This study reports a case of occupational ultraviolet cataract caused by prolonged exposure of a magnetic particle testing worker to ultraviolet radiation exceeding standard intensity levels. {"title":"[Characteristics of 972 poisoning inpatients].","authors":"L Yan, Y Y Wang, Y Peng, W He, J L Tang","doi":"10.3760/cma.j.cn121094-20240407-00150","DOIUrl":"https://doi.org/10.3760/cma.j.cn121094-20240407-00150","url":null,"abstract":"<p><p><b>Objective:</b> To analyze the clinical characteristics of poisoning inpatients in the first affiliated hospital of chongqing medical and pharmaceutied hospital, and to provide basis for improving the level of prevention, control, diagnosis and treatment of regional poisoning. <b>Methods:</b> In January 2024, using the HIS medical record system of The First Affiliated Hospital of Chongqing Medical and Pharmeceutical College, we collected the clinical data of poisoning inpatients admitted to our hospital from 2018 to 2023. The data of sex, age, time of poisoning, poison species, poisoning cause, poisoning route, season, treatment and prognosis were analyzed. <b>Results:</b> A total of 972 patients were enrolled, among whom 71.71% (697/972) were young and middle-aged adults, with a higher incidence in summer. Chemical poisoning accounted for 53.61% (521/972) , followed by pesticide poisoning and mixed poisoning in the second and third places. The overall detection rate of toxins was 58.23% (566/972) . The number of domestic poisonings increased year by year, while occupational poisonings decreased year by year, with statistically significant differences (<i>P</i><0.05) . The overall cure and improvement rate was 97.22% (945/972) , and the cure and improvement rate of pesticide poisoning was the lowest at 90.23% (120/133) . Among the 46 pesticide occupational hazard factors with occupational exposure limits, only 25 are supported by current valid standard detection methods. {"title":"[Resource distribution and current status of diagnosis and treatment of clinical nuclear medicine in Gansu Province in 2023].","authors":"L M Niu, Y Wang, Y Li, G Liu","doi":"10.3760/cma.j.cn121094-20240329-00130","DOIUrl":"https://doi.org/10.3760/cma.j.cn121094-20240329-00130","url":null,"abstract":"<p><p><b>Objective:</b> To analysis the current utilization of nuclear medicine diagnosis and treatment equipments in Gansu province, evaluate the fairness of its distribution and provide a theoretical basis for government decision-making and the allocation of nuclear medicine diagnosis and treatment resources. <b>Methods:</b> In May 2023, Basic data were collected through questionnaires from the \"Occupational Radiation Disease Monitoring\" and \"Radiation Protection Monitoring in Medical and Health Institutions\" projects undertaken by Gansu Provincial Center for Disease Control and Prevention.The Gini coefficient and Theil index were calculated using STATA SE software to evaluate the equity of clinical nuclear medicine diagnosis and treatment resource allocation. Simultaneously, personal dose monitoring was conducted for nuclear medicine personnel. The fairness of clinical nuclear medicine diagnosis and treatment resource allocation was evaluated using the Gini coefficient and Theil index. <b>Results:</b> As of December 31, 2023, a total of 12 medical institutions in Gansu Province engaged in nuclear medicine diagnosis and treatment, employing 137 nuclear medicine radiation workers. Throughout the province, there were a total of 3 PET/CT scanners, 13 SPECT scanners and 3 medical cyclotrons available. In addition, 33712 nuclear medicine diagnostic procedures and 9365 various types of nuclear medicine treatments were administered over the year, resulting in a diagnostic frequency of 1.35 procedures per thousand population and a treatment frequency of 0.37 procedures per thousand population. The Gini coefficients for nuclear medicine personnel, equipment and diagnostic and treatment applications calculated per capita were 0.50, 0.53 and 0.61, respectively. The corresponding Theil indices were 0.0672, 0.0669 and 0.1277. In 2023, the average annual effective dose from external exposure for nuclear