{"title":"Histopathologic study of the liver after allogeneic bone marrow transplantation: Significance of hepatic graft-versus-host disease.","authors":"N. Hirabayashi, Masafumi Ito","doi":"10.3960/JSLRT1961.29.21","DOIUrl":"https://doi.org/10.3960/JSLRT1961.29.21","url":null,"abstract":"Postmortem livers from 38 cases were histologically examined to assess relative importance of hepatic graft-versus-host disease (GVHD) for liver dysfunction after allogeneic bone marrow transplantation. Hepatic GVHD, based on the finding of bile duct atypia, was present in 24 of 25 cases with clinical signs of GVHD (acute 14, chronic 11), and in none of 13 cases without clinical GVHD. Of the 25 cases with clinical GVHD17 (68%) had veno-occlusive disease (VOD), and 12 (48%) had opportunistic viral infections (cytomegalovirus 7, varicella-zoster virus 3, adenovirus 1, herpes simplex virus 1). Hepatic peliosis was present in two cases given azathioprine to suppress chronic GVHD. Azathioprine was also causal factor for the late occurrence of VOD in three cases. Extensive hepatocellular necrosis in the centrilobular region was found in a case treated with cyclosporin-A (CsA). All of the seven cases with hepatic failure had serious complications including VOD, peliosis, CsA hepatotoxicity and viral hepatitis either alone or in combination. In contrast, these hepatic comlications could not be found in five cases whithout cholestasis in the liver. Electron microscopic observation revealed an amorphous substance in the intercellular spaces and histochemical analysis identified it to be acid mucopolysaccharide. Blue-green granules in marrow hitiocytes were observed in one case. Acanthocytosis was observed in both cases. Shimoyama","doi":"10.3960/jslrt1961.31.479","DOIUrl":"https://doi.org/10.3960/jslrt1961.31.479","url":null,"abstract":"Fourteen adult patients with resistant malignant tumors (3 with malignant lymphoma, 3 with acute lymphocytic leukemia, 5 with recurrent breast cancer and 3 with germ cell tumor) received high dose chemotherapy followed by autologous bone marrow transplantation (ABMT) and daily infusion of granulocyte colony stimulating factor (G-CSF). All the 14 patients showed bone marrow engraftment. Median neutrophilic recovery over 500/cmm was 12 days after ABMT, and no life-threatening infection occurred during neutropenic period. As compared with literature controls of high dose chemotherapy followed by ABMT not using G-CSF in Western countries, our results showed earlier recovery of neutrophils. It is suggested that G-CSF may shorten neutropenic period after ABMT. Non-hematologic regimen-related toxicities were under control. No therapy-related death was observed. The author examined by scanning electron microscopy the sinuses of spleens resected from 3 patients with IPH, as well as from 10 patients with liver cirrhosis and from 3 patients with gastric cancer as controls. In IPH, the arrangement of rod cells of the splenic sinuses was severely disordered, and the slits between rod cells were irregularly enlarged. Rod cells were also deformed and accompanied branching or process formation. The bridges, rod cells which traversed the sinusal lumen, were frequently observed. Decreased resistance in the intrasplenic circulation arising from enlargement of the slits was considered to produce increased splenic blood flow. In liver cirrhosis, the slits tended to close. The age-related decline of immunological function primarily occurs in T cell-dependent immune system and is generally associated with increase in susceptibility to infections as well as in incidence of autoimmune phenomena in the elderly. The age-related change in T cell-dependent immune functions can be ascribed to the physiological thymic atrophy which starts in an early stage of life. Emigration of T cells from the thymus to the periphery mainly takes place in the late fetal and newborn stage, and dramatically declines after puberty. In other word, the thymic capacity to promote T cell differentiation starts to change in the early stage of life in terms of quantity and quality of T cells. Age-related quantitative changes of aging T cells are seen in their number and subsets. Thus, the composition of T cell-subsets in the periphery gradually changes with age, resulting in the alteration of T cell functions in the elderly. In relation to the qualitative change of aging T cells, disturbance of intracellular signal transduction is responsible for the impairment of proliferation and cytokine production upon antigenic stimulation. However, age-related change of immune functions are controlled not only by T cells but also by endogenous and exogenous factors.
