The study involved 60 people, 30 men and 30 women, whose average age was 50–60 years. All participants were divided into three groups. The first group included people without type 1 and type 2 diabetes, but with cardiovascular diseases. The second group included people with type 2 diabetes mellitus diagnosed 2–3 years ago and cardiovascular diseases. The third group included people with type 2 diabetes mellitus diagnosed 10–15 years ago and cardiovascular diseases. All three groups were analyzed according to the following averages: blood pressure, glycated hemoglobin, total cholesterol concentration, Sokolow–Lyon index and SCORE scale values. As a result of the study and comparison of three researched groups, it was revealed that people with diabetes mellitus diagnosed 2–3 years ago already have changes in the cardiovas-cular system and people with type 2 diabetes mellitus diagnosed 10–15 years ago have significant changes. There are also contradictions in literary sources when assessing signs of self-harm. The purpose of this research was to analyze the frequency of occurrence of signs of self-injury by sharp objects, after determining the content of each sign. To do this, we analyze 33 expert conclusions from the archive of Medical Examiner’s Office in Saratov and 34 expert conclusions from the archive of Medical Examiner’s Office in Tambov. All damages (100%) were in the areas accessible to one’s hand. Thus, injuries to the upper limbs were in 63.4 % of all cases, thorax – in 30.3 %, neck – 21.2 %, abdomen – 15.2 %, lower limbs and head – 9.1 % each. Multiplicity was found in 81.82 % of cases. The predominance of superficial wounds over deep ones was found only in 39% of cases. The injuries were more often located in groups (69.7 %), the majority had unidirectionality (66.67 %). There is no damage to clothing in 84.85 % of cases. Due to the high prevalence, a dentist must be able to eliminate the causes of chronic injuries and eliminate their consequences, as well as know the causes and mechanism of this disease. We consider the etiology and pathogenesis of biting the buccal mucosa, effect on the oral mucosa. We used the methods of collecting and analyzing available information on the research topic on online library platforms. We conducted an anonymous survey, the participants of which were 18 boys and 18 girls, the average age of which was 21 years. An observation was made among patients of the dental clinic in order to determine the frequency of occurrence of pathology. The issue of biting the mucous membrane of the cheeks is common among groups of all ages. This tumor formation usually proceeds asymptomatically for several years. The causes include hereditary factor, hormonal failures, chronic inflammatory diseases of the genitourinary system. With a tumor growth of more than 5 cm, the risk of spontaneous rupture of the neoplasm wall with the development of retroperitoneal bleeding, which can turn into hemorrhagic shock, significantly increases. Diagnostics is performed by means of palpatory examination, ultrasound examination of the kidneys, computed tomography, magnetic resonance imaging, radiopaque angiography, biopsy with histological examination of tissues. In the absence of a detailed clinical picture, treatment is limited to observation, surgical treatment is carried out exclusively according to indications. With the combined asymptomatic course of angiomyolipoma with acute or chronic kidney diseases, a comprehensive laboratory and instrumental diagnosis is necessary at the early stages of the examination. There are rare cases when it is impossible to diagnose angiomyolipoma by the above methods, therefore it is necessary to conduct computed tomography or magnetic resonance imaging with contrast. The purpose of this analysis is to draw the attention of anesthesiologists to the issue of assessing the adequacy of neuraxial blockade and, in particular, spinal anesthesia, as the most commonly performed anesthetic management of surgical interventions. The adequacy of spinal anesthesia implies a pronounced motor block, which allows achieving complete relaxation of the muscles of the lower extremities and, if necessary, the muscles of the anterior abdominal wall, as well as the loss of pain, temperature and, as the most optimal level, this is the loss of tactile sensitivity. Considered methods for assessing motor, sensory and sympathetic blockade are P.R. Bromage scale to assess motor block, assessment of temperature and pain sensitivity (test with an ice tube, so-called Cold-test, test for pain sensitivity of the skin pin-prick). Until recently, in anesthesia practice, insufficient attention was paid to assessing the level of sympathetic blockade. The research describes an alternative method for assessing this type of blockade: laser doppler flowmetry. The authors conducted a study on a sample of 96 patients divided into 3 groups. Each group was offered motivational texts with different frames: negative, neutral and positive. The research discusses how framing can influence patients' motivation to take care of their own health and adherence to medical examinations. The framing effect depends on the choice of