In modern conditions, it is generally accepted that the treatment of femoral neck fractures should be surgical. This is due to the extremely high rates of hypostatic complications in case of refusal to perform the operation. However, extensive traumatic surgery is not always justified in the presence of severe somatic pathology in the patient. Delay or cancellation of the operation dramatically increases the risk of complications associated with physical inactivity and the risk of a lack of fracture consolidation, aseptic necrosis of the femoral neck and the formation of a false joint. In order to activate patients as soon as possible, it is recommended to perform the operation as soon as possible. There are many methods of osteosynthesis, as well as arthroplasty, however, for patients with severe somatic pathology, we have proposed a minimally invasive method of percutaneous osteosynthesis of the femoral neck with needles without the need for traumatic open reposition. According to the proposed method, 10 patients with femoral neck fractures were treated, for whom extensive surgery, such as open reposition, osteosynthesis or arthroplasty, was contraindicated or there was a need to delay them in order to correct concomitant diseases. Active surgical tactics in relation to this group of patients made it possible in all cases to activate the patient as quickly as possible, to avoid the development of delirium and hypostatic complications. The use of the proposed method of osteosynthesis in debilitated patients makes it possible to simplify treatment, minimize the risk of perioperative complications, reducesurgical trauma and blood loss.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"12 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121507155","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

For intra-articular fractures, it is extremely important to restore the normal anatomical structure, precise reposition and fixation of the fracture, without which it is impossible to restore the full function of the joint and limb. Surgical treatment should be carried out as early as possible in order to prevent the development of contractures. The purpose of this publication is to demonstrate the successful experience of treating a patient with an open comminuted intra-articular transcondylar fracture of humerus with displaced fracture fragments and the validity of the choice of an operative method of treatment of this pathology. Materials and methods : description of a clinical case and a brief analysis of the literature on this issue. Presented case of open reposition of a comminuted intra-articular transcondylar fracture of the shoulder and extra-cortical osteosynthesis with two reconstructive plates is successful. The effectiveness of treatment was evaluated based on the data of X-ray examination methods, as well as assessing the function of the limb and the amplitude of movement in the elbow joint. Results : performed open reposition of a comminuted intra-articular transcondylar fracture of shoulder and extra-cortical osteosynthesis with two reconstructive plates. Complete restoration of limb function and anatomical integrity of the distal metaepiphysis of the humerus has been achieved, the patient’s ability to work has been restored.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"36 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124582144","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

They are one of the most common diseases of the musculoskeletal system in children and adolescents, leading to disability in the absence of early diagnosis and timely treatment. Despite the fact that the etiology is currently insufficiently studied, there are many modern methods of diagnosing this pathology. Diagnostic radiology techniques are successfully used in modern medicine, but have both advantages and disadvantages. We review literature on radiology techniques for osteochondropathies diagnostic, such as radiography, ultrasound diagnostics, magnetic resonance imaging, computed tomography and some others. We analyze the results of the study of various researchers in this field and make the relevant conclusions. According to most researchers, the examination of patients with suspected osteochondropathy should begin with ultrasound examination. Osteodensitometry, perfusion magnetic resonance imaging, and pneumoarthrography can be used as auxiliary research methods in case of difficulties in making a diagnosis.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"131259706","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The number of adult patients using a bracket system as an orthodontic treatment is steadily increasing every year. In the process of long-term orthodontic treatment, various complications may arise, such as decrease in oral hygiene level due to new atypical areas of plaque accumulation and further formation of dental tartar – one of the leading factors of inflammatory diseases of periodontal tissues. We study the hygienic state of the oral cavity and the level of dental education in dynamics among patients undergoing orthodontic treatment. We assess the hygienic condition of the oral cavity in patients undergoing orthodontic treatment and their motivation for proper and successful treatment. We examined 110 patients aged 18 to 35 years. The results showed that the level of hygiene during the year of treatment was unstable and decreased, and therefore it is necessary to pay special attention to the motivation of the patient and the duration of the treatment.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"37 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"123669874","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We analyze results of surgical treatment of long-standing injuries of the flexor tendons of the hand in 20 patients. Patients were examined clinically and X-rayed. The clinical study took into account the fingers anatomy, coarse skin scars in the area of the surgical suture, sensitivity, blood supply, grip strength and flexion-expansion function. The X-ray examination took into account the articular slit in the phalangeal joints, their congruence. We present an objective assessment of the results of surgical treatment of chronic tendon injuries of finger flexors, as well as developed practical recommendations for the surgical treatment of chronic tendon injuries of finger flexors. Conclusions. The results of treatment are directly dependent on the duration of damage exposure. The earlier surgery is performed from the moment of injury, the better the results of treatment. The choice of treatment tactics depends on the pyoinflammatory processes in the injury area, on the injury zone, the condition of the bone-fibrous channels and the individual characteristics of each patient. Patients with chronic tendon injuries in zones II and III and damage exposure for more than 80 days we recommend to perform two-stage tendon grafting. Rehabilitation should be started from the 2nd day of the postoperative period, after the relief of the pain syndrome.","PeriodicalId":199692,"journal":{"name":"Tambov Medical Journal","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"129649135","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
