In the study analyzed, aimed at reviewing the feasibility, acceptability and safety of HAH for bronchiolitis and assessing the cost impact on hospitals and society, 10 studies that met the inclusion criteria were analyzed. Home oxygen therapy (HOT) was used in all studies, and HOT was found to be feasible in terms of utilization (70%-82%) and successful completion. In the reviewed studies, 7 adverse events (0.6%) with no mortality were reported in 1.257 patients. On costs, the studies showed some savings, although only costs to hospitals were analyzed. In conclusion, there is evidence to support HOT as feasible, acceptable, and safe. Cost-effectiveness evidence remains limited. Further studies are needed to understand the relevant impact of HAH with regard to oxygen therapy, nasogastric feeding support, and remote infant monitoring.","PeriodicalId":168380,"journal":{"name":"PAGINE ELETTRONICHE DI QUADERNI ACP","volume":"106 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121918440","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The mother-child interaction was assessed in terms of maternal linguistic \"input\", maternal responsiveness and non-responsiveness, and conversational shifts, whose operational decoding were defined. Mothers engaged with smartphones or printed magazines produced fewer expressions and responses to baby's calls, missed baby's calls more often, and exchanged fewer conversational shifts compared to the free play setting. The quality of maternal responsiveness was also reduced. Smartphone are constantly used in current daily life and a negative effect on the development of the child is hypothesized. There is growing interest in the effects of parental use of smartphones on the relationship with the child, but data are still very limited, and the complexity of the aspects to be considered insufficiently studied.","PeriodicalId":168380,"journal":{"name":"PAGINE ELETTRONICHE DI QUADERNI ACP","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"117312970","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This is what emerges from this bimonthly review. Some articles put in evidence the importance of soil remediations for the health of people living near polluted sites. Others articles underline children's vulnerability to the heavy metals present in street dust or the risks of pesticides for pregnant women and future offspring. Other articles emphasize the importance of environmental education to increase awareness both in children and teenagers and how activities in natural environments can help cognitive development as well as physical and mental health. Finally, it is highlighted how much work still needs to do be done to train health workers on mitigation and adaptation actions to the impacts of climate change we are experiencing. A part of this work is also up to us pediatricians, and this is one of the tasks of this review. This issue summarizes the main articles published in the monitored journals. Age (≤15 days of life), and blood values of procalcitonin (PCT ≥0.6 ng / mL) and C reactive protein (CRP ≥ 20 mg / L) were independent risk factors for invasive bacterial infection (RISeuP score). This score was then applied prospectively to 449 infants enrolled in 21 ERs and managed according to the individual centers’ protocols, for validation. The results were comparable, however, in the absence of data relating to the treatment received, the authors' conclusions regarding the possible home management of infants at low risk of IBI require careful verification.","PeriodicalId":168380,"journal":{"name":"PAGINE ELETTRONICHE DI QUADERNI ACP","volume":"7 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130535729","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

ARCT with not reassuring results. The 2021 guide of the Global Initiatve for Asthma (GINA) states that the use of oral steroids in preschool children with acute bronchospasm is only recommended in cases of severe exacerbation, however the literature continues to present conflicting results. The Wheeze and Steroids in Preschoolers (WASP) Study, conducted in 3 New Zealand emergency rooms, randomized 477 children aged 24–59 months with acute bronchospasm to receive oral prednisolone for 3 days versus placebo. The respiratory outcomes measured were conflicting: the change in severity score (PRAM) at 24 hours was not different between the 2 groups, while the absolute value of the score at both 4 and 24 hours (secondary outcomes) was significantly lower in the prednisolone group. Hospitalization rate, need for further oral prednisolone treatment, and intravenous drug use were also lower in the prednisolone group. It is debated whether the equivalence design of the study and the margins chosen by the authors for the primary outcome have attenuated the difference between the treatments.","PeriodicalId":168380,"journal":{"name":"PAGINE ELETTRONICHE DI QUADERNI ACP","volume":"217 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121728587","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The half-hour sessions in which the parent-child interaction is recorded are followed by the discussion of the video and the provision of educational material appropriate to the child's development level (book, toy, ...) aimed at improving child and pa- rent interaction. 400 families of two large US cities randomized to intervention (VIP) or to receive usual care were involved. An increase in cognitive skills and parent-child interaction was obser- ved in the intervention arm through three different questionnaires at 6 months of age (quality of words, verbal response to parents, involvement of the parent in the advancement of the child's deve- lopment). This is an interesting study that shows how in primary care it is possible to improve parenting practices and prevent dispa- rities in neuropsychological development in families at high risk of educational poverty.","PeriodicalId":168380,"journal":{"name":"PAGINE ELETTRONICHE DI QUADERNI ACP","volume":"38 32","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"113971219","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Inclusion criteria matched those commonly used for the diagnosis of acute bronchitis both in clinical practice and in previous Cochrane reviews. 432 children were randomized to the antibiotic (n=221) or placebo (n=211) group. Parents completed a diary of symptoms and daily activities for at least 1 week and until persistence of symptoms in the 4 weeks following randomization. The average duration of symptoms was not significantly different in the 2 groups (5 days in the antibiotic group vs. 6 days in the placebo group), nor in subjects belonging to specific subgroups with symptom severity in which antibiotic is commonly prescribed. Despite the limitations related to the subjective evaluation of the outcomes, the study allows us to conclude that, in the absence of clinical suspicion of pneumonia, in most children with lower respiratory tract infections, doctors should provide explanations on the course of the disease and not prescribe antibiotics.","PeriodicalId":168380,"journal":{"name":"PAGINE ELETTRONICHE DI QUADERNI ACP","volume":"46 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132674813","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The results of a \"network\" meta-analysis Responsiveness is the dimension of parenting most strongly associated with positive developmental outcomes in children. In this study, with the methodology of network meta-analysis, the optimal combination of teaching methods for adults about responsive care for children aged 0-6 was investigated in order to determine how to develop effective training programs in the general population to improve the responsive skills of caregivers. 119 studies (12.376 participants) were collected. 71% of parents who receive these interventions improve their responsiveness. The authors conclude that programs aimed at the general population can be low-cost, short (less than a year), carried out in groups, for target parents of 1-6 years old children. These programs can also be focused on responsive care, can use observation of responsive caregiving models and the assignment of homework. Individual video feedback, home visiting and work-intensive programs, should be reserved for particular groups of parents, not included in this study, such as parents with psychiatric problems.","PeriodicalId":168380,"journal":{"name":"PAGINE ELETTRONICHE DI QUADERNI ACP","volume":"34 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125200228","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
