A total of 1134 questionnaires were distributed, with 1000 valid questionnaires collected, yielding an effective response rate of 88.18%. The Core Occupational Stress Scale (COSS), the Patient Health Questionnaire-9 (PHQ-9), and the Generalized Anxiety Disorder-7 (GAD-7) were adopted to assess occupational stress, depressive symptoms, and anxiety symptoms, respectively. Binary logistic regression was employed to analyze influencing factors of occupational stress and its effects on depressive and anxiety symptoms. <b>Results:</b> The detection rates of occupational stress, depressive symptoms, and anxiety symptoms were 15.1% (15/1000), 7.9% (79/1000), and 13.8% (138/1000), respectively. The influencing factor analysis of occupational stress revealed that female gender (<i>OR</i>=0.621, 95%<i>CI</i>: 0.434-0.890) and weekly working hours of 45-48 hours (<i>OR</i>=0.537, 95%<i>CI</i>: 0.309-0.935) were protective factors (<i>P</i><0.05), while weekly working hours ≥55 hours (<i>OR</i>=2.176, 95%<i>CI</i>: 1.290-3.670) and shift work (<i>OR</i>=2.038, 95%<i>CI</i>: 1.412-2.940) were risk factors (<i>P</i><0.05). Spearman correlation analysis showed that the COSS score was positively related with the PHQ-9 score and the GAD-7 score (<i>r</i>(s)=0.438, 0.289, <i>P</i><0.01). Workers judged with occupational stress had 3.596 times (95%<i>CI</i>: 2.181-5.931, <i>P</i><0.001) and 3.121 times (95%<i>CI</i>: 2.046-4.761, <i>P</i><0.001) higher risks of detecting depressive and anxiety symptoms compared to those without occupational stress. <b>Conclusion:</b> Electronic manufacturing workers in Qingdao experience a certain level of occupational stress, which may increase the risk of developing depressive and anxiety symptoms. Qualitative analysis was conducted on the organic solvents used by enterprises in their production processes, and the detection results of 1, 2-DCE were statistically analyzed. <b>Results:</b> A total of 88 organic solvent samples were collected, with a qualitative detection rate of 21.6% (19/88) for 1, 2-DCE. A total of 30 enterprises using organic solvents were surveyed and tested, with an enterprise detection rate of 36.7% (11/30) for 1, 2-DCE. The workplace exceedance rate for 1, 2-DCE was 37.0% (34/92), and the post exceedance rate was 15.4% (4/26) . <b>Conclusion:</b> The degree of occupational exposure hazard related to 1, 2-DCE remains severe. A multidimensional and hierarchical assessment system was designed based on the two core elements of \"comprehensive prevention-on-site hazards.\" <b>Results:</b> The consultation results from 20 authoritative experts demonstrated that the expert authority coefficient (0.84), coordination coefficient (0.73), and reliability coefficient (0.83) all reached robust levels. The importance scores of the indicators (ranging from 3.47 to 5.59 points) exhibited clear discriminative validity. Ultimately, an assessment system comprising 2 first-level indicators, 5 second-level indicators, and 14 third-level indicators was established. <b>Conclusion:</b> The index system developed in this study demonstrates high scientific rigor and practical applicability. Occupational health investigation, on-site detection, and physical simulation of air distribution were used to analyze the distribution of the xylene concentrations in the impregnation room. In addition, the causes for excessive concentrations were analyzed, and the combination ventilation design of enclosed negative pressure cabinets and jet directional supply of fresh air was proposed. The concentration of xylene, the capture velocity and the differential pressure of ventilation facilities before and after the transformation were tested on the spot. The rationality of the airflow organization was determined through smoke testing, and the ventilation protection effects before and after the transformation were analyzed. <b>Results:</b> The time-weighted average allowable concentration (<i>C</i>(TWA)) of xylene was reduced from 41.73 mg/m(3) to 7.78 mg/m(3), and the <i>C</i>(TWA) of xylene were reduced by 81.4%. The values of capture velocity at all areas were higer than 0.5 m/s, and the values of velocity unevenness in all regions were lower than or equal to 0.25. The differential pressure between the enclosed cabinet and the impregnation room corridor was -2 to -1 Pa. Eighty cases exhibiting a decline in forcect expiratory volume in one second (FEV(1)) and pulmonary imaging changes (primarily ground-glass opacity variations) were chosen as