Due to its broad insecticidal activity, low resistance, rapid action, and long-lasting effects, its use has been increasing. In the past three years, there have been incidents of imidacloprid poisoning in China. We reviewed and analyzed the clinical characteristics of patients treated for chlorfenapyr poisoning at the People's Hospital of Chuxiong Yi Autonomous Prefecture from November 2019 to November 2023. The 23 patients included 15 males and 8 females, with ages ranging from 11 to 71 years. Twenty-two cases were due to gastrointestinal exposure, and one case was due to skin or respiratory tract exposure. Clinical symptoms included nausea, vomiting, fever, profuse sweating, and altered consciousness. Laboratory tests showed elevated levels of aspartate aminotransferase and creatine kinase. There was no specific antidote available, and 10 patients died after treatment, resulting in a mortality rate of 43.5%. The patient presented with multiple organ dysfunction syndrome, manifested as consciousness disorders, abnormal coagulation function with hematuria, and delayed liver injury. After comprehensive treatment including fluid replacement, diuresis, protection of vital organs, and nutritional support, the patient was discharged after recovery. Poisoning caused by oral administration of large doses of benzene is rare. {"title":"[Investigation and analysis of the etiology of suspected occupational asthma caused by toluene diisocyanate].","authors":"Y Wu, Y C He, J Zhang, Y L Song, Y Hua, Z H Zhou","doi":"10.3760/cma.j.cn121094-20240507-00198","DOIUrl":"10.3760/cma.j.cn121094-20240507-00198","url":null,"abstract":"<p><p><b>Objective:</b> To conduct an occupational hygiene investigation and simulated sampling and testing on welding positions in a company in a certain province, and to perform a simulated analysis of the concentration of occupational hazard factors in workers suspected of occupational asthma. <b>Methods:</b> In November 2023, the investigation and analysis of workers' exposure to occupational hazard factors were carried out through occupational hygiene surveys, laboratory simulated sampling and testing, and a combination of qualitative and quantitative detection methods. <b>Results:</b> Toluene diisocyanate (TDI) was present in the workplace air of the employer. TDI was detected in the raw materials provided by the workers. Laboratory simulations of the production process and working conditions at the position were conducted with sampling at different distances, revealing a maximum TDI concentration of 0.49 mg/m(3) and a minimum concentration of 0.01 mg/m(3). The results showed that the closer the sampling distance was to the breathing zone, the more significantly the TDI concentration increased. The relationship between N-methylcarbyl hemoglobin adduct (NMHb) and age and serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), AST/ALT and gamma-glutamyl transpeptidyase (γ-GT) was analyzed by logistic regression, interaction effect analysis, and restricted cubic spline models. <b>Results:</b> The median age of 388 workers was 45.98 years old, the median concentration of NMHb was 165.50 nmol/g Hb, and the abnormal rate of liver function was 40.7% (158/388). NMHb and age of DMF-exposed workers were the influencing factors for liver dysfunction (<i>P</i><0.05), and the multiplicative interaction between the two was not statistically significant (<i>P</i>=0.303), nor was the additive interaction statistically significant [relative excess risk due to interaction (RERI) =0.17 (95%<i>CI</i>: -2.35, 2.69), attributable proportion (AP) =0.03 (95%<i>CI</i>: -0.47, 0.54), synergy index (S) =1.04 (95%<i>CI</i>: 0.54, 2.02) ]. NMHb concentration was positively correlated with the abnormal risk of ALT, AST, AST/ALT, γ-GT and the concentration of ALT, AST, γ-GT (<i>P</i>(overall) <0.05, <i>P</i>(nonlinear)>0.05), and negatively correlated with AST/ALT (<i>P</i>(overall)=0.032, <i>P</i>(nonlinear)=0.781). Age was negatively correlated with abnormal risk of ALT, AST/ALT and ALT concentration (<i>P</i>(overall)<0.001, <i>P</i>(nonlinear)>0.05), and positively correlated with AST/ALT (<i>P</i>(overall)=0.025, <i>P</i>(nonlinear)=0.485) . <b>Conclusion:</b> There is a close relationship between NMHb and age of DMF-exposed workers and abnormal liver function. γ-GT can be used as a sensitive index, and special attention should be paid to the change of ALT index in elderly workers. In this paper, the treatment process of 12 patients with acute 2- (2-thiophenyl) scopolamine glycolate poisoning was reviewed and their clinical data were analyzed. After a large amount of 2- (2-thiophenyl) scopolamine glycolate was absorbed through the respiratory tract, the poisoning patients presented with central nervous system injury as the main clinical manifestations, accompanied by symptoms such as blurred vision and irritability. After fluid replacement, ovulation induction and symptomatic supportive treatment, all patients improved and were discharged. The patients were followed up. After 6 months, only 3 patients had residual headache and dizziness. It is indicated that early intervention for acute 2- (2-thiophenyl) scopolamine glycolate poisoning has a good prognosis. The operability of GBZ 158 is not strong, and there is a certain overlap with the signs in the fields of firefighting, safety and other fields. It urgently needs to be modified to comply with the reality of occupational health management in China. There are many