In Japan, up till now, more than 30 patients with this disease have been described in the literature, in which the autopsy studies ware rare.A 39 year-old male was admitted of the hospital, because of cough and dyspnea on exertion. The family history revealed consaguineous marriage of his parents. He was albino and had often experienced the prolongation of epistaxis since his high school days. On admission, chest X-rays revealed bilateral pneumothorax and diffuse reticulonodular shadows. At sewing operation for pneumothorax, lung biopsy was performed, demonstrating pulmonary fibrosis and deposits of brownish granular materials in alveolar epithelia and macrophages. Lumi-aggregometer represented a decrease of secondary platelet aggregation and an absence of ATP release. From these results, the diagnosis of HPS associated with pulmonary fibrosis was settled. In spite of steroid therapy, he was died of pleural hemorrhage 9 days after operation. Autopsy revealed accumulation of brownish granular materials in the epithelial cells of renal tubules, alveolar macrophages, and in the cells of reticuloendothelial tissues, particularly in the bone marrow, spleen, and liver. They retain antigen-antibody complexes on their surface and may play the leading role in the induction of immunological memory and the maintenance of long-term immunity. The author observed their unique distribution with the aids of their characteristics to retain immune complexes and new type of fluorescent microscope, confocal scanning laser microscope. Detailed structure of cytoplasmic processes was described by ultrastructural observation of serial ultrathin sections.Twelve days after human serum and red blood cells were injected to the Sprague-Dawley (SD) rats intravenously or peritoneally, FDC in the spleen were detected by surface human IgG with FITC-conjugated antibody using confocal scanning laser microscope. FDC had long cytoplasmic processes and formed complex labyrinth-like structures. FDC-clusters were not observed. S-100 protein was also a useful marker to detect FDC of the rat.Ultrastructural study on human tonsils with follicular hyperplasia showed that the nucleus was irregularly shaped, with well-defined nucleolus and prominent fibrous lamina. The cytoplasm had rough-surfaced endoplasmic reticulum, mitochondria, Golgi apparatus, polysomes, and bundle of microfilaments along the cell membrane. Extracellular dense materials were common findings. Observation of serial sections showed that the processes of FDC were ruffled dendritic processes with extensive branching and anastomoses. Lymphocytes also came in active contact with FDC.Author also made an observation on the S-100-positive cells in the rat lung, and found that reticular cells in the lymphoid follicles of bronchus-associated lymphoid tissue (BALT) have similar features to FDC ultrastructurally, although their cytoplasmic processes were not developed as those in the spleen. Asai","doi":"10.3960/JSLRT1961.36.187","DOIUrl":"https://doi.org/10.3960/JSLRT1961.36.187","url":null,"abstract":"In this experiment, we examined the structure and the cellular reaction in lymphatic sinuses and the surrounding regions caused by T-independent (LPS) and T-dependent (SRBC) antigens administration in rats and SMA mice (LPS high responder strain).Lymphatic sinuses are incompletely lined by smooth lymphatic endothelial cells, and a distinctive basement membrane is lacking. Reticulum cells form a loose meshwork not only the regions around the lymphatic sinuses but also in the lumen of the sinuses. On days 1 and 3 after intraperitoneal injections of either LPS or SRBC, the lymphatic sinuses were dilated, and many macrophages, which bear LPS antigens immunostained by IC6H or EO9, were found in and out of the marginal and trabecular sinuses. After the carbon solution was injected into footpad subcutis, carbon laden-macrophages were also found there in a few minutes. That is to say, antigens reached LN via lymphatics, and spreaded around lymphatic sinuses easily in short time. The structure of lymphatic sinuses allows easy passage of antigens in and out of the sinuses, and the meshwork of reticulum cells causes the stasis of lymphatic flow.We also found in lymph node (LN) that B-lymphocytes (B-lys), which have the same phenotypes as marginal zone B-lys (MZB), were induced by LPS injection. These B-lys infiltlated around marginal and trabecular lymphatic sinuses on days 1 and 3 after LPS injection. Moreover, plasmacytic B-lys and plasma cells were found in the same regions as the B-lys infiltlated preceding the formation of activated germinal centers (GC) in SMA mice. These findings suggested that the B-lys mature into plasma cells around the lymphatic sinuses independent of B-lys differentiation in GC. Biopsy specimen from cervical lymph nodes was diagnosed as malignant lymphoma. Physical examination revealed systemic lymph nodes swelling, splenomegaly and bone marrow involvement. He received two courses of CHOP and MEVP therapy without much effect and died after five months later. Tissue specimen showed diffuse atypical lymphocytes infiltration and no residual atrophic germinal centers are found. Most of the tumor cells showed medium to large sized and round (without cleaving) nuclei and a predominant nucleolus. Starry sky