Herpes is in the second place among infectious human lesions, according to the World Health Organization, almost 90 % of the inhabitants of the planet are infected and are virus carriers. In these conditions, it is necessary to focus on the features of the correct tactics of managing patients with recurrent herpes at a dental appointment. Herpes has a complex clinical picture, which is difficult to monitor, so the doctor and medical staff need to resort to preventive methods in order not to harm the patient and not to get themselves infected. The research discusses the symptoms of recurrent herpes depending on the stage of the lesion and determines the correct treatment tactics for this pathology, in order to avoid the spread of infection and infection of the doctor, provides up-to-date information on the prevalence, methods of diagnosis and treatment of recurrent herpes. The relevance of the prevention of herpes simplex virus disease is increasing due to its ubiquitous spread, lifelong existence in the body of infected people, a significant variety of clinical manifestations of herpes infection, resistance to existing treatment methods. The current situation indicates the need for a differentiated approach to the assessment of the clinical components of the disease and the management of a patient with herpesvirus infection; it is relevant to determine the calculated coefficients and risk programs compiled on the basis of anamnestic and questionnaire data.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"108 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121499473","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Knowledge of dentists and dental patients about diabetes is insufficient. The role of a dentist in the early diagnosis of diabetes mellitus and the features of the treatment of dental diseases in diabetic patients are discussed. Diabetes mellitus is one of the diseases receiving much attention in modern scientific and medical societies. It is easy to explain such a high interest: diabetes mellitus ranks third among the direct causes of death after cardiovascular and oncological diseases, therefore, the solution of issues related to diabetes mellitus is put at the state level in many countries. According to various sources, there are from 120 to 180 million diabetic patients in the world, which is 2-3 % of the total population of the planet. The incidence of diabetes mellitus seemed to become endemic and a crisis in the health sector - according to forecasts of the World Health Organization, by 2025 the number of diabetic patients in the world will increase to 333 million people. Along with the increase in the number of patients with diabetes mellitus, the number of patients suffering from complications of this serious disease increases, which leads to rapid disability. The objective of this study was to increase the effectiveness of prevention and treatment of inflammatory periodontal diseases in patients with diabetes mellitus.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"13 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"122417093","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The level of knowledge about the nature of the infection also influences the development of new treatment algorithms. We consider the formation stages of drug therapy of coronavirus infection as research by Russian and foreign medical communities shows previous treatment experience of diseases caused by other coronaviruses representatives. Based on the results and experience of using different groups of drugs, clinical recommendations for the treatment of COVID-19 have been formed. The purpose of the research: to trace the stages of approaches and tactics of drug treatment of a new coronavirus infection. The material for this research is the data obtained in the analysis of modern foreign and national literature on this problem. Conclusion: despite the fact that the COVID-19 epidemic has been spreading around the world for several years, there is still no “gold standard” treatment with 100% effectiveness. In this regard, the issue of forming a competent approach to COVID-19 therapy in order to reduce mortality, increase the effectiveness and safety of the treatment remains relevant today.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130823360","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Often these fractures require surgical treatment. However, comminuted fracture, osteoporosis make it difficult to carry out osteosynthesis. This article describes a clinical case of successful treatment of a fracture of the surgical neck of the humerus in a patient with osteoporosis. Surgical intervention was carried out on the basis of the developed method of osteosynthesis of a fracture of the surgical neck of the humerus (RF Patent no. RU 2749296 C1). To fix the fragments we used a T-shaped plate, screws and a wire loop-tie passed through the head of the humerus. Carrying out the metal structure extramedullarly through the fracture line minimizes additional intraoperative damage and creates a reliable fixation of fragments without the risk of loosening and migration of the metal structure, which allows this method to be used in the osteoporotic process in the head of the humerus. Obtained results of the patient's treatment are good: restored function and amplitude of movement in the shoulder joint, no pain syndrome. The presented clinical case demonstrates the expediency and effectiveness of osteosynthesis according to our proposed method for fractures of the proximal humerus on the background of osteoporosis.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"2000 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128284895","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Chronic obstructive pulmonary disease is a total chronic disease of the human respiratory system. The process of formation of pathological changes in the lungs is primarily associated with the impact of harmful chemicals – tobacco and smoke on the epithelium of the respiratory tract. The increase in life expectancy in recent decades allows most people to reach the age of high probability of chronic obstructive pulmonary disease and related pathologies. According to various sources, chronic obstructive pulmonary disease is in third or fourth place among the causes of death. The purpose of this review is to systematize practical methods for diagnosing and treating patients with chronic obstructive pulmonary disease on an outpatient basis. Materials and methods. The research methods involve the systematization of materials describing the identified problem, the collection and classification of medical facts about it, the assessment of existing scientific and practical approaches to solving the problem under consideration. Results. The prognosis for chronic obstructive pulmonary disease in terms of complete recovery and increased life expectancy is generally unfavorable. Therefore, it is important to take into account all the possibilities to eliminate any factors provoking deterioration of the condition, the patient's readiness for treatment, as well as the social and economic conditions of his residence. Conclusion. Treatment for chronic obstructive pulmonary disease is determined by the severity of the obstruction, symptoms, and the risk of exacerbation with escalation of therapy to control persistent symptoms or further exacerbations. The severity of an exacerbation varies according to the site of treatment: moderate exacerbations are treated on an outpatient basis with oral corticosteroids, antibiotics, or both, and severe exacerbations require hospitalization or emergency care.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"91 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121586455","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

