Kourkouta","doi":"10.5604/01.3001.0053.7461","DOIUrl":"https://doi.org/10.5604/01.3001.0053.7461","url":null,"abstract":"Background: Burnout is found at elevated levels inhealthcare professionals who work in critical units.At the same time, high levels of burnout areassociated with strong negative effects on a personaland professional level.Aim: The aim of this study was to examine thefactors associated with burnout and its associatedconsequences in workers in surgical departments andintensive care units.Materials and Methods: A systematic review of theliterature was applied. The search and inclusion ofarticles in the systematic review was performedusing the PRISMA Statement. A total of 31 differentstudies were included in the search.Results: Firstly, most of the relevant studiesconcerned nurses, not physicians or other healthprofessionals. Second, all the studies were crosssectional. Third, a common finding of the effects ofsociodemographic factors cannot be drawn as therewere conflicting findings between the studies.Fourth, it appears that evidence that demonstrates abroader burden of mental health is associated withburnout (e.g. anxiety). Fifth, parameters that arerelated to resilience, for example hardness as apersonality trait, were associated with lower burnoutscores. It was aimed to reach the wholeuniverse, no sample size was calculated. Thequestionnaire was delivered via WhatsApp inNovember-December 2021. Sociodemographicinformation, skin cancer risk factors, sun protectionmethods were questioned (38 questions). Thebehaviors of sun protection methods were evaluatedby scoring the questions (6 questions). Higher scoreindicates higher protection behavior. Median,minimum, maximum values, numbers (n) andpercentages (%) were used for descriptive data. TheMann Whitney U test was used to compare thequantitative variables that did not fit the normaldistribution. The significance level was consideredas p<0.05.Results: Of the participants 58.3% (n=354) werewomen, 25.2% (n=153) were sixth grade. Of thestudents 34.9% (n=212) use sunscreen, 32.3%(n=196) use a hat, 39.9% (n=242) use sunglassesonly in summer. Of the students 48.9% (n=297) hadFitzpatrick skin type 3, 20.3% (n=484) reportedhaving more than 50 nevi, 37.2% (n=226) reportedhaving nevi larger than 2 mm. Female students,students educating in upper classes and living incoastal areas, having Type 1-2-3 skin types andfreckles had higher sun protection behavior scores(p<0.05).Conclusions: The rate of medical faculty studentspaying attention to sun protection methods is notsufficient. It isusually found in women over 60 years of age as apalpable tumour of the breast or armpit. Due to thesmall number of relevant case reports, this type ofcancer presents a diagnostic and therapeuticchallenge.Case presentation: A 85-year-old woman showedup at the Oncology Clinic with a palpable node in theleft breast. On physical examination, the patient wasin general good condition, ECOG performancestatus of 1. In deviations from the norm, there was apalpable tumour with a diameter of ca. 4cm in thelower outer quadrant, 4 o'clock position of the leftbreast; and an enlarged, movable left axillary lymphnode, with a diameter of about 3 cm. The diagnosis,strongly influenced by the abovementioned tests,was small cell breast cancer TNBC-cT2N1M0. Asagreed at the case conference meeting, the patientwas qualified for neoadjuvant chemotherapyaccording to the 4-EP scheme, followed bymastectomy with lymphadenectomy of the leftarmpit and complementary radiotherapy. The patientreceived four courses of chemotherapy from 28March to 30 May 2023. The visible tumourregression in physical examination revealed goodtreatment tolerance – a reduction of tumour size onpalpation by half. The patient is undergoing followup checks after chemotherapy and awaits ascheduled surgery.Conclusions: Literature on primary small cell breastcarcinoma is still limited . Diagnostics of SCNCBinclude positron emission tomography (PET) and CTscans to rule out metastases from another primarylocation. Clinical treatment of SCNCB is stillunderdeveloped. A particularly important prognosticfactor is the size of tumour at the time of treatment.Early detection and interdisciplinary therapies maybe relevant for improving prognosis. The pandemic outbreak wasassociated with a reduced quality of life forpopulations around the world and contributed towidespread health, social and economic impacts.The aim of this study was to assess the level ofinternet addiction in a group of Poles living inPoland and the UK in light of the COVID-19pandemic.Material sand Methods: In January 2023, an onlinecross-sectional survey was conducted among Polishadults living inPoland and the United Kingdom. Thequestionnaires in the form of a link to the form weresend via Facebook to groups of Poles living inUnited Kingdom and in the private social media ofthe researcher. The Kimberly Young questionnairewas used to assess the extent of internet addictionamong Poles living in Poland and the UnitedKingdom.Results: Among the respondents, women slightlypredominated (51.5%). The vast majority of therespondents were people who suffered fromCOVID-19 at least once during the pandemic (90%)and were vaccinated against SARS-CoV-2 (61.5%).The analysis of the results showed that 24.5% of therespondents abused the Internet, there were nopeople strongly addicted to the Internet in the studygroup. It usually occurs in teenagers oryoung women below 40 years. Most SPNs areasymptomatic or minimally symptomatic andincidentally found by imaging. The most commonclinical symptoms are abdominal pain andabdominal tumour.Case presentation: A 41-year-old woman withmorbid obesity and type 2 diabetes was admitted tothe hospital with upper abdominal pain, vomiting,and diarrhoea. The ultrasound found a cystic lesionin the tail of the pancreas. Subsequent MRIexamination revealed polycyclic litho cystic tumour63x 52 mm, 72mm c-c within the tail of the pancreas.Afterwards, the pancreatic tail with the tumour waslaparoscopically resectioned. Histopathologicalevaluation of collected tumour tissues found a solidpseudopapillary neoplasm (SPN) in those pancreaticcells. Subsequently, the