Caudal epidural blockade is the most commonly used regional technique in children and is considered the safest access to the epidural space. This method of anesthesia is used in combination with general anesthesia. With the correct implementation of this anesthetic manual, the risk of complications is small. The technique is used for peri- and postoperative analgesia in children of different ages. The main advantage of the method is high-quality anesthesia during caudal blockade, it is provided in 95–98 % of cases. The level of anesthesia depends on the amount of anesthetic administered; in some cases it may reach the level of the chest. The same advantage is the rapid onset of adequate analgesia and its long-term effect. For successful regional anesthesia in a pediatric patient, it is necessary for the anesthesiologist to know the anatomical and physiological structures of the body, the pharmacology of local anesthetics and the availability of the necessary medical equipment. The research presents a method of using caudal anesthesia as a component of anesthetic support for surgical interventions performed in children. Due to the increase in injuries, the urgency of the problem of treating these fractures is increasing. Difficulties in osteosynthesis of fractures of the forearm bones are associated with the peculiarities of the topographic anatomy of the forearm, the subtlety of the biomechanical interaction of the radius and ulna, a large number of damage options, the need for precise reposition and stable fixation, the risk of damage to tendons and nerves. The purpose of this research is to demonstrate the successful experience of treating a patient with a closed fracture of the upper third of both bones of the left forearm. Materials and Methods: description of a clinical case and a brief analysis of the literature on this topic. The experience of revision osteosynthesis of a closed fracture of the upper third of both bones of the left forearm with angular deformation of the metal structure and secondary displacement of fragments is presented. The effectiveness of treatment was evaluated based on the data of the X-ray examination method, computed tomography, as well as when assessing the function and volume of movements in the elbow joint. Results: revision osteosynthesis of the fracture of the upper third of both bones of the left forearm with a plate with angular stability with autotransplantation of bone tissue from the crest of the tibia of the left tibia was performed. Gynecological health of women is closely related to the normal functioning of their reproductive function, which directly affects demographic indicators. According to the latest statistics, recently there has been a significant increase in the incidence of inflammatory diseases of the pelvic organs, infertility, various menstrual disorders, which requires their detailed study. The purpose of the study: to analyze the prevalence of gynecological diseases among patients of the daytime and round-the-clock inpatient facility of the Gynecology Department of the Archbishop Luke Tambov City Clinical Hospital in 2020. Materials and methods. We conduct a survey of 500 women in 2 age groups of 18–25 years and 26–35 years old. Results. Analyzing the data obtained, we can talk about a confident increase in pelvic inflammatory diseases, abortions complications, frequency of ectopic pregnancies and operations performed for infertility with increasing age of the studied groups. Conclusion. To prevent the development of inflammatory diseases of the pelvis, it is necessary to carry out preventive measures for the sexual education of girls, explanatory conversations about modern methods of contraception, promotion of a healthy lifestyle, prevention of abortions, medical examinations. We analyze the perioperative management results among patients with proximal femur fractures after surgical treatment in the Traumatology Department of the Kotovsk City Clinical Hospital in 2021. An increase in the proportion of the elderly and senile population (60–90 years old) inevitably leads to an increase in the number of proximal femur fractures, which obliges the traumatology departments in hospitals to allocate more forces and funds to assist this category of patients, to introduce and apply not only modern surgical and anesthesiological techniques, but also new approaches to the perioperative management of patients and especially patients over 60 years old, who, as a rule, have several concomitant diseases complicating the course of the underlying disease. The use of a multidisciplinary approach, a full-fledged laboratory and instrumental examination, detection of concomitant pathology and its correction, performing surgical interventions no later than 48 hours from the moment of admission to the hospital, adequate high-quality anesthesia, early initiation of mobilization measures no later than 24 hours, correct calculation of the permissible load on the operated limb allow achieving good results in the surgical treatment of patients with proximal femur fractures. The treatment of severe combined injuries is successful in a multidisciplinary hospital with the possibility of early diagnosis of injuries and prevention of further complications due to a combination of correct surgical and traumatological tactics, correctly selected infusion-transfusion therapy with the complex of modern rehabilitation measures. The purpose of the study: to describe a clinical case of effective provision of emergency medical care to a victim with severe polytrauma. The research shows the difficulties of diagnosis and the results of surgical treatment. Materials and methods: the study used materials of medical documentation of the Traumatology Department in the Archbishop Luke Tambov City Clinical Hospital. Disability and mortality of patients due to the inability to maintain airway patency remains a serious problem in anesthesiology and resuscitation