The first part of the present review described procedures that utilize autogenous ocular tissues, homologous donor tissues, and heterologous donor tissues in dogs, while the second part reviewed the use of biomaterials and keratoprosthetics in this species. This third part is dedicated to the review of the use of corneal sutures including suture type and suture pattern in corneal reconstruction of small animals including dogs and cats. The review also focused on the way studies report postoperative ocular discomfort/pain and how this is treated. {"title":"Contrast computed tomography dacryocystorhinography identifies nasolacrimal duct obstruction secondary to chronic dental disease in two chinchilla langeria.","authors":"Erin A Hisey, Paula Rodriguez, Danielle K Tarbert, Joanne R Paul-Murphy, Carli Hendrickson, Kelsey Brust, David J Maggs, Brian C Leonard","doi":"10.1111/vop.13281","DOIUrl":"https://doi.org/10.1111/vop.13281","url":null,"abstract":"<p><strong>Objective: </strong>To determine the impact of chronic dental disease on the nasolacrimal duct of chinchillas using contrast CT dacryocystorhinography.</p><p><strong>Animals studied: </strong>Two 12-year-old female chinchillas with uni- or bilateral ocular discharge and a history of chronic, moderate (Chinchilla 1, one-year) or severe (Chinchilla 2, three-years) dental disease.</p><p><strong>Procedures: </strong>Contrast CT dacryocystorhinography was performed to identify abnormalities in the nasolacrimal duct and dentition, and to correlate those changes.</p><p><strong>Results: </strong>Chinchilla 1 had a focal soft tissue attenuating expansion of the maxillary bone rostral to the first left premolar interpreted as possible abscessation causing deviation of the nasolacrimal duct over its dorsomedial margin. The right nasolacrimal duct appeared normal. Chinchilla 2 had periapical abscessation of a retained subgingival left maxillary incisor fragment which extended into the nasal cavity causing focal narrowing and distal dilation of the left nasolacrimal duct. Complete contrast infusion of the right nasolacrimal duct could not be completed on Chinchilla 2. A focal area of superficial corneal fibrosis ipsilateral to the obstructed nasolacrimal duct was also identified in Chinchilla 2. Treatment consisted of occlusal adjustments to correct the coronal elongation, systemic antibiotics (metronidazole and either marbofloxacin or azithromycin), and topical tear replacement therapy and diclofenac as needed. Ocular discharge decreased in both chinchillas but did not resolve long-term in either animal.</p><p><strong>Conclusions: </strong>Chronic dental disease including periapical abscessation in chinchillas can obstruct the nasolacrimal duct, leading to impaired tear drainage. Each animal received one drop of 0.5% tropicamide or 1% atropine in a randomly selected eye. PD, IOP, and TP were evaluated before drug instillation and at 0.25 h, 0.5 h, 0.75 h, 1 h, 2 h, 4 h, 6 h, 8 h, 10 h, 12 h, 24 h, 36 h, 48 h, 60 h, and 72 h post-instillation. Data were analyzed using a paired two-sample repeated measures T-test with Bonferroni correction.</p><p><strong>Results: </strong>In both tropicamide and atropine treated eyes, the mean PD significantly increased from 15 min until 12 h after treatment with a maximum PD at 45 min (+2.7 mm and + 2.4 mm respectively). Following tropicamide and atropine instillation, IOP increased significantly in treated eyes at 45 min (+2.9 mmHg) and 15 min (+5.2 mmHg) respectively, compared to untreated eyes. No significant effects were found on TP, in both tropicamide and atropine treated eyes at any time. Nine stranded animals were found alive, 10 dead.</p><p><strong>Procedures: </strong>Carcasses were classified according to decomposition state. Globes were assessed ultrasonographically, biometric measurements were taken and then the tissues were sectioned for histological analysis.</p><p><strong>Results: </strong>Seventeen carcasses were classified as decomposition code 2 and two as code 3. Fifteen animals were calves, three juveniles and one adult, twelve males, and five females. The cornea was thinner in the center of the eye and thicker at the periphery. The lens had an oval shape. Fourteen animals showed a structure in the vitreous body which looked triangular and filamentous by ultrasonography and, histologically, originated in the central part of the optic nerve and extended to the posterior region of the lens, composed of connective tissue. The most common abnormalities found by ultrasonograph were retinal detachment (n = 13) and displacement of the lens into the vitreous (n = 4), along with alterations suggestive of hemorrhage, fibrin deposits, and increased echogenicity in the optic nerve (n = 2).