All patients underwent postoperative epidural analgesia. We applied a method for assessing the degree of external dislocation of the epidural catheter and an algorithm of actions when a dislocation of the epidural catheter is detected. When changing the fixing aseptic sticker, the degree of epidural catheter dislocation was assessed, and if a pronounced dislocation was detected, additional measures were taken to enhance the fixation of epidural catheter. In 14 cases, we used additional fixation devices “Epi-Fix”, which was associated with the threat of falling out of the epidural catheter with a pronounced degree of dislocation. Additional fixation in this case helped to avoid prolapse and ensure continuation of anesthesia. However, in two cases, the epidural catheter was removed because the dislocation corresponded to the 6th degree of the proposed scale, and its continued use was useless. Thus, the use of the scale we developed enables to monitor and assess the risk of worsening anesthesia and catheter migration or prolapse. Further tactics in relation to prolonged anesthesia depend on an objective assessment of the position of the catheter.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"32 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114795445","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The requirements for dental treatment of the frontal group increase with the improvement of materials and tools. The methods of recreating the aesthetics of the tooth used in the conditions of the dental laboratory are not always suitable for use in the clinic of therapeutic dentistry. This is due to the peculiarities of performing restoration in the oral cavity, the properties of composite materials, as well as the limited working time of the doctor. The improvement of materials and restoration techniques made it possible to meet the high aesthetic requirements of patients. So, the vast majority of people expect a dentist to create so-called invisible restorations. Patients are interested in preserving more tooth tissues and want to take part in choosing the color and shape of future work. They also set the dentist the task of predicting the final result even before the start of treatment. Therefore, it is important for a doctor to keep up with the development of new technologies and improve his manual skills, improving the quality of restorations. Dentists often face a choice between direct and indirect methods of restoring the teeth of the frontal group. The most common causes of dental injuries are falls and bumps. Mainly the upper central incisors suffer, most often there is an uncomplicated fracture of the crown parts of these teeth (without opening the tooth cavity). Many materials and methods are used to restore the front teeth after injury. The possibilities of adhesive dentistry allow to restore the natural appearance of teeth with almost complete preservation of intact hard tissues. We describe recommendations for the use of composite restorations for conservative restoration of injured teeth.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"20 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115128474","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

There are no specific pathogens of chronic adenotonsillitis, but there are still disputes about the viral, bacterial and fungal etiology of this disease. Chronic tonsillitis is a persistent chronic inflammation of the palatine tonsils, characterized in the vast majority of patients by recurrent exacerbations in the form of sore throats and a general toxic-allergic reaction, which is a frequently realized etiological factor of many local and general diseases and a catalyst for pathological processes in the body. Adenoid vegetations are pathological hypertrophy of the pharyngeal tonsil. Chronic adenoiditis is a chronic polyethological disease, which is based on a violation of the physiological immune processes of the pharyngeal tonsil. In children, it is impossible to talk about isolated inflammation of the pharyngeal tonsil, since as a result of exposure to antigens, an immune response occurs that involves all structures of the lymphoepithelial pharyngeal ring in the process, therefore some researchers use the term “adenotonsillitis”. With a pronounced violation of the microflora in the nasopharynx and oropharynx, the body's resistance to pathogenic microorganisms decreases, as a result, decay products and toxins lead to damage to the vascular endothelium, disrupt their permeability and, penetrating through the epithelial barrier, contribute to the development of chronic intoxication and sensitization of the body.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"93 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126212241","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and methods. We conduct the study on mice of the Balb/c line. The work was carried out according to the recommendations set out by the Ministry of Health of the USSR no. 755 of August 12, 1977 and the Helsinki Declaration. We form a control group, a comparison group and an experimental group. In the comparison groups, carbon tetrachloride 0.1 ml of 40 % solution and methotrexate at a dosage of 0.2 mg were administered intraperitoneally to animals. The experimental group included animals with formed malignant tumors. Main results. Pathomorphological changes indicate the progression of nonspecific necrobiotic, cholestatic, inflammatory and microcirculatory processes under the influence of a hepatotropic agent. When using cytostatics, necrobiotic, cholestatic and microcirculatory changes progressed without a cellular reaction. Conclusion. The revealed pathomorphological features in the liver tissue indicate the pathogenetic significance of toxic liver damage leading to the progression of liver failure in tumor processes.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"326 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133850458","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
