Data from the literature indicate about the development of diabetic polyneuropathy in 5 years after the onset of type 1 diabetes and in 10-15% of patients with newly diagnosed type 2 diabetes. \u0000The purpose of the study was to find out the clinical course of diabetic polyneuropathy and to determine the frequency of the occurrence of diabetic foot syndrome in conditions of the comorbidity. \u0000Methods. We examined 121 patients, aged from 19 to 69 years, diagnosed with diabetic polyneuropathy due to type 1 and type 2 diabetes. The examined patients were divided into three groups: with diabetic polyneuropathy and cardiovascular comorbidity (group A, n=52), multimorbidity (more than 3 nosologies) (group B, n=30), without comorbidity (group C, n=39). \u0000Conclusions. Clinical manifestations of the diabetic polyneuropathy were more significant in the individuals with the cardiovascular comorbidity. Svyrydova","doi":"10.33444/2414-0007.9.2.8-13","DOIUrl":"https://doi.org/10.33444/2414-0007.9.2.8-13","url":null,"abstract":"The objective of our research was to define the relationship between orthostatic hypotension (OH) and frailty syndrome (FS) in the older age group. \u0000Materials and methods: the cross-sectional study involved patients of the older age group diagnosed with Parkinson's disease (PD) and cerebrovascular disease (CVD - chronic cerebral ischemia, chronic phase of ischemic stroke). All patients had comorbid OH. OH was established on the basis of a classical orthostatic test [10]. Each group included patients with different functional classes of OH (FC OH), from 1 to 4 [7]. The functional class of OH was determined using the orthostatic hypotension questionnaire (assessment of OH symptoms and the OH daily activity scale) [8]. All participants were assessed for the FS index based on the Edmonton Frail Scale [9]. \u0000Results: OH and FS has a close relationship, which is reflected in the direct correlation between OH and FS index (r=0.9). The higher FC OH, the more severe FS (p=0.001). The FC OH 1 corresponds to vulnerable, FC OH 2 to mild FS, FC OH 3 to moderate FS, and FC OH 4 to severe FS. \u0000Conclusions: OH, having a close relationship with FS, may make an additional contribution to the reduction of the duration and quality of life of patients with FS and elderly patients [3, 5]. Unfortunately, currently, OH has not been included in the criteria of FS and is not evaluated in this cohort [9]. Therefore, we believe it is important to perform orthostatic testing in all patients with FS, which will prevent the occurrence of life-threatening complications caused by the persistence of OH [3, 5]. Materials and methods. Specialized medical literature, medical databases PubMed and Medscape were analyzed. Discussion. In the genesis of the acute sensory neuropathy take part sharply pronounced fluctuations of glycemia, in connection with which they can be united into a single pathogenetic whole. A rare lesion of the peripheral nervous system in diabetes is “Bruns-Garland syndrome”. In all patients at the stage of clinically asymptomatic neuropathy, on electroneuromyography are showed the signs of sensory axonal polyneuropathy, mainly of the lower extremities. {"title":"ORTHOSTATIC HYPOTENSION AS A NON-MOTOR SYMPTOM OF PARKINSON’S DISEASE","authors":"A.S. Ovchynnykova, Y. Materials and methods. This work was performed by searching for current information on OH in PD, reflecting the pathophysiology, classification, symptoms, diagnosis, and treatment. PubMed and Google Scholar resources were used to write the review. Results. An important task in the management of patients with PD is diagnostic and treatment of OH due to the negative impact of the syndrome on somatic, psychological and cognitive status. According to recent data, the prevalence of OH among PD patients is 30-50% [1] with no association with the stage of the disease [2]. OH in PD is neurogenic (NOH); it results from neurodegenerative damage to the central and peripheral structures of the sympathetic nervous system, which leads to reduced postganglionic release of epinephrine [3]. It is often asymptomatic, as a result of compensatory tolerance mechanisms [1]. In about of 50% of patients [4], OH is associated with supine hypertension [2] and may have a connection with REM-sleep disorders [5]. Classification features of OH include: onset period under orthostatic stress, the relationship of OH to different parts of the cardiac cycle, mechanism of pathophysiology, clinical course, presence of symptoms and clinical severity [20, 31, 32]. OH symptoms usually occur during orthostatic stress. They include dizziness, blurred vision, cognitive slowing, syncope, coat-hanger pain, difficulty breathing, leg buckling or leg weakness, general weakness and fatigue [1, 5, 17, 18]. Bedside orthostatic test with blood pressure measurement at the 1st, 3rd and 5th minutes of standing and ambulatory blood pressure monitoring (ABPM) are used for diagnosis [6]. The treatment strategy is to evaluate the medications taken by the patient and obligatory inclusion of non-pharmacological treatment. Pharmacological treatment for supine hypertension and OH is only given if necessary [1, 2]. Conclusions. OH is a widespread non-motor symptom of PD [1], which should be timely diagnosed and treated because of the negative impact on quality of life with increased risk of death and injuries due to falls [31, 32, 33]. We found scant information on extracerebellar manifestations of SCA5.\u0000Objective: The purpose of our review was to find any information about extracerebellar signs of spinocerebellar ataxia type 5.\u0000Methods: PubMed and Google Scholar resources were used to write the review article.\u0000Results: Ranum L.P., et al. (1994), and Bürk K., et al. (2004) described writer’s cramp dystonia, facial myokymia, limited lateral gaze, brisk reflexes, and impaired vibration sense in patients with SCA5 [1, 2]. \u0000Maschke M., et al. (2005) noticed the possible presence of hyperreflexia, incontinence, tingling and cramps, diplopia, horizontal and vertical ophthalmoparesis in patients with SCA5 [3].\u0000Spagnoli C., et al. (2020) documented infantile-onset SCA5 with optic atrophy and peripheral neuropathy, and bilateral cataracts as associated findings [4].\u0000Stevanin G., et al. Early diagnosis and timely treatment (gluten-free diet) of gluten ataxia leads to a significant improvement of clinical symptoms and prevents their progression. Gluten ataxia is one of the most common and commonly misdiagnosed ataxia. The objective of our research was to study the attitudes towards disease in patients with chronic phase of ischemic stroke. Material and methods. 100 consecutive patients with chronic phase of ischemic stroke and 17 patients with chronic cerebral ischemia were questioned in neurology in-patient department. The types of attitude towards disease were identified with the help of the Personal Questionnaire of the Bekhterev Institute. Results. We found that pathological types of attitude towards disease begin to form before the development of a stroke as a result of chronic cerebral ischemia. Among patients with chronic phase of ischemic stroke, pathological types of attitude towards disease prevailed significantly, and the harmonious type was rare (84.0% vs 16.0%, p < 0.001). The most frequently observed were neurasthenic (36.0%), harmonious (16.0%), anxious (16.0%), ergopathic (10.0%) and sensitive (8.0%) types of attitude towards disease.