medicine workers was 0.37 mSv per person, while the average annual effective dose from internal exposure to thyroid (131)I for nuclear medicine workers was 1.41 mSv. <b>Conclusion:</b> The distribution of nuclear medicine health resources and applications in Gansu Province is uneven. The general situation of workers, workload factors, social psychological factors and the occurrence of WMSDs were collected by using questionnaires. logistic regression, Extreme Gradient Boosting (XGBoost) , ensemble learning and classification chain model were adopted to explore the key factors influencing WMSDs, and the area under curve (AUC) was used to evaluate the model performance. <b>Results:</b> The incidence of WMSDs among workers in optoelectronic enterprises in the past year was 47.7% (511/1071) , among which the incidence of multi-site WMSDs was 54.4% (278/511) . logistic regression analysis showed that prolonged sitting, personnel shortage and forward neck tilt were risk factors for the occurrence of WMSDs in workers (<i>P</i><0.05) . XGBoost identified the key social psychological factors influencing WMSDs as low mood, mental tension, perceived happiness level, psychological calmness and tranquility. The integrated classification chain model based on the ordered label order had certain efficacy (AUC>0.7) when analyzing WMSDs at the neck, waist, shoulder, back, elbow and hip positions. <b>Conclusion:</b> Workload factors are the main risk factors for the occurrence of WMSDs among workers in optoelectronic enterprises, and social psychological factors also have potential influences. The construction of a classification chain model can accurately identify the occurrence of WMSDs in multiple parts. The study conducted surveys of workers on the use of HPD. The 3M(TM) E-A-R Fit(TM) binaural verification system was used to measure the personal attenuation device (PAR) of workers wearing HPD. <b>Results:</b> The <i>M</i> (<i>Q</i>(1), <i>Q</i>(3)) of baseline PAR obtained by 3634 workers was 12 (2, 19) dB. There was a statistically significant difference in baseline PAR among the three types of HPDs (<i>H</i>=336.39, <i>P</i><0.01) . After pairwise comparison, it was found that the baseline PAR of workers wearing foam earplugs and earmuffs was higher than that of pre-molded earplugs (<i>P</i><0.01) . There were differences in baseline PAR among workers in different industries (<i>Z</i>=359.73, <i>P</i><0.01) . Education level, age of using HPD, types of HPDs, noise exposure intensity, with or without knowledge of correct methods, and comfort evaluation were the main factors affecting baseline PAR (<i>P</i><0.05) . There were 1536 workers (43.4%) failed the baseline PAR test. After the intervention, the median PAR increased significantly from 1 (0, 6) dB (baseline) to 18 (14, 22) dB (after the intervention) (<i>P</i><0.01) . The follow-up test found that the follow-up PAR of 328 workers was higher than the baseline PAR of the initial test, and the follow-up PAR was higher than the post-intervention PAR of the initial test (<i>P</i><0.01) . <b>Conclusion:</b> Under the conditions of this study, the protective effect of HPD was affected by factors such as incorrect understanding of wearing methods, exposure to high-intensity noise, low education level, a short period of time of HPD use and low comfort of hearing protectors. The protective effect could be improved through training, optimizing the wearing of hearing protector models, and follow-up interventions. Severe poisoning cases can lead to respiratory failure, shock, and even death. There are relatively few reports on thiamethoxam poisoning, and few literature reports on concurrent rhabdomyolysis cases have been found so far. This article reviews the treatment process of a case of oral thiamethoxam poisoning. The patient developed mild rhabdomyolysis (CK: 1581 U/l) and renal dysfunction (creatinine: 127 μmol/L) immediately after taking the medication, reaching its peak on the second day (CK: 16329 U/l) . After active treatment (such as Blood purification, drainage, gastrointestinal decompression and organ support) , the symptoms of rhabdosis gradually improved and