patient was prescribed anadditional 2U of Lispro insulin before each andCreon enzyme replacement therapy for the SPN. Amonth after the procedure, the patient was admittedto the ICU with sharp abdominal pain, abdominalcramps, and high inflammatory markers. She wassuccessfully treated for acute pancreatitis anddischarged after ten days. The patient is in stablecondition and continues the Creon treatment.Conclusions: SPN can be detected by ultrasound,computed tomography, magnetic resonanceimaging, and positron emission tomography. SPN isa type of cancer with low malignancy potential dueto the low metastasis and vascular invasionprobability. Metastases to other organs have beenreported in 15% to 20% of SPN. Distant metastasesare typically found in the liver and lymph nodes, butthe peritoneum, omentum, and lungs can also beinvolved. Instability is often accompanied by pain, possible damage to neurological structures, and progression of the deformity. Symptoms resulting from cervical instability mainly affect young, middle-aged, and women. Assessing the instability of the cervical spine is often problematic. CT (computed tomography) and MR (magnetic resonance) examinations should be performed to diagnose patients with peak-rotational instability. There are several recommendations in the literature for treating patients with cervical instability; however, there are still no common guidelines. Guzińska-ustyMowiCz","doi":"10.5604/01.3001.0016.1742","DOIUrl":"https://doi.org/10.5604/01.3001.0016.1742","url":null,"abstract":"Introduction: The aim of this study was to analyze the technical evaluation of cervicovaginal smears as well as the correlation of the technical quality of the smear with the patient’s age. \u0000Materials and methods: The study included a group of 43,696 women who underwent exfoliative cytology. The smears were stained according to the Papanicolaou method and then underwent cytological evaluation based on the Bethesda system.\u0000Results: The technical quality of the cytology smears was found to be satisfactory (AI) in 80.6% of cases, acceptable for cytological evaluation (AII) in 19% mainly as a result of poor readability of the smear due to numerous inflammatory cells (35.5%), whereas not suitable for evaluation (AIII) was in 0.4% of cases. Moreover, there was a high proportion of smears without cells from the cervical canal in women under 50 years of age. In women over 50, the most common cause of conditional evaluation of the smear was its excessive dryness\u0000Conclusions: In all age groups, an abundance of inflammatory cells was a factor that hindered cytologic evaluation of the smear. Only representative material containing cells from the cervical canal should be collected. The presence of lymphocytic infiltration in the main mass of the tumor and its front indicates a different clinical course of the disease. The aim of the study was to presence of CD3+ T lymphocyte infiltration in the tumor and its front in correlation with clinicopathological parameters. \u0000Materials and Methods: The study included a group of patients operated on due to colorectal cancer. The histopathological results of the patients were analyzed, including the assessment of the expression of CD3 lymphocytes in the main mass of the tumor and its front, and an analysis of correlation with the patient’s age, sex, histological malignancy stage, presence of metastases to lymph nodes and distant metastases was performed. \u0000Results: We found the correlation between the differential infiltration from CD3+ T lymphocytes in the front of the invasion and the depth of tumor infiltration (p=0.008). Both effective self care\u0000management and e-health literacy are important\u0000terms in order to control of the chronic diseases. The\u0000aim of this study is to determine the relationship\u0000between e-health literacy and the self-care\u0000management of patients with Type-2 diabetes.\u0000Materials and Methods: The type of the study is\u0000descriptive. A questionnaire was applied to\u0000individuals who have been diagnosed with Type-2\u0000diabetes in the endocrine policlinic of the internal\u0000medicine clinic of a tertiary state hospital in Istanbul,\u0000Turkey. The sample size was calculated as 384 by\u0000accepting the adequate e-health literacy rate among\u0000Type-2 diabetes patients as 50%, the margin of error\u00005%, and the confidence level as 95%. In addition to\u0000sociodemographical questions; our questionnaire\u0000contains two scales: Self-Care Management\u0000Protection scale and the E-health Literacy Scale.\u0000Number (n) and percentage (%) values were used in\u0000the expression of descriptive data. Mean, standard\u0000deviation, median and minimum, maximum values\u0000were given. For data comparison; Mann-Whitney U\u0000test was used for non-normally distributed variables.\u0000The Spearman correlation used to measure the linear\u0000relation between two numeric variables.\u0000Significance level has been accepted as p<0.05.\u0000Results: Totally 330 patients were included our\u0000study by a participation rate of 85.9 %. The mean age\u0000of the participants was 57.74±13.83 and 52.4% were\u0000female. A positive correlation was observed between\u0000scores of e-health literacy scale and self-care\u0000management scale (r=0.235, p<0.001) and self-\u0000protection subscale of self-care management scale\u0000(r= 0.345, p<0.001). It is also estimated that about 30-50 percent. people have suffered from depressive disorders even once in their lifetime. The rela-tionship between depression and addiction is two-way, because people with depression are more likely to use stimulants, and depression itself is the most common complication of addic-tion. A disturbing phenomenon is the frequent use of addictive substances by people with de-pression: mainly alcohol, but also drugs or medi-cations. Depression and addiction are said to reinforce each other, and this is increasingly ruining the patient's condition. Any addictive or self-destructive behavior can result in symptoms of depression. The paper analyzes the available literature in the aspect of the problem of depres-sion in addicts.
