Patients in the control group were treated with acitretin orally, and patients in the combined group were given NB-UVB irradiation treatment based on the treatment of the control group. The efficacy, skin lesions, itching symptoms, immune function and incidence of adverse reactions were compared between the two groups. \u0000 \u0000 \u0000Results \u0000After treatment, the total effective rate of treatment in combined group was 95.00% (38/40), significantly higher than the 77.50% (31/40) in control group (P 0.05). After treatment, the dermatological life quality index scale score in combined group was significantly lower than that in control group (P<0.05). \u0000 \u0000 \u0000Conclusions \u0000NB-UVB combined with acitretin has significant efficacy in the treatment of psoriasis vulgaris. Among them, 80 patients aged over or equal to 80 years were set as observation group, 106 patients aged less than 65 years old were set as control group. General information, comorbidities, ERCP operation and postoperative complications of the two groups were analyzed. \u0000 \u0000 \u0000Results \u0000The rates of hypertension, cerebrovascular disease and diabetes mellitus in the observation group were higher than those in the control group, and the differences were statistically significant (P 0.05). The rate of performing endoscopic sphincterotomy in the observation group (67.50%, 54/80) was greater than that in the control group (44.34%, 47/106), P 0.05). Four patients had failed intubation in the observation group, and their symptoms were improved by percuteneous transhepatic cholangio drainage. In the control group, five patients had failed intubation, and recovered after laparoscopic choledocholithotomy. There was no significant difference in the incidence of complications between the two groups (P>0.05). \u0000 \u0000 \u0000Conclusions \u0000Emergency ERCP is safe and effective in the treatment of elderly patients with acute cholangitis complicated with common bile duct stones, but close attention should be paid to comorbidities and early detection of possible adverse events. {"title":"Relationship between PAK6 expression and radiosensitivity in breast cancer patients undergoing radiotherapy","authors":"Xiaohong Wang, Zhenkun Han, Jingjing Wang","doi":"10.3760/CMA.J.ISSN.1674-4756.2020.03.006","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1674-4756.2020.03.006","url":null,"abstract":"Objective \u0000To investigate the relationship between P21-activated kinase 6 (PAK6) expression and radiosensitivity of breast cancer patients undergoing radiotherapy. \u0000 \u0000 \u0000Methods \u0000MDA-MB-453 breast cancer cell line, small interfering RNA (siRNA), and cells transfected with PAK6 siRNA were set as untransfected group, control group and observation group, respectively. Western blot and quantitative real time polymerase chain reaction were used to detect the expression of PAK6 in each group. Every group was given 0, 1, 3 and 5 Gy radiotherapies, respectively, with one radiation dose for once. Thereafter, cell proliferation was detected by cell counting kit-8, and apoptosis was monitored by flow cytometry. Western blotting was used to detect the level of Caspase-3 protein in cells of three groups. \u0000 \u0000 \u0000Results \u0000The PAK6 protein and PAK6 mRNA expression levels of the untransfected group and control group were higher than those of the observation group (P<0.05). The proliferation of breast cancer cells , with the radiotherapy doses of 0, 1, 3 and 5 Gy, in the untransfected group and the control group were higher than that in the observation group (P<0.05); while untransfected group and control group had lower levels of apoptosis than observation group (P<0.05). The expression level of caspase-3 in the observation group was higher than that in the control group and untransfected group (P<0.05). Compared with the untransfected group and control group, the cell inventory score of the observation group was reduced, and the sensitization ratio of radiotherapy was 1.774. \u0000 \u0000 \u0000Conclusions \u0000Interference on PAK6 expression in breast cancer cells can inhibit the proliferation of breast cancer cells, promote the apoptosis of breast cancer cells and increase the radiosensitivity of breast cancer patients. Control group received the TPN, while the observation group received the EN by endoscopic jejunal feeding tube placement. Then, the nutrition level, including serum albumin (ALB), prealbumin (PA), rehabilitation quality (ApacheⅡ), blood amylase level, C-reactive protein (CRP), clinical efficacy and complications were compared between the two groups. \u0000 \u0000 \u0000Results \u0000The ALB and PA levels of the observation group were higher than those of the control group on 14 d after treatment (P<0.05). The Apache Ⅱ scores of the observation group were lower than those of the control group on 7 and 14 d after treatment (P<0.05). The blood amylase level and CRP in the observation group was lower than that in the control group on 14 d after treatment (P<0.05). The total clinical efficacy rate of the observation group was 86.67% (26/30), higher than the 63.33% (19/30) of the control group (P<0.05). Qin","doi":"10.3760/CMA.J.ISSN.1674-4756.2020.03.001","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1674-4756.2020.03.001","url":null,"abstract":"Objective \u0000To study the clinical characteristics of hypertrophic olivary degeneration (HOD). \u0000 \u0000 \u0000Methods \u0000Clinical data from 6 HOD patients, diagnosed and treated in Department of Neurology of the First Affiliated Hospital of Zhengzhou University from January 2013 to December 