words, intonation, context or emphasis on certain aspects that can create an emotional reaction in the audience. The text with a negative frame containing threats and requiring an urgent examination caused fear and anxiety in patients; this could lead to the rejection of preventive measures. At the same time, the research proves that text with a positive frame emphasizing the importance of trusting doctors and regular check-ups motivated patients to follow disease prevention recommendations. In modern medicine, there is a tendency to minimize injuries to the patient in diagnostic search to achieve a compensatory state as soon as possible, to improve the quality of life, and to maintain working capacity. All these conditions meet the requirements of the method of diagnostic laparoscopy. The purpose of this research is to assess the current state of the scope and prevalence of diagnostic laparoscopy in modern surgery. Materials and methods: the search was carried out in the open electronic scientific databases PubMed and the databases of the Russian scientific electronic library eLibrary using keywords and phrases: laparoscopy, diagnostic laparoscopy, surgical intervention, current trends, abdominal cavity. Results. In modern surgical practice, diagnostic laparoscopy is considered as a highly informative preoperative method for detecting intra-abdominal pathologies and postoperative complications. This technique is widely used in gynecological practice, oncogynecology, in acute and chronic surgical pathologies in patients of various ages in order to assess the state of the intra-abdominal space and determine the correct tactics and extent of surgical intervention in the earliest possible time. In urgent abdominal pathology, laparoscopy remains the leading minimally invasive technique that allows assessing the extent and prevalence of the process. To assess the advantage of a new method of insertion and fixation of an epidural catheter (EC) in the subcutaneous canal using a modified spinal needle (MSN) when performing combined two-level spinal-epidural anesthesia (CTSEA) in comparison with the standard method of fixing EC in elderly and senile patients during bone fracture repair of the lower extremity. Material. We analyzed 118 cases of CTSEA during trauma surgery in elderly and senile patients. The patients were divided into two groups. The first group (control) consisted of 63 patients who, during the course of CTSEA, had an epidural catheter fixed to the skin by a standard method with an adhesive patch. In the second group (experimental), 55 patients underwent EC which were fixed in the subcutaneous canal using a modified spinal needle. Result. In the study group, no internal dislocation of EC was noted. In the same group, compared with the comparison group, there were 15.9 % fewer cases of severe dislocation in which there is a risk of deterioration in the quality of anesthesia; external dislocation in which there was a threat of EC falling out oc-curred 12 % less; EC fell out of the epidural space (ES) 2.6 times less frequently. Conclusion. A new method of conducting EC in the subcutaneous canal using MSN promotes reliable fixation of the catheter, creates conditions that prevent its dislocation and loss from the ES, thereby allowing an effective and long-term postoperative analgesia. We describe issues of medicine and health care in rural areas both in Russia and abroad. The considered factors influence the increase in the availability and quality of qualified medical care in rural areas. An integrated approach is important in healthcare in rural areas. The specific features of the organization of medical care in rural areas are directly related to environmental factors. Rural medicine is a specific area of domestic healthcare, since two parallel processes are combined in Russia: the reform of the agricultural sector as a whole and the reform of rural health care. The inconsistency of these processes makes it difficult to provide quality medical care in rural areas. The formation and implementation of joint federal and regional programs require adherence to unity in solving problems. We examine the indications, contraindications of each method, their advantages and disadvantages, common complications. We identified most relevant methods of anesthesia depending on age, height, urgency of the operation, the presence or absence of preoperative preparation, the duration of the operation, the state of hemodynamics, the extent of the injury on the basis of research materials review. Various methods of performing anesthesia are provided. The main research objective is to inform about modern methods of anesthesia and drugs for anesthesia, their place in the complex anesthesiological provision of various traumatological and orthopedic operations. The considered anesthesia methods reduce the intensity of postoperative pain and contribute to the prevention of chronic pain syndrome. The effectiveness and safety of each method are analyzed in comparison. Neuroaxial blockades used in the complex of anesthetic aids provide analgesia, hyporeflexia in the area of surgery; general anesthesia leads to analgesia, muscle relaxation and hypnotic effect.