and concentrated revision opinions on \"6. Notification Cards for Occupational Hazards of Toxic Substance in Workplaces\" and \"7. Setting of Warning Signs in Workplaces with Toxic Substances\". Their health examination results and occupational environmental data were collected. A total of 3, 320 coal miners were included. Randomly divide the research subjects into a training set (2324 people) and a validation set (996 people) in a ratio of 7∶3, and the balance of the two sets was tested. Perform LASSO regression analysis using R 4.2.2 software to select relevant important variables, and determine the model's input variables by combining them with relevant literature. Utilize Python 3.8 to construct logistic regression, random forest, support vector machine, and XG Boost models, assess the models' discriminative ability using metrics like accuracy, sensitivity, specificity, F1 score, ROC curve, and AUC, evaluate the models' calibration using Brier score, Log loss score, and calibration curve, and further analyze the clinical performance of the developed models through DCA decision curve analysis. <b>Results:</b> Among the 3 320 coal miners, 856 had abnormal lung function (25.78%). The XG Boost model was identified as the optimal model, achieving a training set accuracy of 87.39%, sensitivity of 86.60%, specificity of 87.67%, F1 score of 0.779, AUC of 0.945, Brier score of 0.071, Log loss of 0.267 and demonstrated good calibration curve consistency. <b>Conclusion:</b> The XG Boost model exhibits superior predictive performance compared to other models, and the model has high application value. Existing living pneumoconiosis patients in kashgar were selected as the research objects. The \"Survey Questionnaire on Medical Behaviors and Influencing Factors of Pneumoconiosis Patients\" developed by the National Institute of Occupational Health and Poison Control of China Centers for Disease Control and Prevention was used to collect pneumoconiosis patients infomations from March to October 2021. This study investigated the demographic characteristics of pneumoconiosis patients, the types and stages of pneumoconiosis, as well as hospitalization in the past year, reasons for hospitalization, treatment measures, reasons for discharge. Additionally, it examined factors influencing medical behaviors, including medical insurance reimbursement, work-related injury insurance, and compensation situations.Logistics regression was used to analyze the influencing factors of hospitalization utilization. <b>Results:</b> The average age of the pneumoconiosis patients was elder with the age of 64.8. 86.7% (156/178) of them were retired, 75.8% (135/178) of them relied primarily on pensions for their income. The main types of pneumoconiosis were silicosis and coal workers' pneumoconiosis with the proportion of 57.3% (102/178) and 24.2% (43/178). 67.4% of them were stage I. The hospitalization rate in the year prior to the survey was 37.1%. The factors that promoted hospitalization were pension as the main source of income, pneumoconiosis in stage II, silicosis as the pneumoconiosis type, suffering joint pain, with two OR more other lung diseases (tuberculosis, cor pulmonale, COPD, pulmonary infection, bulla or pneumothorax, lung cancer, or mesothelioma) (<i>P</i><0.05) . <b>Conclusion:</b> The hospitalization rate of pneumoconiosis patients in the previous year was low. Three follow-up surveys were subsequently conducted in April 2019, April 2020, and January 2021. The presence of renal dysfunction in the study population was determined based on the glomerular filtration rate tested at the medical examination hospital. The exposure of the study subjects to shift work was assessed using the weighted shift index (WSI), the relationship between different levels of shift work and renal dysfunction was analyzed using Cox regression, and the measure of WSI and renal dysfunction was explored by restricted cubic spline (RCS). response relationship. <b>Results:</b> A total of 2292 study participants were included in this study, and the prevalence density of renal dysfunction was 87.44 k/year, of which the prevalence of renal dysfunction in females (30.31%), those with per capita monthly income <2, 000 yuan (27.00%), those who consume alcohol (27.10%), those who are hypertensive (23.05%), those who are exposed to high temperatures (27.37%), those who are exposed to organic solvents (30.42%), and those who are engaged in shift work (25.87%) were to be found had a higher prevalence of renal dysfunction (<i>P</i><0.05). After correcting for age, sex, and other risk factors, there was a nonlinear association between intensity of shift work and renal dysfunction, with a hazard ratio (<i>HR</i>) of 1.29 (95%<i>CI</i>: 0.98-1.59) for the development of renal dysfunction in petroleum workers for shift work performed at higher intensities, and moderate intensity of exposure to shift work reduced the risk of renal dysfunction in petroleum workers (<i>HR</i>=0.54 with a 95%<i>CI</i>: 0.39-0.75, <i>P</i><0.001) . <b>Conclusion:</b> Prolonged shift work increases the risk of renal dysfunction in oil workers, and the occurrence of renal dysfunction in oil workers is influenced by multiple factors.</p>","PeriodicalId":23958,"journal":{"name":"中华劳动卫生职业病杂志","volume":"43 5","pages":"379-384"},"PeriodicalIF":0.0,"publicationDate":"2025-05-20","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144226838","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