2018, were retrospectively analyzed, and patients were followed up once by telephone. \u0000 \u0000 \u0000Results \u0000There were 4 cases of unilateral HOD and 2 cases of bilateral HOD. Clinical manifestations were ataxia in 5 patients, limb tremor in 3 patients, bilateral eyeball tremor in 2 patients, bilateral palatal muscle tremor in 1 patient, limb pain in 1 patient, systemic myoclonus in 1 patient, diplopia in 1 patient. Magnetic resonance imaging (MRI) showed high signal on T2 weighted imaging, slightly low signal or isointensity signal on T1 weighted imaging. Susceptibility weighted imaging (SWI) were performed on 3 patients, and no significant increase in the density of the red nucleus was found. One patient presented hyperintensity on fluid-attenuated inversion recovery (FLAIR). One patient underwent enhanced MRI, but no enhancement in the inferior olivary nucleus was found. \u0000 \u0000 \u0000Conclusions \u0000In addition to typical symptoms, HOD also presents atypical symptoms. MRI can clearly show the secondary degeneration of olive nucleus, and SWI can not show the red degeneration. The effect of medicine on limb tremor is not good. Patients in the study group were treated by complete mesocolic excision, while patients in the control group were treated by traditional radical operation. The operation, postoperative complications, inflammatory factors, immune function and outcomes of follow-up were compared between the two groups. \u0000 \u0000 \u0000Results \u0000There was no significant difference in operation time between the study group and control group (P>0.05). The volume of bleeding, time of extubation, time of first anal exhaust and postoperative hospital stay in the study group were less than those in the control group (P 0.05). The levels of C-reactive protein (CRP), tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) increased in both groups 24 hours after operation, but the increases in the study group were lower than those in the control group (P 0.05). The incidence of postoperative complications in the study group (6.06%, 2/33) was lower than that in the control group (21.21%, 7/33), P<0.05. Patients in the control group were treated by small bone window craniotomy, and patients in the observation group were treated by transsylvian approach microsurgery. The treatment outcomes of the two groups were recorded and compared. \u0000 \u0000 \u0000Results \u0000After treatment, the recovery rate of the observation group was 98.11% (52/53), higher than the 62.26% (33/53) of the control group (P<0.05). However, the severe disability rate was 1.89% (1/53) in the observation group, lower than the 33.96% (18/53) in the control group (P<0.05). Compared with the control group, the observation group had better outcomes in the health index, emotional score, social index, life satisfaction and other quality of life indicators (P<0.05). According to the treatment methods, the patients were divided into observation group and control group, with 62 cases in each group. Patients in control group was given symbicort inhalation therapy on the basis of conventional symptomatic treatment, and patients in observation group was given tiotropium inhalation therapy on the basis of treatment of control group. All patients were continuously treated for 12 weeks. According to the random number table method, they were divided into the control group and observation group, with 49 cases in each group. Patients in the control group were given conventional treatment, while patients in the observation group were treated with α-lipoic acid injection based on the treatment of the control group. {"title":"The expression and clinical significance of fractional exhaled nitric oxide and indoleamine 2, 3 dioxygenase in children with allergic asthma","authors":"Chunlin Zhang, Jiangtao Niu, Jinlin Zhang","doi":"10.3760/CMA.J.ISSN.1674-4756.2020.03.019","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1674-4756.2020.03.019","url":null,"abstract":"Objective \u0000To investigate the expression of fractional exhaled nitric oxide (FeNO) and indoleamine 2, 3 dioxygenase (IDO) in children with allergic asthma and their diagnostic values for the disease. \u0000 \u0000 \u0000Methods \u0000Sixty children with allergic asthma hospitalized and treated in Luoyang Maternal and Child Health Hospital from 2016 to 2019 were selected as observation group, and 60 healthy children who underwent physical examination during the same time period in the hospital were selected as control group. The levels of FeNO, IDO, eosinophils and lung function of the two groups were observed, and the correlation among them was also analyzed. Then the diagnostic values of FeNO and IDO levels for allergic asthma were analyzed by receiver operating characteristics (ROC) curve. \u0000 \u0000 \u0000Results \u0000The observation group has higher FeNO level and lower IDO activity than the control group (P<0.05). The forced expiratory volume in 1 second (FEV1)/ forced vital capicity (FVC) levels in the observation group were significantly lower than those in the control group (P<0.05), and the eosinophil count was significantly higher than that in the control group (P<0.05). FeNO levels were negatively correlated with FEV1/FVC in children with allergic asthma (r=-0.667, P<0.05), and positively correlated with eosinophil count (r=0.813, P<0.05). The level of IDO was positively correlated with FEV1/FVC (r=0.725, P<0.05), and negatively correlated with eosinophil count (r=-0.583, P<0.05). ROC curve analysis showed that single FeNO, single IDO and FeNO combined with IDO had certain diagnostic value for allergic asthma in children.
