{"title":"Urinary Tract Infection Associated with Multidrug-Resistant Bacteria in a Second level Hospital During a Two Year Period","authors":"","doi":"10.33140/jnh/03/04/00001","DOIUrl":"https://doi.org/10.33140/jnh/03/04/00001","url":null,"abstract":"Objective: determine the frequency of urinary tract infections associated with MDR bacteria, characteristics patients with such infections and the mortality rate associated in Hospital ISSSTECALI during 2015-2016. Design: Descriptive, observational, analytic, transversal Methods: We collected data from clinical files of each patient that had been diagnosed with a urinary tract infection and had an uroculture of 10x105 CFU of bacteria with a multidrug-resistant profile. It included, gender, age, comorbidities, predisposing risk factors, obtained isolation, sensibility pattern, days of stay and proper technique of culture. We calculated frequencies and rates. Results: During January 2015- November 2016 a total of 2401 urocultures were solicited, we isolated bacteria in 123 of them (5.12%). 94 urocultures were included, 71% of the cultures were from women with a median age of 68.14 years; comorbidities: Hypertension (50%), diabetes (41.5%), chronic renal disease (14.9%), history of stroke and bed-rest. An average of 14.15 days of stay was calculated. They all had a urinary catheter. Of the total of urocultures obtained, 54 urocultures demonstrated bacteria growth with a MDR phenotype (attack rate: 0.43 cases/1000 discharges) Escherichia coli was isolated in 26 (48.14%) cultures; Pseudomona aeruginosa 7.4%; and Klebsiella pneumonia 5.5%; its mechanism of resistance was calculated according to the reported phenotype on the antimicrobiogram, demonstrating resistance to more than two family of antibiotics. A mortality rate of 21.3% was calculated of which the direct cause was related to the infective process (rate: 0.23 deaths/1000 discharges). Conclusion: the isolation of bacteria with a multidrug-resistant profile is not very common; however, they generate a high morbimortality index and a great weight in resources to our unit. {"title":"The Different Meanings of Flexible Shift work Scheduling: An Integrative Literature Review","authors":"S. Background: There is a significant need for today’s nursing workforce to have more flexible shift work schedules due to demographic and social changes that have increased the demands on their time. For nurses, a flexible shift work schedule will help them to get the balance they need to reduce work-family conflict and for healthcare organizations it would be a successful tool that would improve nurse retention and recruitment. This paper approaches the study of the term “flexible shift work schedule” for nurses by identifying the meaning of the flexible shift work schedule for nurses based on the various perspectives of nurses, policy-makers and health organizations. Methods: an integrative literature review was completed via a systematic search in the CINHAL and MEDLINE electronic databases. The literature was divided into research and non-research studies, and then it were categorized into three categories; (1) studies investigating the perspective nurses, (2) studies investigating perspective of policy-makers, and (3) studies investigating the perspective of health organizations. Results: there are different interpretations of the meaning of the flexible shift work schedule for nurses as well as different goals within the three groups; nurses, policy-makers and health organizations. Furthermore, each group has a different interpretation of the meaning of the flexible shift work schedule based on level of experience and authority; however, within each group the meaning remained almost the same and goals seemed to be consistent. This review suggests that the difficulty in achieving a desirable schedule that fits the needs of each party might be because the groups do not have a common meaning for the term “flexible schedule” as they do not have shared goals to implement it. In addition, the review found that the voice of registered nurses is missing in the nursing research that discusses flexible shift work scheduling issues for nurses and this is another reason that is delaying implementation of flexible schedules that suit all parties. The objective of this study was to compare the professional values orientation among the graduates of Diploma and Baccalaureate degree Nursing (BSN) program in hospital settings. Design and Methods: A cross sectional study was conducted from October -December 2011 in four Hospitals of Punjab. Data was collected from a total of 300 nurses, one fifty each from BSN and Diploma graduates. Selection of hospital as well as nurses was done by convenient sample design. Mean and frequencies were reported for demographic and professional attributes while two sample Independent T- test and ANOVA were used to compare the means of groups under study. Results: The study showed that diploma graduate nurses had higher level of values of professionalism than BSN graduate nurses in all four subscales including Professional self-concept (4.38 ± 0.73 and 3.80 ±0.83, P= 0. 000), social awareness (4.14±0.68 and 3.46 ± 0.84, P= 0. 000), nursing professionalism (4.58±0.54 and 4.10±0.81, P= 0. 000), and nursing services role (4.46±0.60 and 3.78±0.93, P= 0. 000). But in fifth subscale, nursing originality, BSN graduates had higher mean score than diploma graduates (2.47 ± 0.98 and 2.36 ± 1.03, P= 0. 