The endoscopic characteristics of patients with early esophageal and gastric cancer were recorded. The pathological features of the disease were analyzed, and its correlations with lymph node metastasis and invasion depth were recorded. \u0000 \u0000 \u0000Results \u0000There were no significant differences in age, gender, tumor major diameter or depth of invasion among patients with different depths of invasion (P>0.05), but there were significant differences in endoscopic classification and tissue differentiation among them (P 0.05); but there was a significant difference in the degree of differentiation or the depth of infiltration (P<0.05). The results of logistic regression analysis showed that the risk factors of lymph node metastasis were differentiation degree and invasion depth. The endoscopic classification of patients was mainly type Ⅱb, accounting for 36.00% (36/100). There were 40 cases of low differentiation, 34 cases of middle differentiation and 26 cases of high differentiation. Thirty-four cases (34.00%, 34/100) were with tumor major diameter≤1 cm, 51 cases (51.00%, 51/100) were with tumor major diameter≥4 cm, 15 cases (15.00%, 15/100) were with tumor major diameter≥4 cm. \u0000 \u0000 \u0000Conclusions \u0000Endoscopic examination is an effective clinical method for the diagnosis and detection of early esophageal and gastric cancer. According to random number table method, they were divided into control group and observation group, with 36 cases in each group. On the basis of routine treatment, the control group was treated with aspirin, while the observation group was treated with aspirin combined with low molecular weight heparin. The levels of coagulation factors, pregnancy outcomes and adverse reactions were compared between the two groups. \u0000 \u0000 \u0000Results \u0000After treatment, the levels of fibrinogen (FIB) and D-dimer (DD) in the observation group were (3.29±0.27) g/L and (1.85±0.68) mg/L respctively, lower than the (4.56±0.43) g/L and (2.91±0.53) mg/L in the control group (P 0.05). {"title":"Detection and epidemiological analysis of human papillomavirus infection subtypes in males in Dongyang, Zhejiang Province","authors":"Qiaogang Wang, Liya Chen","doi":"10.3760/CMA.J.ISSN.1674-4756.2020.03.007","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1674-4756.2020.03.007","url":null,"abstract":"Objective \u0000To investigate the prevalence and epidemiologic distribution of human papillomavirus (HPV) infection in males in Dongyang area, and to provide scientific basis for the prevention and treatment of related diseases. \u0000 \u0000 \u0000Methods \u0000A retrospective analysis was made on the results of HPV 21 typing test in 712 male patients who were treated in Department of Andriatrics in Dongyang People’s Hospital from January 2018 to December 2018. Data differences were analyzed by SPSS 19.0 software. \u0000 \u0000 \u0000Results \u0000A total of 712 male samples were detected, of which 210 males were HPV positive, with the positive rate of 29.54%. Among the 210 positive samples, there were 141 cases of single infection, accounting for 67.14%, and 69 cases of multiple infection, accounting for 32.86%. After patients were grouped by ages, the highest positive ratio was 6/11 in the > 60 years old group, followed by the <20 years old group (37.50%, 15/40), 21-40 years old group (29.56%, 149/504) and 41-60 years old group (25.48%, 40/157) sequentially. And there were statistical differences among these groups in HIV positive rate (P<0.05). The positive rates in the third quarter and the fourth quarter were 35.20% (63/179) and 32.04% (58/181) respectively, which were significantly higher than the first quarter and the second quarter, of which the positive rates were 28.90% (50/173) and 21.79% (39/179) respectively (P<0.05). Among the 210 HIV patients, the most common symptoms were warts (26.67%, 56/210), followed by penile vegetation (18.10%, 38/210) and urinary tract infection (14.76%, 31/210). \u0000 \u0000 \u0000Conclusions \u0000The HPV infection rate of male in Dongyang area is high, with HPV6, HPV11 and HPV16 as the main types, and most of patients have single type infection. The positive rate of HIV was higher in patients younger than 20 years and patients older than 60 years. The positive detection rates in the third quarter and the fourth quarter are high, and the distribution shows a certain seasonal correlation. Patients in the study group were treated with CHAG chemotherapy regimen, and patients in the control group were treated with arabinosylcytosine, aclarubicin and granulocyte colony stimulating factor chemotherapy regimen. The short-term efficacy and follow-up results of the two groups were recorded. \u0000 \u0000 \u0000Results \u0000The total effective rate of clinical treatment in the study group was 93.33% (28/30), higher than the 73.33% (22/30) in the control group (P<0.05). All patients underwent imaging examination before surgery. Thirty-two cases of parapharyngeal space tumors were resected by trans-cervical approach and 16 cases by trans-oral approach. The therapeutic effects of the two methods were compared. \u0000 \u0000 \u0000Results \u0000Pathological results showed 20 cases (41.7%) of schwannoma, 17 cases (35.4%) of pleomorphic adenoma, 4 cases (8.3%) of neurofibroma, 4 cases (8.3%) of hemangioma and 3 cases (6.3%) of