{"title":"Përdorimi i diod lazerit në mjekimin endodontik te periodontitet apikale kronike","authors":"Agron Meto, Aida Meto, Etleva Qeli","doi":"10.59138/ymffeoxwsknml","DOIUrl":"https://doi.org/10.59138/ymffeoxwsknml","url":null,"abstract":"The complete elimination of microorganisms from the root canal is difficult due to the complex anatomy of the canal, the microbial invasion of the dentinal tubules, and the for- mation of the intracanal biofilm, which gives rise to contin- uous apical periodontitis. The use of lasers in the treatment of caries complications has led to the improvement of various endodontic diagnoses. The aim is to provide clin- ical-radiological data on increasing the effectiveness and quality of endodontic treatment, using a diode laser with a wavelength of 980 nm and a laser power of 0.8–1.0 W in the radicular canals, in the treatment of chronic apical periodontitis. Materials and methods: Eighty-four patients aged between 20 and 70 years were treated, with a total number of teeth of 91; of these, 64 were mono radicular and 27 multi radicular. We divided the patients into two groups; where the first group was the control one, with 40 patients in which 30 single-root teeth and 11 multi- root teeth were selected and treated with the traditional method by performing chemical-mechanical processes. The second group, which was the study one, treated 44 patients from which 36 single-root teeth with chronic peri- odontitis with periapical destruction and 14 multi-root teeth were selected; in this group, in addition to the chemical-mechanical processes, was also applied the diode laser with a wavelength of 980 nm. The cases were observed clinically and radiologically in dynamics for a period of time ranging from 6 to 12 months. Results: We highlighted that the number of complications in chronic apical periodontitis was reduced to a large per- centage in the diode laser group (study group), which achieved 96 % success after 12 months and 4 % failure. As for the control group, there were many complications in the first 6 months and after 12 months, where the final result was 78.1% success, while complications were 21.9%. Conclusions: The quality of root canal treatment with the traditional endodontic treatment protocol has not been sufficient to achieve a clinical effect for a long time. These chang- es range from an average vertical bone loss of 1.5 to 2 mm and an average horizontal loss of ridge width of 40 to 50% during six to twelve months of healing. The purpose of the study was to present the clinical case of a female patient with Multiple Sclerosis who underwent bone augmentation with xenograft and to show the degree of horizontal and vertical bone regeneration after bone augmentation. The work is a presentation of a case that was carried out in the private Dental Ordinance during the year 2021. The case presented is with horizontal and vertical resorption of the alveolar ridge, in which artificial bone – bone particles (Par- ticulate Bone Grafting Technique) produced by the Bionno- vation biomedical company was used. The measurement of parameters is quantitative. 6 months after guided bone regeneration, through 3D analysis we have the following re- sults: he length of the bone from 7.5mm became 10.7mm, while the width of the bone at the top of the alveolar ridge from 1.7mm to 3.4mm, while the part of the base of the ridge from 5.4mm to 7.4mm. Directed bone regeneration is a procedure that is recommended in cases where we do not have enough bone to place implants. The aim of this paper is intended to summarize up-to-date and evaluate the reliability of subjective data obtained from Fonseka questionnaires in assessing the prevalence and severity of signs and symptoms of tem- poromandibular disorders and oral parafunction habits and to compare the findings with other studies. It is gen- erally accepted that the etiology of TMJ is multifactorial related to a variety of dental and medical conditions such as occlusal changes, body posture, pre-functional habits, restorative treatments, orthodontic treatments, emotion- al stress, trauma, disc anatomy, muscle pathophysiology, genetic conditions and psycho-social, age and gender. Since TMD are a heterogeneous group of pathologies that affect the temporomandibular joint, masticatory muscles or both. The multifactorial etiology and the lack of stan- dardized criteria in the evaluation of Temporomandibu- lar disorders (TMD), despite voluminous literature makes comparison between different studies difficult and even today is a unsolved issue. In some cases, or- thodontic treatment is accompanied by side effects that the orthodontist must pay attention to prevent. The pur- pose of this research is to evaluate the condition of the gin- giva and periodontium in patients with fixed orthodontic appliances, before and after cleaning and sanitizing the mouth with ozone therapy for a period of 3 months. In a group of 45 patients with braces, aged 10-47 years, hy- giene and gingival problems were checked. The main cri- teria were the color of the marginal gingiva and the depth of the gingival pockets in the buccal sulcus. In a period of 3 months we treated them with cleanings and ozone ther- apy of the mouth, every three weeks and compared the results with those of the previous months. Tooth move- ment during orthodontic treatment promotes inflam- matory processes and gingivitis. Poor oral hygiene, foods and bacterial plaque easily adhere to braces. The bacteria present in this plaque cause gingival inflammation. Pa- tients in altered health conditions such as during pregnan- cy, diabetes, obesity are more prone to gum disease. Poor oral hygiene leads to food sticking and the creation of bacterial plaque around braces. The tooth reinforced with reconstruction fulfills the functional and aesthetic criteria and as such can be the carrier of fixed and mobile prosthetic work. There- fore, the purpose of this in vitro study was to determine and compare the fracture resistance of endodontically treated teeth reinforced with cast metal and fiber-post restorations. In this in vitro study, in samples of 60 intact canines extract- ed for periodontological reasons, the crown was removed, 2 mm below the enamel-cementum border. The teeth un- derwent endodontic root canal treatment and were divid- ed into two experimental groups (n=30) depending on the type of reconstruction applied: cast metal and fiber-post. For fracture resistance research, compressive load at an an- gle of 130° in the direction of the longitudinal axis of the tooth was used with the Universal Testing Machine (Matest, Italy). The testing was done with the help of a 1mm wide steel extension at a speed of 0.5mm/min that was posi- tioned on the oral surface, in the loading area that was lo- cated 2mm below the incisal blade and that served for sta- ble uniform loading. The load was continued until fracture and the values obtained are expressed in Newton (N). A priori reconstructions contribute to increasing the tooth’s frac- ture resistance. The data from this study are facts, which make it possible for fiber-post to be the right choice of dentists in dai- ly clinical practice due to its higher resistance. Therefore, in order to find a new alternative, we have de- termined that the purpose of this in vitro research is: to evaluate the antibacterial efficiency of the extract of Thy- mus serpyllum alone and in combination with NaOCl 2%, as well as Chlorhexidine against Enterococcus faecalis in the solution with dentin powder using the agar disk diffusion method. The zones of inhibition caused by the herbal ex- tract of Thymus serpyllum were evaluated after 5 minutes, 60 minutes and 24 hours, and were compared with the standard solutions of NaOCl 2 % and Chlorhexidine after mixing with the dentin powder. The statistical analysis of the results was done with One Way ANOVA and the Kruskal Wallis test. Sodium hypochlorite 2% has shown antibacte- rial effect against Enterococcus faecalis by forming a wide zone of inhibition, but after mixing with dentin powder, its effect is reduced. Chlorhexidine 2% has shown an antibac- terial effect against E.faecalis, giving an inhibition zone, which is reduced under the influence of dentin powder. Chlorhexidine after mixing with plant extracts of Thymus serpyllum has resulted in more powerful antibacterial effect against E.faecalis. The X-effect is associated with additional complications to the teeth and periodontium that develop as a result. This pa- per presents the clinical retreatment of a patient who presented himself with an X-effect, 11 years after the completion of the previous orthodontic treatment.