although they have significantly decreased in recent years. Clinical cases of unsuccessful or difficult intubations are always a direct threat to the patient’s life both in emergency situations and during planned surgical intervention. The purpose of the research: to consider the main components of airway management in critically ill patients, as well as more subtle details of physiologically complex airways. Materials of the research: foreign and Russian literary sources on the problem. To assess the difficulty of intubation, the prediction on the LEMON scale was used. Conclusion. Difficult intubation in emergency conditions should be based on a strict algorithm of actions that helps anesthesiologists and resuscitators to prevent gross mistakes and unsuccessful manipulations in conditions of increasing patient’s hypoxia. However, pain in the maxillofacial region has its own characteristics. The tissues of the oral cavity are very richly innervated compared to other organs, which affects the greater sensitivity of this area. Patients most often seek medical help for pain-related reasons, which requires mandatory anesthesia. The endodontic treatment for complicated forms of carious lesions is based on invasive intervention. In addition to stress, there are other types of adaptive reactions of the body. A natural consequence of this is the initiation of nociceptive, neurovegetative and psychoemotional reactions. The result is the formation of chronic postoperative neuropathic pain syndromes affecting the dental health and life quality of the patient. Up to 70 % of patients' complaints are acute pain when seeking emergency care – this statistic is cited by the International Association for the Study of Pain (IASP, 2011). Pain is the main reason for patients seeking dental care. Karavaeva","doi":"10.20310/2782-5019-2022-4-4-53-59","DOIUrl":"https://doi.org/10.20310/2782-5019-2022-4-4-53-59","url":null,"abstract":"Diffuse idiopathic skeletal hyperostosis, or Forestier's disease, is a rare disease of the musculoskeletal system, leading to calcification of ligaments and tendons and, as a consequence, to ankylosing due to bone proliferation in the places of the tendon and ligamentous apparatus of the spine. As radiation imaging shows, the disease features are uniform ossification of anterior longitudinal ligament, mainly of thoracic spine, as well as damage to entheses. The disease may be detected accidentally and not manifest itself for many years, or it may be asymptomatic. In some cases, Forestier's disease can be characterized by pain in the back, limited movement in the spine. The development theory of Forestier's disease, leading to calcification, is based on an excessive number of growth factors, such as insulin-like growth factor 1, insulin, transforming growth factor beta 1, prostaglandins I2 and endothelin 1, platelet growth factor, leading to the transformation of mesenchymal cells into fibroblasts and osteoblasts. There is an opinion that the lesion of the left longitudinal ligament occurs extremely rarely, unlike the right one, since due to the pulsation of the descending aorta there is a so-called protective effect and a mechanical barrier. The pathogenetic mechanism and causes of the development of uterine fibroids are unknown, hence there are questions and disputes on identifying causation. A predisposition of fibroids can serve as chronic inflammatory diseases of the female genital organs, methods of surgical treatment (artificial abortions, cesarean section, biopsy of lining of uterus), dysfunctions of the hypothalamus-pituitary gland-adrenal glands-ovaries, heredity, occupational hazards, extragenital diseases. One of the current issues of modern obstetric and gynecological care is the increasing frequency of cesarean section. Caesarean section is one of the most important risk factors for infection, which causes adverse long-term complications for a woman’s health. Research purpose : to highlight with an example of clinical case how a complicated caesarean section provoked the occurrence of complicated large uterine fibroids in a woman of the reproductive period. The presented material describes a clinical case of a patient who underwent surgical treatment in the gynecological department with a diagnosis of “Multiple uterine fibroids. Tumor growth. Hypermenstrual syndrome. Posthemorrhagic anemia”. The presented clinical case emphasizes the need for a qualified approach to operative delivery, the technique of performing a caesarean section and postoperative management. Conclusion . Treatment of patients with uterine fibroids should be carried out in full, including observation and monitoring for early detection of this pathology, as well as timely preoperative preparation with surgical treatment such as myomectomy and metroplasty. Materials and methods. A retrospective analysis of emergency medical care for ophthalmological diseases was carried out according to the logs of emergency medical service call records of the Tambov Regional Ambulance and Disaster Medicine Station, and the registers of patients treatment in the emergency ophthalmological care and the emergency department of the Tambov Ophthalmological Clinical Hospital from January 1, 2018 to December 31, 2021. Results. In total, during four calendar years (2018–2021), 53121 patients asked for emergency ophthalmological care in the Tambov Ophthalmological Clinical Hospital. The overwhelming majority of patients were urban residents 69.3 %, and the proportion of rural residents was 30.7 %. In the structure of the causes of emergency calls, injuries of the eye, its appendage and orbit are leading, and among patients with traumatic lesions, the male population prevails (72.4 %). For the period 2018–2021 there was an increase in the share of receipts to the emergency department of the Tambov Ophthalmological Clinical Hospital with an ambulance team from 19.3 % in 2018 to 30.8 % in 2021. Conclusion.
