A detailed review of her medical history revealed that her acute pyelonephritis was masking a chronic abdominal pain issue accompanied by significant bilirubinemia. MRCP showed a complete cystic transformation of the left liver lobe and liver fibrosis. This mini-review outlines historical aspects, physiological principles, and the risks associated with hypoxemia and hyperoxia. Current target ranges for oxygen saturation are presented, including patient-specific considerations in those at risk of hypercapnia. Emphasis is placed on conservative oxygen administration. There is still no cure. The clinical presentation is heterogeneous. Besides rapid cognitive decline, the dominant symptoms are ataxia, visual disturbances, extrapyramidal symptoms and myoclonia. Diagnostic workup revealed severe immune hemolytic anemia, specifically cold agglutinin syndrome (CAS), characterized by a positive direct antiglobulin test (DAT) for complement C3d and IgM antibodies. The patient's clinical course was complicated by significant hemolysis, requiring supportive care and multiple blood transfusions. Over the course of hospitalization, her neurological condition progressively worsened, with the development of myoclonus and ultimately a comatose state. Cerebral imaging, EEG, and cerebrospinal fluid analysis revealed no diagnostic abnormalities. Retrospective review showed an elevated lithium level seven weeks prior to presentation in the emergency department. Following hemodialysis, her neurological status gradually improved. Two-thirds reported good job satisfaction, with paediatricians slightly more satisfied than GPs. High job satisfaction was strongly associated with lower stress (general practitioners: OR = 0.280; p  0.001; paediatricians: OR = 0.431; p = 0.012). Administrative workload was associated with higher stress among general practitioners (OR = 1.038 per hour; p  0.001). This case demonstrates the benefit of a multimodal rehabilitation approach and supports its early use in difficult-to-control bronchial asthma. Evidence on the efficacy and safety of drugs in this patient population is limited, as they are often underrepresented or exclu-ded from clinical trials. Most guidelines focus on single diseases and only occasionally address the challenges of pharmacotherapy for older, multimorbid patients taking multiple drugs. A regular and structured evaluation of the medication is therefore essential. The decision-making process for further therapy should consider not only existing diagnoses, but also the individual situation and life goals of the patients.
