Problem: There is a need to improve resources and access to health care services to adequately meet the health needs of refugees. Approach: Patient-Orientated Research is required to enhance all stakeholders’ understanding the health status and healthcare needs of refugees and immigrants who have resettled in Canada. Outcome: This article reflects our experience using methods to engage refugees as partners on our research team.We learned about the sensitivities, ideas, emotions and life experiences of the refugee community and how that intersect with patient-oriented research methods. This project compared 17 second semester nursing students self-reported perception of their ability to recognize and act on a patient condition change, with Meti-Vision data coding software for actual recognition and action time in real time during the scenario. Three scenarios were created with patient condition changes (Phases) on 3 minute intervals. Students were placed in groups of 2 with 12 total coded groups, each student group received at least 2 patient condition changes. Post simulation the nursing students completed a survey self-reporting 7 areas of perceived changes recognized and/or actions performed. What this entails varies dramatically from patient to patient. Most people, however, never discuss their wishes regarding the care they desire for this very personal, final journey. As technology improves, as well as the ability to successfully treat almost any condition, both patients and clinicians alike have begun to ignore the reality of death. In this paper, the term clinicians will refer to doctors and nurses. In our present culture discussing death has become taboo. Patients may think about the end of their lives but are unsure or uncomfortable broaching the topic with their medical staff and families. Additionally, many clinicians are uneasy with end-of-life (EOL) discussions and, consequently, avoid them entirely. It is, however, the ethical responsibility of health care providers to address their patient’s wishes regarding EOL care. Clinicians need to take the initiative to begin the conversation regarding prognosis and care choices. They need to make the patients feel that they can comfortably discuss this delicate issue without fear of judgment, pressure, or the possibility of abandonment. Ethical dilemmas are common when caring for a dying patient, often a direct result of avoiding or delaying difficult, EOL conversations [1]. By not adequately preparing patients for the dying process, clinicians are in danger of ignoring the ethical principles of autonomy, justice, beneficence and non-maleficence. Nurses and physicians alike share this responsibility, although nurses have historically shied away from this obligation. The American Nurses Association (ANA), the premier nursing organization representing registered nurses throughout the U.S., leaves no doubt that it is nurses’ ethical duty to assure that patients have a dignified death - one that respects their spiritual, emotional, and physical needs, as well as those of their families. This duty is spelled out in the Nursing Code of Ethics. Nurses are called on to identify barriers that prevent early EOL discussions with patients so that they may work to eliminate them and ultimately improve the patient’s ethical rights to a good death. Due to the great time-zone differences between Nigeria and these countries, there exists the possibility of experiencing adjustment problems. Aim of the study: To determine the burden of jetlag phenomenon and the distribution of its symptoms among Nigerians studying in these countries. Methodology: Online surveys using Google forms were disseminated to Nigerians studying in Indian and Malaysian Universities and the data were analyzed. Results: A hundred and three (103) eligible persons responded to the survey, most of whom were males (86.4%) and married (58.3%). Many of them did not know what jetlag was (52.4%). Most, also reported falling asleep less easily on their first night of arrival (78.6%), of whom 44.4% continued to experience same for months and even up to a year. Many also reported having more wakeful episodes during the night (41.7%); later waking time (56.3%); feeling less alert 30 minutes after waking from sleep (58.3%) and generally feeling more tired since arrival (57.3%), with many of them haven experienced same for prolonged durations. A higher proportion of those studying in Malaysia, reported experiencing these symptoms, compared to those studying in India. As an adjustment/coping technique, most of them had attempted maintaining daytime alertness (72.8%) and maintaining a dark room at night (55.3%), of whom 21.0% and 31.6% respectively found these measures very effective, while 57.3% and 50.8% respectively found them slightly effective. Conclusions: This study reveals the great enormity of this problem. It is recommended that prospective students be enlightened on the possibility of experiencing this problem and adjustment techniques even before departure from Nigeria. It is possible that new and innovative medical therapies have provided patients with treatment options that enhance survival of a critical illness and allow them go home with more medical support than previously available in earlier years. This may explain why there are now millions of ICU survivors are a result of declining mortality rates (8-19%) even though there are increased ICU admissions [3, 4]. This also indicates a change in direction of the attitudes and beliefs of the family unit today and their desire for the family member to return home after critical illness instead of moving to a tertiary facility for care. However, survivorship is not without its negative consequences especially financially. In 2005, the cost of critical illness in the ICU was estimated at $81.7 billion [5].
