAE descendió en casi todas las frecuencias desde 6.6kHz a 15.2kHz, con un rango de disminución menor cuando la intensidad fuera mayor. Mientras que la caída promedio de los DPOAE después de lhr era de alrededor de 5dB sin relatión con la intensidad del estimulo, tendía a incrementarse un poco después de dos horas a pesar de umbra","PeriodicalId":30299,"journal":{"name":"Audiology","volume":"40 1","pages":"113 - 122"},"PeriodicalIF":0.0,"publicationDate":"2001-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.3109/00206090109073106","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69474269","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The Gothenburg profile and the SF-36 questionnaire were mailed to 634 subjects appointed for audiological examination with a request to complete the questionnaires at home. Three questions were enclosed concerning the capacity to complete the questionnaires, showing that 1.9 per cent were incapable of completing them, 17.5 per cent needed assistance, 43.5 per cent completed the questionnaires, and 37.1 per cent did not respond to them. Thus the response rate was only 55.5 per cent and, in addition, these responses were characterised by missing data. An arbitrary criterion of an average score per question of 3 for the averaged HD and HH items respectively was applied, resulting in 72.7 per cent indicating HD, whereas 30.3 per cent indicated HH according to the items in the profile. Significant differences in hearing levels were found between those experiencing HD and HH having a score 3 and those with a score <3. When analysing the general health status, general perception of health and social functioning, significantly lower scores were found for those experiencing HD/HH, whereas no significant differences were found between the total sample of hearing-impaired subjects and the age-matched population for these parameters. It is concluded that the present procedure cannot be implemented as a routine outcome measure in a clinical setting and alternative outcome measures should be considered. Para obtener información detallada sobre los resultados de la rehabilitation en un contexto clínico, se introdujeron medidas clínicas sobre discapacidad y desventaja auditiva (hearing disability/handicap) y el presente estudio describe las experiencias preliminares con estas medidas, en una serie de sujetos con hipoacusia que fueron referidos para evaluación audiológica. Para subrayar la discapacidad/desventaja auditiva (HD/ HH), antes del inicio o continuatión del trata-miento y para evaluar cualquier efecto adverso resultante de los trastornos auditivos, se utilizaron el perfil Gothenburg y el cuestionario genérico de calidad de vida relacionada con la salud (SF-36). El perfil Gothenburg y el cuestionario SF-36 fueron enviados por correo a 634 sujetos citados para examen audiológico, con la solicitud de que llenaran los cuestionarios en casa. Se incluyeron tres preguntas relacionadas con la capacidad de completar los cuestionarios, mostrando que el 1.9 por ciento er","PeriodicalId":30299,"journal":{"name":"Audiology","volume":"40 1","pages":"208 - 214"},"PeriodicalIF":0.0,"publicationDate":"2001-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.3109/00206090109073116","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69474860","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

AMFR is represented in the frequency domain by a single spectral line. This simplifies the objective statistical detection in comparison to other responses with a more complex waveform. The aim of the present study is to compare the performance of four known tests (phase coherence (PC), new Hotelling T2 (HT2*), modified PC (PC*), magnitude-squared coherence (MSC)) on the basis of a large sample (n = 1484) of AMFR recordings (stimulus level of 30 dB nHL in normally-hearing subjects, and 30 dB SL in hearing-impaired subjects) to find the test best suited for AMFR detection. The decision was made on the basis of the detection rates as well as of the ROC curves. Based on the large data pool, MSC and PC* show equal performance and a small but consistent advantage in objective detection of AMFR over HT2* and PC.","PeriodicalId":30299,"journal":{"name":"Audiology","volume":"40 1","pages":"245 - 252"},"PeriodicalIF":0.0,"publicationDate":"2001-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.3109/00206090109073118","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69474912","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