study subjects. These patients were randomly divided into a control group and an experimental group using the sealed envelope method, with 40 patients in each group. The control group received conventional outpatient follow-up and occupational health surveillance management, while the experimental group underwent a remote occupational health management model combining \"group+individual\" approaches over 12 months, with interventions administered once every 3 months. Comparative analysis was conducted on pre-and pos-management indicators between the two groups, including pulmonary function, pulmonary imaging changes, and self-management ability scores. <b>Results:</b> After occupational health management, the experimental group showed superior improvement in observed indicators (including pulmonary function and pulmonary imaging indicators) compared to the control group (<i>P</i><0.05). Underwent systematic treatment and a 2-3 week pseudo-healing period, some ACMP patients may still develop delayed encephalopathy after acute carbon monoxide poisoning (DEACMP). DEACMP is the most severe complication that could happen to ACMP patients and comes with an exceeding high disability rate. Early identification and adequate intervention measures of DEACMP are particularly crucial for preventing its occurrence in clinical practice. At present, multiple studies have found that after ACMP occurred, a series of biomarkers showed predictive value for detecting the occurrence and development of DEACMP. {"title":"[Analysis of characteristics of carpal tunnel syndrome outpatients with complaints of \"numbness and pain in the hands\"].","authors":"Y M Zheng, S Q Li, D X Li, R J Ling, L Guan","doi":"10.3760/cma.j.cn121094-20240116-00020","DOIUrl":"https://doi.org/10.3760/cma.j.cn121094-20240116-00020","url":null,"abstract":"<p><p><b>Objective:</b> To analyze the situation of outpatient patients diagnosed with carpal tunnel syndrome (CTS) due to typical hand symptoms and evaluate the diagnostic factors of CTS patients. <b>Methods:</b> Through an electronic medical record information system, a retrospective analysis was conducted on the data of outpatient patients diagnosed with CTS with \"numbness and pain in the hands\" as the complaints from January 1, 2018 to December 31, 2022 in eight medical institutions including Peking University Third Hospital, Third People's Hospital of Guizhou Province, Zhejiang Provincial People's Hospital Bijie Hospital, then the demographic characteristics and diagnostic factors such as symptoms, signs, and auxiliary examinations were analyzed. Chi-square test was used to compare the differences in CTS diagnosis rates between different genders and age groups. <b>Results:</b> There were a total of 43105 outpatients with complaints of \"numbness and pain in the hands\" over the past 5 years, of whom 754 were diagnosed with CTS, with a diagnostic rate of 1.7%. The there were 166 males and 588 females diagnosed with CTS, and the diagnostic rate of female patients was significantly higher than that of male patients (<i>χ</i>(2)=158.13, <i>P</i><0.001). Among the CTS patients, the proportion of patients aged 45 to 60 was the highest (43.8%, 330/754), and there was a statistically significant difference in CTS diagnosis rates among patients of different age groups (<i>χ</i>(2)=36.99, <i>P</i><0.001). All CTS patients had varying degrees of hand symptoms, and some had definite abnormalities in auxiliary examinations such as physical examination, wrist ultrasound, MRI or neuroelectrophysiology. <b>Conclusion:</b> The diagnosis rate of CTS among outpatients presenting with complaints of \"numbness and pain in the hands\" is relatively low, and there are gender and age distribution differences. {"title":"[Analysis of the potential disease burden of occupational polycyclic aromatic hydrocarbons exposure-associated lung cancer in China from 1990 to 2021].","authors":"J Y Zhao, Y Liu, N Shang","doi":"10.3760/cma.j.cn121094-20240912-00428","DOIUrl":"https://doi.org/10.3760/cma.j.cn121094-20240912-00428","url":null,"abstract":"<p><p><b>Objective:</b> To assess the disease burden of lung cancer associated with occupational exposure to polycyclic aromatic hydrocarbons (PAHs) in China from 1990 to 2021 using the 2021 Global Burden of Disease (GBD) data. <b>Methods:</b> In July 2024, data on occupational PAHs exposure-associated lung cancer from 1990 to 2021 were extracted from the GBD database, including mortality rates, age-standardized mortality