appearance and frequent mitoses are observed, suggesting rapidly proliferating nature.Immunohistchemical study on paraffin section and flow cytometrical analysis demonstrated that CD5, CD20, CD21, sIgM and sIgD were positive on neoplastic cells but CD3 and CD10 were negative. Hanada","doi":"10.3960/JSLRT1961.30.401","DOIUrl":"https://doi.org/10.3960/JSLRT1961.30.401","url":null,"abstract":"We studied clinical and pathological features of gastrointestinal (GI) lesions in patients with adult T cell leukemia (ATL) by examining 84 ATL patients treated in our department from September 1977 to May 1990, and 134 ATL patients autopsied in Kagoshima prefecture from November 1976 to December 1988.We found the following results:1) Abnormal mucosal pattern throughout the GI tract was demonstrated by radiography using barium meal and enema in most of the patients.2) Endoscopy demonstrated edematous and reddish mucosa in stomach and/or colon in many cases. Multiple ulcer and erosion were frequently found. Usual endoscopy failed to discriminate granular lesions in stomach and colon which were demonstrable by radiography, but spreading methylene blue on the mucosa surface enabled to discriminate the lesions.3) In biopsied specimens, ATL cells were found to be invaded into mucosal area of stomach in 28 out of 48, and colon in 5 out of 7 patients examined.4) In autopsied patients, infiltration of ATL cells into the GI tract was found to be evident in 59 out of 134 patients (44.0% of total). Among the 48 patients that GI lesions were examined thoroughly, 24 patients had the cell invasion in the GI tract except for esophagus. ATL cell invasions were found to be in stomach, small intestine, and colon in 13, 16, and 13 out of 48 patients, respectively. In 5 patients, simultaneous invasions into the three lesions were observed.5) ATL cell invasion was also frequently found around lymph vessels. In small intestine, invasion into lymph follicles and resulting tumor formation was characteristic.6) HML-1, a monoclonal antibody produced from small intestinal mucosal lymphocytes, reacted with ATL cells from peripheral blood. The reactivity was higher in the cells from the patients having cell invasion in GI tract as compared with patients without the invasion.7) In immunohistochemical study, HML-1 reacted with invaded ATL cells in lymph nodes, skin, stomach and tonsil.In conclusion, GI lesions were observed in most of ATL patients. Characteristic views demonstrated by radiography and endoscopy were present.Pathologically, ATL cell invasions were evident in mucosal area and around lymph vessels. Its morphological features involve nuclear and cytoplasmic condensation, apoptotic body and rapid uptake by phagocytes. This form of cell death effects single cells with living tissue without disruption of either architecture or function. The process of apoptosis avoids both the release of intracellular contents into the tissue and the resulting inflammation which characterizes necrosis. Apoptotic phenomenon was recently confirmed by biochemical and immunohistological techniques such as electrophoresis and the TUNEL method for DNA fragmentation.We used 36 cases of lymphadenitis including necrotizing lymphadenitis (24 cases), abscess forming lymphadenitis (3 cases), tuberculous lymphadesitis (3 cases), tularemia (3 cases), non-specific lymphadenitis (3 cases) and 140 cases of non-Hodgkin's lymphomas (NHLs).In necrotizing lymphadenitis, apoptotic cells which correspond to CD 4+ lymphocytes are localized at the necrobiotic focus (7.9%). Fas antigen is observed in blastoid cells (CD 8+) and Fas-ligand is present in macrophages. In addition, we detected human herpesvirus 6 (HHV 6) particles, antigens and genomes within phagocytized cells. These findings suggest that apoptosis in necrotizing lymphadenitis may be induced by virus or non-bacterial agents rather than the Fas/Fas-ligand system.In the high-grade group of NHLs, c-myc and p-53 oncogens are highly expressed but bcl-2 protein is lower. C-myc positive NHLs are significantly rich in TUNEL-positive (apoptotic) cells. In addition, it is known that bcl-2 protein tends to impede apoptosis mediated by c-myc oncogen.Our data show that the coexpression of c-myc and p-53 oncogens may induce apoptosis rather than Fas and bcl-2 oncogens because of a lower inhibitory effect of bcl-2 protein in the high-grade group.Apoptosis is an important cell regulator in tissues under both physiological and pathological conditions. These histologic subclassification was based on the modified International Working Formulation (WF). Of these, 58 were derived from T-cell and 74 were from B-cell due to the results of immunophenotype. In high grade group of T-cell lymphomas, bigenotypic pattern with Cβ and JH probe was 14% and multiple rearranged bands pattern was 13%. On B-cell lymphomas, the bigenotypic pattern was 12% in high grade, 4.7% in intermediate grade and nothing in low grade. The multiband pattern was observed in only two cases (one is lymphoblastic subtype and the other one large). However, there is no corellation between the frequency of biphenotypic or multibands pattern, and WF subclassification in B-cell lymphoma. The presence of these specific rearrangement patterns in T-cell lymphoma (except for AILD) was associated with a poor prognosis.
