And no less common among the population of the clinical and hematological syndrome is anemia. Anemia in coronavirus infection can be both a contributing factor to its development and a direct consequence of infection. We consider dyserythropoietic “deficiency” anemia – iron deficiency and anemia of chronic diseases characterized by a decrease in serum iron levels. In turn, iron is an active participant in tissue respiration and ensures the normal functioning of the immune system. Traumatology, as well as medicine in general, is developing up to date, which leads to a significant expansion of diagnostic capabilities in traumatology practice. At the same time, the musculoskeletal system has many age-related features. The growth and development of the child’s body determine the specific course of various pathological processes and the great variability of their clinical manifestations. In this regard, there are clinical forms of intraarticular fractures of the long tubular bones of the lower limb characteristic of children, difficulties in recognizing which can still lead to diagnostic errors. {"title":"Un cas clinique d'anesthésie rachidienne-péridurale combinée à deux niveaux avec fixation d'un cathéter épidural dans le canal sous-cutané avec une sonde à la paroi abdominale antérieure","authors":"Sergey A. Emelyanov","doi":"10.20310/2782-5019-2022-4-3-11-18","DOIUrl":"https://doi.org/10.20310/2782-5019-2022-4-3-11-18","url":null,"abstract":"We present an example of a combined two-level spinal-epidural anesthesia with fixation of an epidural catheter in the subcutaneous canal and its removal to the anterior abdominal wall in an 80-year-old patient during surgical treatment of a closed pertrochanteric fracture of the left femur with fragment displacement. After preoperative preparation and examination, the patient underwent surgery: Open reposition of a comminuted pertrochanteric fracture of the left femur, osteosynthesis with a dynamic femoral screw. Anesthesiological pro-vision of surgical intervention: combined two-level spinal-epidural anesthesia with a method developed in our clinic for fixing an epidural catheter in the subcutaneous canal using a modified spinal needle and bringing the catheter to the anterior abdominal wall. After the operation, the patient underwent postoperative analgesia in the form of prolonged epidural analgesia for 96 hours. The postoperative period passed without complications. On the 12th day, the patient was discharged for outpatient treatment by a traumatologist. The use of neuraxial methods of anesthesia in an elderly patient with a high anesthetic risk in the surgical treatment of a fracture of the proximal femur with postoperative analgesia in the form of prolonged epidural analgesia, in which the epidural catheter was tunneled under the skin of the lumbar region and brought to the anterior abdominal wall, contributed to the successful implementation of surgery, early activation and verticalization of the patient, the absence of complications in the postoperative period. COVID-19 has a direct impact on the cardiovascular system, and in severe cases, it entails adverse consequences for the patient. The links of pathogenesis that form various disorders in the cardiovascular system are unclear and remain unexplored. The purpose of the study: based on the studied literature, to present and systematize possible pathophysiological mechanisms that can potentiate the development of various arrhythmias, including atrial fibrillation in patients who had a coronavirus infection. Materials and Methods. A literature review of scientific papers over the past 3 years was carried out in the search engines PubMed and eLIBRARY using keywords. For this meta-analysis, we used articles containing the evidence and experimental base on the most recent issues related to the epidemiology and pathophysiology of arrhythmias in patients who underwent a new coronavirus infection SARS-CoV-2 – atrial fibrillation. For the systematization of facts and a detailed study of literary sources in scientific work, general theoretical research methods were used: methods of analysis and synthesis. Results and Discussion: we present data of epidemiology, pathophysiology of the most frequent arrhythmias in patients who had a new coronavirus infection SARS-CoV-2 – atrial fibrillation. Conclusion. We analyze principles of project management in healthcare, discuss principles of PMI PMBoK (Project Management Institute Project Management Body of Knowledge) management and its application features. Project management is applicable and effective in modern conditions of the healthcare system, as it allows to respond promptly to challenges and changes; PMBOK-6 project management can be successfully adapted for projects of the regional healthcare system and medical organizations; when implementing PMBOK-6 management projects in the healthcare system, it is necessary to take into account the features of the system, its relative inertia, social significance, vulnerability of patients, limited resources. We demonstrate the PMBOK-6 principles dependence on the project status and an effective algorithm for managing the program and its components. Discussed use of PMBOK-6 tools enables to identify priority areas of program planning, including at the regional level. In the last decade, the number of surgical interventions has been growing, surgical techniques and methods of anesthesia have been improved. In practiced anesthesia various drugs are injected into the patient's bloodstream or through a mask, completely turning off his consciousness. Methods of prophylactic pain relief allow to reduce the intensity of postoperative pain and contribute to the prevention of chronic postoperative pain syndrome. Currently, there have been significant changes in the range of anesthetics. Opioid analgesics and non-opioid analgesics of central action are used for pain relief, non-steroidal anti-inflammatory drugs and local anesthesia are also widely used. Superficial or application anesthesia is possible. In operations on the lower extremities, spinal and epidural anesthesia techniques and their modifications are becoming more widespread. Complex (multimodal) pain relief tactics involves the use of a combination of non-steroidal anti-inflammatory drugs, local anesthetics, centrally acting analgesics and drugs acting at the level of nerve impulse conduction. In addition, the diagnosis of injuries of the cervical spine is often difficult due to the peculiarities of the anatomical structure of the cervical spine, fixing devices, as well as the forced position of the patient. Therefore, it is important to have an idea of various research methods for the timely, competent construction of a diagnostic algorithm in case of suspected injury to the cervical spine and the early start of therapeutic measures. Presented literature review provides information on modern techniques of diagnostic radiology for injuries of the cervical spine. Such methods as radiography, computed tomography, CT myelography, and magnetic resonance imaging are used in modern medicine individually or in combination to diagnose injuries of the cervical spine of various nature. Assisted reproductive technologies are methods to solve the problem of infertility. From the point of view of bioethics, there are a number of controversial issues: determining the personal status of the embryo, the ratio of risks and benefits for participants in the procedures, the possibility of cryopreservation, the destruction of “extra embryos”, etc. The purpose of our research was to study the attitude of the population to assessing the importance of bioethical aspects of the use of assisted reproductive technologies. During the study, an anonymous survey was conducted on the Google platform. The study involved 104 respondents of both sexes (83 women, 21 men) of various ages, mostly (59.6 %) in the category of 15–20 years. The survey questions were aimed at determining the degree of awareness of the participants about the problem of infertility and assisted reproductive technologies, the bioethical issues that they affect, as well as the subjective attitude to the use of these methods. Most of the respondents had an idea about the goals of assisted reproductive technologies and the bioethical problems that they generate. In modern conditions, it is generally accepted that the treatment of femoral neck fractures should be surgical. This is due to the extremely high rates of hypostatic complications in case of refusal to perform the operation. However, extensive traumatic surgery is not always justified in the presence of severe somatic pathology in the patient. Delay or cancellation of the operation dramatically increases the risk of complications associated with physical inactivity and the risk of a lack of fracture consolidation, aseptic necrosis of the femoral neck and the formation of a false joint. In order to activate patients as soon as possible, it is recommended to perform the operation as soon as possible. There are many methods of osteosynthesis, as well as arthroplasty, however, for patients with severe somatic pathology, we have proposed a minimally invasive method of percutaneous osteosynthesis of the femoral neck with needles without the need for traumatic open reposition. According to the proposed method, 10 patients with femoral neck fractures were treated, for whom extensive surgery, such as open reposition, osteosynthesis or arthroplasty, was contraindicated or there was a need to delay them in order to correct concomitant diseases. Active surgical tactics in relation to this group of patients made it possible in all cases to activate the patient as quickly as possible, to avoid the development of delirium and hypostatic complications.
