This concept was reiterated once again by the authors and readers of the bmj, as seen in this statement: https://blogs.bmj.com/bmj/2021/10/24/we-must-protect-our-planet-for-our-childrens-future/. Our alleged powerlessness in the face of the complexity of climate change can be overcome through awareness of what we know and what we can put into practice, and this belief also supports this column: https://blogs.bmj.com/bmj/2021/10/24/the-climate-crisis-how-do-we-show-we-care/. As in the previous issues, we summarize here briefly the main articles published in the monitored journals, among which numerous are precisely those relating to climate change and air pollution. Unfortunately, during the study, the authors changed the primary outcome from \"referral to hospital treatment\" to \"vomiting in the first 4 hours after randomization\" due to the low enrolment of patients (only 194 children enrolled in 2 years by almost 600 primary care doctors). The new outcome chosen, although statistically significant, is of little clinical interest. Enrolled patients arrived at late evaluation, after a mean symptom duration of 2 days. A pragmatic randomized clinical trial The use of videofeedback technique at home (Video-feedback In- tervention to promote Positive Parenting and Sensitive Discipli- ne - VIPP-SD) is evaluated. This is the first video-feedback RCT carried out in a primary care setting. 300 families with children aged 12-36 months who had an upper range score for behaviour problems (Strengths and Difficulties Questionnaire - SDQ) were involved. The intervention proved to be effective in reducing early behavioural problems in 1 and 2 year old children. The benefits of the intervention are seen in behavioural problems rather than in attention problems and are more evident when the initial symp- toms are worse. Uga, Anna Valori, Luisella Zanino","doi":"10.53141/peqacp.2022.1.as1","DOIUrl":"https://doi.org/10.53141/peqacp.2022.1.as1","url":null,"abstract":"Environment and health This issue goes to press shortly after the inclusion in the Italian Constitution of a law on Environmental Protection, (8th February 2022) amending two articles: Article 9 in which is added the text: \"the Italian Republic ... protects environment, biodiversity and ecosystems, also in the interest of future generations\", and Article 41 in which is added the text: \"Private economic initiative... may not be carried out in a manner contrary to social utility or which adversely affects security, freedom, human dignity, health and the environment.\" This is a first step towards the perspective promoted by the Global Alliance for the Rights of Nature: including the rights of Nature or of the Earth in the Constitutions gives them a legal value (https://www.garn.org/). It certainly bodes well to have written down the interests of future generations and the protection of biodiversity. Precisely for this reason we believe that It is important to stress the need to move on to concrete actions, such the necessity to control soil, air and water pollution. All this needs to be resolved now, and further delays are not tolerable. In this issue, which summarizes the main articles published in the monitored journals, a great deal of data has been collected showing that environmental contamination harms human health, and that human health rights are in line with both the duty to protect environment and the right of the Earth to be protected. This study conducted in 2 emergency departments in Canada randomized 281 children aged 6 months to 10 years with CAP without the need for hospitalization to treatment with high-dose amoxicillin for 5 days versus a traditional 10 day therapy. In terms of clinical recovery, both groups presented comparable results. In fact, the “per protocol” analysis, recommended for a “non-inferiority” design, did not formally provide this result. This Australian study followed 1.910 children from the age of 7.5-10 months, collecting data on 12 risk factors associated with language development. A subgroup of 839 children was analyzed with full follow-up at 11 years, language assessment at 2, 4, 5, 7 years and evaluation of school results with national tests at 8, 10 and 12 years. Early risk factors were good predictors of school achievement at 11. Language proficiency at 4 years was the best predictor of language outcomes at 11 years. In the patients analyzed, a concentration of rare and probably pathogenic genetic variants of immune-related genes was found, and a possible association between genetic results, onset of MIS-C and resistance to treatment was also scored. Significant dysregulation of inflammatory markers was found in all patients analyzed, while mucocutaneous and gastrointestinal manifestations were found in 36 patients (80%), cardiac alterations in 22 (48.9%), neurological manifestations in 14 (31.1%). Genetic alterations in immune-related genes, including TLR3, TLR6, IL22RA2, IFNB1, and IFNA6, were identified in 19 patients (42.2%), with a significant difference with the control group (29 vs 3, P = <0.001). Patients with these variants tend to have an earlier onset of disease than controls (7 patients [36.8%] with genetic variants were 3 years younger at onset than 2 patients [7.7%] without genetic variants) and more resistance to treatment (8 patients [42.1%] with genetic variants who received 2 doses of intravenous immunoglobulins, compared to 3 patients [11.5%] with no evidence of genetic variants). The results of the study suggest that rare and probably deleterious genetic variants may play a role in MIS-C. This is a relatively simple and low-cost therapy that is feasible even in resource-limited countries. A total of 120 patients (60 per arm) were enrolled; treatment efficacy was assessed by questionnaires (ACT/CACT) showing that in treated patients there is a statistically significant improvement in symptoms and a marked increase in patients with well-controlled asthma according to GINA guidelines, which is not the case in the control arm taking baseline therapy alone. Failure to recognize abuse in its less severe forms can lead to repeated abuse and increased morbidity and mortality. In fact, a significant percentage of children with abuse injuries had been previously evaluated by a physician who did not recognize the abuse. Many screening programs have been created using flowcharts or automated checklists to improve the detection of maltreatment in all its forms or to be actively carried out by the health worker in contact with the child. We are experiencing climate change and have to manage and mitigate the effect of the scarcity of water resources. This does not only affect farmers or growers and ranchers but all of us because the water shortage translates into a reduction in crops. The dissociation between public health policies and climate action, which results in millions of avoidable adverse outcomes and deaths every year is worrying. But maybe something is moving: the Ministry's Ecological Transition Plan proposal published in August 2021 for the first time reports the health benefits of climate action and calls for a health approach in all policies. The importance of soil remediation from toxic substances still highlights the difficulty and complexity of the problem, but also the importance for human health of adequate remediation and control systems. The presence of green spaces in schools and urban areas is confirmed as a winning choice for the children’s health as the indications not using electronic devices in the first 1000 days. This issue summarizes the main articles published in the monitored journals, which allows us to deepen these issues.