</p><p><strong>Conclusions: </strong>An intraocular structure not previously described in cetaceans was found in this investigation; its function remains unknown. The findings from the IVCM examination were correlated with clinical details from ophthalmic examinations and diagnostic test results.</p><p><strong>Results: </strong>Eight eyes from eight dogs with unilateral primary corneal SCC were examined. Corneal lesions were characterized with IVCM by abnormal epithelial cells that were polygonal and enlarged with marked morphological variability and anisocytosis within individual corneas. The abnormal cells displayed variable reflectivity, but most had highly reflective cellular borders and moderately reflective cytoplasm. Cells that were markedly and diffusely hyperreflective were also occasionally observed possibly representing keratinization and dyskeratosis. On IVCM, neoplastic cells were grouped into cords, clusters, and sheets, and nests of neoplastic cells often invaded the underlying corneal stroma. Keratin pearls were a frequent finding and appeared as circular whirls of neoplastic epithelial cells arranged around concentric layers of hyperreflective amorphous material within a dark cyst-like space. Abundant blood vessels and scattered leukocytes were present in all tumors. Intracameral tPA (25 μg/0.1 mL) was injected into tPA group eyes before corneal incision closure but not into the eyes of the control group. The grades of anterior fibrin formation and PCO were compared based on slit lamp biomicroscope examination at 1 and 2 weeks, 1 month, and 2–3 months postoperatively. IOP was measured using applanation tonometry every 30 min for 4 h immediately after operation and on the following morning. The IOP of the two groups at each time was compared.ResultsThe grade of anterior fibrin formation and that of PCO were not significantly different between the two groups at any time point (<jats:italic>p</jats:italic> &gt; .05). However, the IOP of the tPA group was significantly lower than that of the control group at each point on the day of surgery (<jats:italic>p</jats:italic> &lt; .05). No complications were observed with tPA injection, except for temporary hyphema (for 3 days) in one eye.ConclusionsAlthough the intracameral tPA injection did not affect anterior fibrin formation and PCO, it effectively maintained normal IOP immediately after phacoemulsification. All animals underwent complete ophthalmic examination. The Schirmer tear test ‐I (STT‐I) was performed for all horses prior to other diagnostic tests. Neuro‐ophthalmic examinations (menace response, dazzle reflex, pupillary light reflex, maze test, and visual tracking) were carried out in different lighting situations (bright and dim lights). Slit lamp biomicroscopy, direct and indirect ophthalmoscopy, fluorescein staining, and tonometry were performed for each horse.ResultsThe mean (standard deviation) age of horses was 5.7 (4.6) years (range of 1–17 years). In total, 18 horses (32.7%) had ocular‐related lesions. Cataract, iris hyperpigmentation and corneal edema were the most identified conditions in the horses of this study.ConclusionsAbout one‐third of studied horses had at least one ocular abnormality. {"title":"Evaluation of ocular surface parameters before and after cryo- and laser therapy for distichiasis in dogs: A pilot study.","authors":"V Zwiauer-Wolfbeisser, A Tichy, B Nell","doi":"10.1111/vop.13275","DOIUrl":"https://doi.org/10.1111/vop.13275","url":null,"abstract":"<p><strong>Objective: </strong>The aim of this pilot study was to establish a satisfactory, scientific approach to effectively compare quantitative measurements of various ocular surface parameters before and after surgical treatment in dogs suffering from distichiasis.</p><p><strong>Methods: </strong>An ophthalmic examination was conducted on 12 dogs (23 eyes) before and after surgical treatments for distichiasis, at four different time points, (t<sub>0</sub> = before surgery, t<sub>1</sub> = 1-2 h after surgery, t<sub>2</sub> = 1 week after surgery, t<sub>3</sub> = 1 month after surgery, and t<sub>4</sub> = 6 months after surgery) between 2021 and 2022, and analyzed retrospectively. The examination included Schirmer tear test-1 (STT-1), interferometry, noninvasive tear film break-up time (NIBUT), tear meniscus size (TMS), and meibography.