Questo induce le famiglie a viaggi talora impegnativi per raggiungere il centro specialistico che ritengono più adeguato alle necessità dei propri figli. Di conseguenza, pochi ospedali specializzati vedono la grande maggioranza di questi pazienti depauperando ulteriormente le strutture viciniori più piccole dell’esperienza potenzialmente utile nei momenti di possibile criticità. Being a social process, shared reading in the family improves the child’s psychosocial development and parenting skills. It has recently been shown that cognitive and linguistic stimulation are associated with an improvement in self-regulation skills. Panza","doi":"10.53141/peqacp.2022.5.am1","DOIUrl":"https://doi.org/10.53141/peqacp.2022.5.am1","url":null,"abstract":"Strategies for reducing antibiotic prescriptions in primary care: from point-of-care tests in the clinic to managing communication without giving up evidence of efficacy The Point-of-care test (POCT) is a diagnostic test that can be performed in the outpatient clinic or at the patient’s bedside; the simplicity of execution and the availability of the result of POCT during the visit has led to a strong diffusion of these diagnostic aids. The results of the trial reviewed in the article of the month show a lack of effectiveness of a POCT, detecting 18 viruses and 3 bacteria, in reducing antibiotic prescribing in children visited for respiratory infection. This pragmatic study carried out in primary care in Spain compared three intervention arms: delayed antibiotic prescription, immediate or no prescription; the 30-day follow-up revealed no differences in the duration of the disease or in the severity of symptoms in the three groups of children. I risultati di un RCT","authors":"Gruppo di lettura di Milano","doi":"10.53141/peqacp.2022.4.n1","DOIUrl":"https://doi.org/10.53141/peqacp.2022.4.n1","url":null,"abstract":"Oral immunotherapy for peanut, better as a child; results of an RCT Oral immunotherapy (OIT) in peanut allergy is to date considered experimental, although numerous experiences have documented its protective efficacy to the risk of severe reaction in case of accidental exposure by increasing the tolerance threshold. In some cases, upon discontinuation of treatment it has been possible to maintain this result for a short period. This randomized trial included 146 peanut-allergic children under 4 years of age in a 134-week continued oral immunotherapy treatment. The children underwent double-blind, placebo-controlled triggering test (DBPCFC) at enrollment (cumulative dose up to 500 mg), at week 134 of therapy, and 26 weeks after discontinuation of OIT (cumulative dose up to 5.000 mg). Oral immunotherapy induced desensitization in the majority (71%) of treated children and remission in a smaller part (21%), especially in the youngest ones with lower specific IgE levels at the beginning of treatment. Results of the English RCT CAP-IT This study is the first one performed in Western countries that used a 3-day therapy for uncomplicated community-acquired pneumonia (CAP), already tested several times in low- and medium-income countries. This study, carried out in UK, is a double-blind RCT of good methodological quality. The study compared the efficacy of 4 different therapeutic regimens by dose (high or low) and by duration (3 or 7 days) versus placebo, in the treatment of 824 children older than 6 months and weighing between 6 and 24kg with CAP, discharged from emergency room or hospital. Approximately 12% of cases across all regimens require further antibiotic therapy for a new respiratory infection within 28 days from randomization; 5% of children enrolled experienced a severe adverse event requiring hospitalization. Both groups demonstrated noninferiority with no significant interaction between doses and duration. In the subgroup with more severe symptoms at enrolment (i.e., those with at least two altered thoracic physiologic parameters) 17.3% of recipients of a lower dose versus 13.5% of those treated with the higher dose require additional antibiotic therapy by 28th day, but the difference is not statistically significant.
