We examined how cross-border telehealth could support health system transformation and consulted experts to identify priority policy considerations for integrating services across jurisdictions. We conducted a multimethod policy analysis comprising a narrative review informed by targeted searches and policy mapping, pre-Delphi working-group discussions, and a three-round modified Delphi consensus process with 18 experts in health policy, health-care regulation, telehealth implementation and international trade. We then made our draft findings available for World Health Organization-hosted public consultation, incorporating feedback and updating our narrative review. Our finalized challenge-policy matrix included 20 key challenges that we categorized within seven domains: knowledge and awareness; policy and governance; technology and operational issues; data security and privacy; quality and ethical standards; financial and economic factors; and adaptability and resilience. In our detailed challenge-policy matrix we identify national and international required policy actions for all 20 key challenges, which we summarize as: reinforce regulatory frameworks and international commitments; strengthen governance and coordination; build digital health capacity; ensure data privacy and security; and promote sustainable financing models. From a public health perspective, cross-border telehealth is essential for the strengthening of global health systems. Tackling the challenges of regulatory heterogeneity, exclusion from international trade agreements, digital literacy, language barriers and financial constraints requires that regulatory, technological and operational constraints be addressed through international policy cooperation. {"title":"Interventions to reduce antimicrobial use in livestock: a systematic review and evidence map.","authors":"Fiona Emdin, Kayla Strong, Jaskeerat Singh, Daniela Corno, Susan Rogers Van Katwyk, Heather Ganshorn, Arne Ruckert, Jeremy M Grimshaw, Mathieu Jp Poirier","doi":"10.2471/BLT.25.294408","DOIUrl":"10.2471/BLT.25.294408","url":null,"abstract":"<p><strong>Objective: </strong>To identify and describe government policy targeting veterinary antimicrobial use and antimicrobial resistance in production animals globally and to map these interventions to identify evidence gaps.</p><p><strong>Methods: </strong>We searched MEDLINE, CAB Abstracts, Web of Science and ProQuest Dissertations and Theses Global databases from inception to April 2025 for studies that quantitatively assessed the impact of government-enacted policy interventions by measuring the change in antimicrobial use (for example, mg/kg biomass, mg of active ingredient or defined daily doses for animals) or antimicrobial resistance in production animals or humans. Two reviewers independently completed screening, data extraction and risk-of-bias assessment for identified studies.</p><p><strong>Findings: </strong>We identified 40 policy evaluations, all originating from the World Health Organization (WHO) Region of the Americas, European Region or Western Pacific Region. No eligible studies from other WHO regions were identified. Most studies (35/40) evaluated legislative or regulatory interventions and 26 were primarily bans or restrictions on specific antimicrobial uses. Most (34/40) articles reported decreases in antimicrobial use in animals or in antimicrobial resistance in animals or humans that were associated with policy implementation. However, the studies were methodologically heterogeneous and had a high risk of bias.</p><p><strong>Conclusion: </strong>This systematic review and evidence map fill an important gap in knowledge supporting evidence-informed action on antimicrobial resistance. Achieving this mandate would require expanding availability and use of evidence-based national clinical practice and public health guidelines.</p><p><strong>Approach: </strong>In 2022, Egypt's Ministry of Health and Population requested support from the World Health Organization (WHO) to establish a national guideline development and adaptation programme. To inform the process, WHO conducted a situation analysis. The findings led to a collaboration between WHO and national stakeholders including the newly established Egyptian Health Council. In partnership they delivered capacity-building, developed a roadmap and implemented the programme.</p><p><strong>Local setting: </strong>Egypt is a lower-middle income country of 117 million people. Health services are provided by agencies in public (governmental and semi-governmental) and private sectors (providers and professional syndicates). No national guideline programme existed before 2022.</p><p><strong>Relevant changes: </strong>In total, 225 individuals attended the capacity-building workshops. After the council ratified the roadmap, the ministry, WHO and the council formally launched the national guideline programme in July 2024. Implementation of the programme led to clearer delineation of responsibilities, eliminated role overlaps and harmonized processes across different professional societies, teaching hospitals, nurses and allied health professionals.</p><p><strong>Lessons learnt: </strong>Consistent, informed political support and stakeholder commitment contributed to successful implementation. WHO's approach to contextualizing global guidelines added value. Challenges included reliance on international experts, constrained resources and insufficient institutional capacity.