</p><p><strong>Results: </strong>No statistically significant changes were found in STT-1 (t<sub>0</sub>: 22.2 ± 6.5 mm, t<sub>2</sub>: 22.5 ± 5.7 mm, t<sub>3</sub>: 20.8 ± 5.1 mm, and t<sub>4</sub>: 22.6 mm ± 4.8 mm) before and after surgery. Mean interferometry scores showed a slight, not statistically significant, decrease from t<sub>0</sub> to t<sub>3</sub> and t<sub>4</sub> (t<sub>0</sub>: 2.1 ± 0.8, t<sub>1</sub>: 2.1 ± 0.7, t<sub>2</sub>: 2.1 ± 0.9, t<sub>3</sub>: 1.8 ± 0.6, and t<sub>4</sub>: 1.9 ± 1.1). Mean NIBUT did not change significantly between time points (t<sub>0</sub>: 3.9 ± 1.3 s, t<sub>1</sub>: 4.0 ± 1.3 s, t<sub>2</sub>: 4.0 ± 1.4 s, t<sub>3</sub>: 3.5 ± 0.7 s, and t<sub>4</sub>: 3.5 ± 0.9 s). TMS showed a slight, not statistically significant increase (t<sub>0</sub>: 0.5 ± 0.3 mm, t<sub>1</sub>: 0.5 ± 0.3, t<sub>2</sub>: 0.5 ± 0.3 mm, t<sub>3</sub>: 0.6 ± 0.2 mm, and t<sub>4</sub>: 0.7 ± 0.3 mm). There were no changes in the gross morphology of the meibomian glands (MG).</p><p><strong>Conclusion: </strong>This pilot study could not detect a negative effect of different forms of treatment of distichiasis on the precorneal tear film parameters in dogs. Teresa Peña","doi":"10.1111/vop.13270","DOIUrl":"https://doi.org/10.1111/vop.13270","url":null,"abstract":"PurposeTo describe the use of coronofrontal rhytidectomy (CFR) for the treatment of severe pseudoptosis and superior entropion in dogs, and to provide guidelines for the selection of surgical technique depending on presentation.MethodsA review of medical records of dogs that underwent rhytidectomy from 2002 to 2023 was carried out, including signalment, clinical signs, type of rhytidectomy, concurrent surgical techniques, re‐interventions, post‐operative complications, follow‐up time, and outcome.ResultsTwenty dogs with a median age of 5.1 years were included in this study. English Cocker Spaniel was the most common breed (8 dogs:40%) and males were overrepresented (13 dogs: 65%). Besides pseudoptosis and visual impairment (100%), the other most common clinical signs were entropion and/or ectropion (19 dogs: 95%), conjunctivitis (17 dogs: 85%), euryblepharon (12 dogs: 60%) and non‐ulcerative keratitis (10 dogs: 50%). CFR was performed in 12 dogs (60%), frontal rhytidectomy in 5 (25%), coronal in 2 (10%), and modified shared in 1 (5%). Concurrent surgical techniques were performed in 17 dogs (85%), being lateral canthoplasty (13 dogs; 65%), and Celsus–Hotz (10 dogs; 50%) the most common. The median follow‐up time was 115 days with no complications and good outcomes reported in all dogs. At last re‐recheck, complete correction of the eyelid positioning was obtained in 92% (11/12) and 87.5% (7/8) of the cases that underwent CFR and other rhytidectomy techniques, respectively.ConclusionCFR is an effective surgical treatment for severe pseudoptosis and superior entropion in dogs. Lasarev","doi":"10.1111/vop.13274","DOIUrl":"https://doi.org/10.1111/vop.13274","url":null,"abstract":"ObjectiveTo examine the effect of client presence on healing rates of spontaneous chronic corneal epithelial defect (SCCEDs) following debridement and anterior stromal puncture (ASP).Animals StudiedSixty‐eight client‐owned dogs.ProceduresDogs presenting prior to the COVID‐19 shutdown were assigned to the C group (client in the room, 31/68), while dogs presenting after were assigned to group NC (no client in the room, 37/68). Inclusion criteria were retention of fluorescein, non‐adherent epithelium, persistence for at least 1 week, and recheck within 1 month. Exclusion criteria were concurrent ocular disorders and endocrinopathies. Success was defined as negative fluorescein retention at first recheck. <jats:italic>t</jats:italic>‐Tests, rank‐sum tests, and chi‐squared or Fisher's exact tests were used to compare findings between groups. Logistic regression was used to determine whether odds of success at first recheck differed between groups or were modified by other characteristics.ResultsDogs in the NC group were older (9.9 vs. 8.7 years, <jats:italic>p</jats:italic> = .014) and had more bandage contact lenses (BCLs) placed (65% vs. 29%, <jats:italic>p</jats:italic> = .003). There were no other significant differences between groups. BCL placement was associated with significantly greater odds of healing by first recheck (OR = 4.00, 95% CI: 0.63–11.2; <jats:italic>p</jats:italic> = .008). The NC group initially had 2.5 times greater odds of healing than the C group; after adjusting for BCL placement, the association between client location and healing weakened (aOR = 1.80, 95% CI: 0.63–5.13; <jats:italic>p</jats:italic> = .277).ConclusionsHealing was marginally associated with not having the client in the room, likely due to increased BCL use.