cyst. During hospitalization, 6 cases who were given trans-cervical approach surgery had complications, and 2 cases treated by trans-oral approach surgery had complications. Postoperative follow-up ranged from 10 to 62 months. Two cases treated by trans-oral approach recurred, and 3 cases treated by trans-cervical approach recurred. All of the 5 patients were cured by performing operation again. And they were divided into study group and control group according to the random number table method, with 20 cases in each group. The control group was treated with intrapleural injection of cisplatin, while the observation group was treated with recombinant human endostatin and cisplatin. The total efficacy and serum tumor markers, such as carbohydrate antigen 125 (CA125), carbohydrate antigen 199 (CA199) and carcinoembryonic antigen (CEA) were compared between the two groups. \u0000 \u0000 \u0000Results \u0000The total effective rate of the observation group was 80% (16/20), higher than 35% (7/20) of the control group (P 0.05). However, the serum tumor markers were improved in the two groups after treatment, and the improvement of the observation group was better than that of the control group (P 0.05). According to random number table method, selected patients were divided into group A, group B and group C, with 38 cases in each group. Patients in group A, B and C were given dexmedetomidine with loading doses of 0 (sodium chloride physiological solution), 0.30 μg/kg and 0.60 μg/kg at 10 min before operation, respectively. And then, application effects were analyzed and compared among three groups. \u0000 \u0000 \u0000Results \u0000The sedation score of group C was better than that of group A and group B (P<0.05). The 24 h blood pressure was detected and recorded by dynamic blood pressure monitor. Patients with peak morning blood pressure greater than or equal to 35 mmHg (1 mmHg=0.133 kPa) were included in morning peak group (n=40), and patients with peak morning blood pressure lower than 35 mmHg were included in non-morning peak group (n=50). The general data, blood pressure parameters, incidences of cardiovascular and cerebrovascular events were compared between the two groups. All patients were followed up for one year. And the correlation between changes in peak morning blood pressure and occurrence of cardiovascular and cerebrovascular events was analyzed. \u0000 \u0000 \u0000Results \u0000The peak morning value, mean morning arterial pressure (MAP) and 24 h systolic blood pressure in morning peak group were higher than those in non-morning peak group (all P 0.05). During one-year follow-up, the morbidity rates of acute coronary syndrome, arrhythmia, ischemic cerebrovascular disease and hemorrhagic cerebrovascular disease in morning peak group were 25.00% (10/40), 22.50% (9/40), 20.00% (8/40) and 20.00% (8/40), respectively, higher than the 8.00%(4/50), 6.00%(3/50), 4.00%(2/50)amd 4.00%(2/50) in non-morning peak group (P 0.05). \u0000 \u0000 \u0000Conclusions \u0000The peak morning blood pressure in patients with hypertension complicated by CHD is closely related to occurrence of cardiovascular and cerebrovascular events. {"title":"Risk factors of urinary tract infection after rigid ureteroscopy for upper urinary tract calculi","authors":"Shengyin Hu","doi":"10.3760/CMA.J.ISSN.1674-4756.2020.03.015","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1674-4756.2020.03.015","url":null,"abstract":"Objective \u0000To analyze the risk factors of urinary tract infection after rigid ureteroscopy for upper urinary tract calculi. \u0000 \u0000 \u0000Methods \u0000A total of 37 patients with urinary tract infection after rigid ureteroscopy for upper urinary tract calculi who were treated in the Fifth People’s Hospital of Datong from April 2017 to July 2018 were enrolled. After rigid ureteroscopy treatment, cleaned urine samples were collected from the 37 patients by sterile tubes, and were immediately sent to the laboratory bacteria room for bacterial culture. The patient’s age, gender, body mass index, major diameter of stone, type of stone obstruction, operation time, and related basic diseases such as diabetes mellitus and hypertension were recorded in detail. The relationships between the above factors and postoperative urinary tract infection were analyzed. \u0000 \u0000 \u0000Results \u0000A total of 64 pathogenic bacteria were cultured in 37 patients with urinary tract infection after rigid ureteroscopy for upper urinary tract calculi. Most of them were Gram-negative bacteria, of which the Klebsiella pneumoniae and Escherichia coli were dominant, accounting for 18.75% (12/64) and 35.94% (23/64) respectively. Among Gram-positive bacteria, Staphylococcus aureus and Enterococcus faecalis were the predominant types, accounting for 7.81% (5/64) and 6.25%(4/64), respectively. There were only 2 strains of fungi, accounting for 3.13%. The risk factors of urinary tract infection after rigid ureteroscopy for upper urinary tract calculi were age, major diameter of stone, type of stone obstruction, diabetes mellitus and operation time (P<0.05). \u0000 \u0000 \u0000Conclusions \u0000Age, major diameter of stone, type of stone obstruction, diabetes mellitus and operative time are risk factors of urinary tract infection in patients with upper urinary tract calculi treated by rigid ureteroscopy.