361). They are not significantly different. Conclusion: The diploma graduates exhibit better professional values as compared to BSN graduates in these hospital settings. Further, the extent of motivation of the respondents that demonstrates in their attitudinal studies using Bigg’s (R-SPQ-2F) to prove the connection of their learning strategies and their drive to study. The correlation of the attitudinal studies of the respondents and the prepared motivation of Bigg’s (R-SPQ-2F) will help determine the students’ insight about their learning practices. This aims to determine the attitudinal studies with learning approaches of the nursing students, the results of which then will become the bases for enhancing the teaching strategies of teachers to upgrade the performance of the students. Methods: The study will take the form of descriptive-evaluative research. A survey was conducted using the standardized test (R-SPQ-2F) questionnaire as propagated by John Biggs for information gathering about respondents profile and their attitudes towards learning. This study is a combination of quantitative and qualitative research, also, this will take the form of an applied type of research, and thus, teachers consequently facilitate low-achieving nursing students perform better in the subject. The study used a statistical tool for the interpretation of results according to sub problems such as frequency distribution, percentage, ranking, weighted mean, likert scale, and f-test. Results: The results revealed that there are significant differences concerning the attitudinal studies of the selected nursing students in terms of surface approach and deep approach. The motivation of the nursing students using Bigg’s (R-SPQ-2F) manifested that their extent of attitudinal studies are well equipped through academic awareness. There are twelve (12) motivational Approaches of the nursing students that are correlated to the Bigg’s (R-SPQ-2F) and eight (8), which are not correlated under deep approach and surface approach. Thru evident findings, the Learning Strategies that are formulated are based on the Extent of Motivational Approach of the nursing students through their attitudinal Studies using Bigg’s (R-SPQ-2F). Conclusions: The results analysis signifies that the attitudes toward learning have been influence students’ scholastic performance. This result manifested that the extent of the attitudinal studies of the nursing students are well-equipped through academic awareness. The result encompasses change and development through the developmental stages of the nursing students. This implies that educators should become aware of the efforts of students made for learning in order for them to reach knowledge. Immediately all the necessary\u0000organizations swing into action to rescue and evacuate victims or casualties. It is always an organized activity\u0000involving the rescue team of the nation or state that is the military, police and other trained personnel. In most cases\u0000the rescue team goes to the site with trained medical personnel who are in most cases military/police with ambulances\u0000and sometimes with Aircrafts on standby for emergency evacuation.\u0000Conflicts can sometimes be positive or negative consequences thus before any rescue operation is carried out it must\u0000be well planned and organized by the organizations concern. The situation must be well understood and surveyed so\u0000as to devise a standardized way of dealing with any type of conflict. Disasters are also included during any conflict\u0000because no conflict or disaster occurs without loss of lives or destruction.\u0000The military in such cases are always deployed to handle such incidents in conjunction with the police and other\u0000trained personnel of the rescue team. However, during insurgency only the military or uniformed personnel are\u0000deployed to handle the situations. As such only a military nurse can be sent to attend to the hostages or casualties\u0000under a special cover with the hope of being a mediator between the authorities and the terrorists.\u0000One cannot discuss conflict without disasters, terrorism and insurgency. This practice is uncomfortable to the patients. Nurses have also described\u0000difficulty in assessing the sheath insertion site in the groin when pressure dressing is in place. A new way of dressing using\u0000transparent film dressing (TFD) has approved and rated better with regard to: comfort, less pain, decrease hematoma\u0000formation and facilitates nurses’ assessment of the puncture wound site after femoral sheath removal.\u0000Aim: To determine the efficacy of a small transparent non pressure dressing compared with the traditional controlled\u0000pressure dressing applied to the femoral artery puncture wound to maintain hemostasis with respect to 3 outcomes: patient\u0000satisfaction, bleeding or hematoma formation, and nurse-reported ease of assessment of the groin site after the procedure.\u0000Materials and Methods\u0000Design: An experimental design, randomized study.\u0000Patients: 80 post cardiac catheterization patients were randomized to have their groins dressed either with pressure dressing\u0000(N = 40) or Transparent Film Dressing (N = 40). Patients ambulated 8 hours after the procedures. Outcome variables were\u0000hematoma formation or bleeding, patient discomfort, and nurse-reported ease of observation of the groin puncture site after\u0000the procedure. Five instruments were used for data collection: Demographic and medical data sheet, Hematoma Formation\u0000and Bleeding Scale, Skin Integrity Scale, Patient Discomfort and Pain Scale and Nurses Ease of Assessment Scale.\u0000Results: Results of the study showed that 100% in TFD group vs. 55% in pressure dressing group reported feeling very\u0000comfortable (p value of 0.003). Hematoma formation was equal in the two dressing groups with no incidence of bleeding\u0000complications. Nurses rated the ease of assessing the groin significantly higher for TFD than for pressure dressings (p value\u0000of 0.000).\u0000Conclusion: Dressing of the puncture site after cardiac catheterization with TFD was more comfortable than the conventional\u0000pressure dressing without any difference in hematoma or bleeding complications.