the patient was discharged. It was separated by C18 column and eluted with 5 mmol/L ammonium acetate water and 5 mmol/L ammonium acetate methanol solution as mobile phase gradient. The metabolites of six psychotropic drugs were qualitatively and quantitatively by using electrospray positive ion multi-reactive ion monitoring scanning mode (MRM) . <b>Results:</b> The linear relationships of six psychotropic drug metabolites were good in the concentration range of 2-100 μg/L. The diagnosis of occupational decompression sickness includes acute decompression sickness and decompression osteonecrosis. Corresponding treatment measures should be taken according to the symptoms, signs and severity of decompression disease. {"title":"[Analysis of health effects of occupational exposure to Dimethylformamide].","authors":"Q Q Gao, Z J Jiang, L Zhou, L Han, Y Zhao","doi":"10.3760/cma.j.cn121094-20240325-00118","DOIUrl":"https://doi.org/10.3760/cma.j.cn121094-20240325-00118","url":null,"abstract":"&lt;p&gt;&lt;p&gt;&lt;b&gt;Objective:&lt;/b&gt; To analyze the abnormal results of health surveillance for workers exposed to dimethylformamide (DMF), and to provide reference for the formulation of relevant policies and standards. &lt;b&gt;Methods:&lt;/b&gt; In April 2024, 11, 224 workers who participated in occupational health examinations in Jiangsu Province in 2023 were selected as the research subjects. Among them, 5, 615 people exposed to DMF were taken as the exposure group, and 5, 609 people not exposed to hepatotoxic chemicals were taken as the control group. By inquiring about related symptoms and combining with the occupational health examination of workers, the abnormal rates of blood pressure, blood routine, urine routine, electrocardiogram, liver function and B-ultrasonography of the survey subjects were statistically analyzed. &lt;b&gt;Results:&lt;/b&gt; Compared with the control group, the abnormal detection rates of indicators such as blood pressure, blood routine, urine routine, and electrocardiogram were higher, and the difference was statistically significant &lt;i&gt;P&lt;/i&gt;&lt;0.05). The abnormal rates of various liver function indicators in the DMF exposure group were all higher than those in the control group. Among them, the abnormal rates of alanine aminotransferase, total protein, total bilirubin, and liver B-ultrasound in the exposure group were all higher than those in the control group, and the differences were statistically significant (&lt;i&gt;χ&lt;/i&gt;(2)=34.88, 42.49, 43.07, 55.28, &lt;i&gt;P&lt;/i&gt;&lt;0.001). Compared with the control group, the detected values of alanine aminotransferase, total protein and total bilirubin in the DMF exposure group were higher, and the difference was statistically significant (&lt;i&gt;F&lt;/i&gt;=5.367, 29.543, 37.766, &lt;i&gt;P&lt;/i&gt;&lt;0.05). In the DMF exposure group, the abnormal rates of liver function, electrocardiogram and liver B-ultrasound in men were much higher than those in women, and the differences were statistically significant (&lt;i&gt;χ&lt;/i&gt;(2)=185.05, 10.06, 141.94, &lt;i&gt;P&lt;/i&gt;&lt;0.001, &lt;i&gt;P&lt;/i&gt;=0.002, &lt;i&gt;P&lt;/i&gt;&lt;0.001). Grouped by length of service, the abnormal detection rate of liver function was similar in each length of service segment, and the difference was not statistically significant (&lt;i&gt;χ&lt;/i&gt;(2)=0.34, &lt;i&gt;P&lt;/i&gt;&gt;0.05). The abnormal detection rate of electrocardiogram increased with the increase of length of service, among which it was the highest in the 10-20 years of service, and the difference was statistically significant (&lt;i&gt;χ&lt;/i&gt;(2)=7.26, &lt;i&gt;P&lt;/i&gt;=0.026). The abnormal rate detected by liver B-ultrasound significantly increased with the increase of working years, and was the highest in the section of ≥20 years of working years. The difference was statistically significant (&lt;i&gt;χ&lt;/i&gt;(2)=44.15, &lt;i&gt;P&lt;/i&gt;&lt;0.001) . &lt;b&gt;Conclusion:&lt;/b&gt; Occupational exposure to DMF can affect the health of workers, especially increasing the detection rate of abnormal liver function and liver B-ultrasound.