rates, number of deaths, disability-adjusted life years (DALYs) rates, age-standardized DALYs rates, and number of DALYs for different genders and ages. Descriptive statistics were used to analyze trends in lung cancer mortality rates, age-standardized mortality rates, DALYs rates, and age-standardized DALYs rates over different years, genders, and ages. Joinpoint regression models were used to calculate the annual percent change (APC) and average annual percent change (AAPC), and to identify change points and trends in different periods within the data. <b>Results:</b> From 1990 to 2021, the mortality rate of lung cancer associated with occupational PAHs exposure increased from 0.085 per 100000 to 0.259 per 100000, and the age-standardized mortality rate increased from 0.107 per 100000 to 0.162 per 100000. The DALYs rate increased from 2.746 per 100000 to 7.389 per 100000, and the age-standardized DALYs rate increased from 3.330 per 100000 to 4.679 per 100000. The number of lung cancer deaths and DALYs both peaked in the 65-69 age group. Joinpoint regression analysis showed significant upward trends in mortality, age-standardized mortality, DALYs rate, and age-standardized DALYs rate from 1990 to 2021, with AAPCs of 3.655%, 1.382%, 3.256%, and 1.108%, respectively (all <i>P</i><0.001). From 2016 to 2021, both the mortality rate and age-standardized mortality rate increased, with APCs of 4.163% and 1.735%, respectively. From 2015 to 2021, the DALYs rate increased (APC: 3.137%), and from 2016 to 2021, the age-standardized DALYs rate also rose (APC: 1.578%). For females, both mortality and DALYs rates increased from 2015 to 2021 (APCs: 6.269% and 5.426%, respectively). Among males, the mortality rate increased from 2018 to 2021 (APC: 3.477%) . <b>Conclusion:</b> From 1990 to 2021, the disease burden of lung cancer related to occupational PAHs exposure in China has significantly increased, particularly among the elderly population. The purification of target metals can be achieved through the volatilization process of heated amalgam. This report presents a case of toxic encephalopathy caused by acute mercury poisoning. The Department of Occupational Medicine and Toxicology at Beijing Chaoyang Hospital, affiliated with Capital Medical University, admitted and successfully treated a clinical case of toxic encephalopathy resulting from acute exposure to metallic mercury vapor. The nursing protocol and clinical outcomes are detailed herein. {"title":"[A systematic review on the clinical characteristics and treatment outcomes of accelerated silicosis].","authors":"L M Huang, S W Zhou, Z D Chen, J Shi, L Q Bian, L Mao","doi":"10.3760/cma.j.cn121094-20250110-00016","DOIUrl":"https://doi.org/10.3760/cma.j.cn121094-20250110-00016","url":null,"abstract":"<p><p><b>Objective:</b> To explore the epidemiological characteristics, imaging findings, pulmonary function changes, dust exposure situations, and treatment outcomes of accelerated silicosis through an analysis of existing literature. <b>Methods:</b> In December 2024, relevant literature from January 1, 1965 to December 15, 2024 was retrieved through the China National Knowledge Infrastructure (CNKI), Wanfang Database, VIP Database, EMbase, and PubMed databases. Keywords included \"rapidly progressive silicosis\" \"accelerated silicosis\" \"subacute silicosis\"\"artificial stone\" and related terms. By incorporating, analyzing, and retrieving data from literatures, a comprehensive review of the epidemiology, clinical features, treatment options, and prognosis of fast forward silicosis was conducted. <b>Results:</b> A total of 72 literatures were included, including 23 cohort studies, 28 case reports, 3 case-control studies, and 18 cross-sectional studies. The average age of all 1794 patients was 33.67 years, with an average dust exposure duration of 5.58 years. The primary occupations associated with accelerated silicosis were stone processing, mining, and artificial quartz stone manufacturing. Imaging findings predominantly included small nodules, ground-glass opacities, and massive fibrosis. Antifibrotic treatment at the early stage of the disease could clearly delay disease progression. However, dust concentrations in workplaces were significantly above safety limits, with inadequate protective measures. <b>Conclusion:</b> Accelerated silicosis is characterized by its rapid onset, swift progression, and unfavorable prognosis. However, it has not garnered adequate attention in the present context.
