Every youth aspires to go and work abroad for a better life perspective. The increasing migration of young mind has been the challenge for every developing state. The attraction of working abroad has pulled many intelligent minds out of the state. Nursing is one of the highlighted profession in this regards. Every new nurse graduate wishes to work abroad. This nurse’s shortage has been the most concerning issue across the globe majorly in developing states. Aga Khan University has been one of the well-known and market competitive education university. They are producing highly capable medical professional over last 3 decades including doctors and nurses. However very few alumni of this institution are serving the country. Moreover this international university is deprived of their own graduates in their organization. This issue created disturbed equilibrium of nurse vs. patient ratio. This causes lack of quality of health care provision to the people of Pakistan.

Likewise, critical thinking is a skillful application of knowledge for judgment and evaluation of complex issues or problems . “The effect of simulation courseware on critical thinking in undergraduate nursing students: Multi-site pre-post study” by has chosen to create a platform for discussion. The purpose of this study was to evaluate the effect of an integrated pediatric nursing simulation used in a practicum on students’ critical thinking and to identify the effects of number of simulation exposures on critical thinking in a multisite environment in three different nursing schools. Health care providers (HCP’s) are obligated to respect patients’ autonomy in almost every situation. Even if patients’ take decisions that poses could threat their life. It is especially challengeable in case of children where the key decision-makers are parents. This paper identifies such conditions in which there is a conflict between the parents’ autonomy to make decisions on behalf of their child and beneficence to improve children's health. A question arises with this situation on that, is it right to respect parent's autonomy or not, but HCP’s are committed to do in the best interest of their patients.

Regarding end-of-life decisions, publications demonstrate that doctors don’t intend for themselves what they practice with patients. We aimed to assess health professionals’ perspectives about their own end-of-life decisions, asking them “In case of advanced oncological disease, would you prefer rescue or comfort therapy?” and “In case of advanced chronic disease, would you prefer admission in an Intensive Care Unit (ICU) or palliative care?”. The sample included 57% doctors. 80% of all participants chose comfort therapy and 84% chose palliative care. Nurses chose comfort therapy and palliative care more frequently than doctors (p<0.05); both doctors and nurses from surgical areas preferred rescue therapy and ICU admission (p<0.05); more than half of pediatricians answered rescue therapy and ICU admission, this trend was also observed in oncologist/palliative care doctors and surgeons, with a statistical difference (p<0.05) compared to other doctors; on the opposite, 90% from Emergency and Intensive Medicine doctors answered comfort therapy and palliative care (p<0.05). Communication with patients and families must be more effective, making them understand that the appropriate clinical decision is the most ethically correct. Death is not something to be avoided at all costs, but rather a moment on life cycle. These issues should be discussed in advance, anticipating the possible need for admission to ICU, rescue therapy or setting limits and gently stops.

However, in emergency situations, clinical judgment often privileges the principle of beneficence, in pursue to save lives. The authors describe three real, imminent and life-threatening clinical situations. Then, it was asked doctors, patients, family members and bioethicists about their judgment of the clinical actions taken. Doctors, patients and their families agreed on almost all answers. Bioethicists, on the other hand, had more opposing evaluations. Is bioethics swerving from patients’ will and from clinical practice reality? Paul Ricouer's thought deserves the authors reflexion on its phronesis dimension. Acting with prudence and providing an humanized clinical practice lead us to find the recta ratio agibilium (the line of reason of those who act).

The present study aimed to assess whether admission of these patients in intensive care units (ICU) should be considered or excluded. Clinicians (internists, intensivists and neurologists) were asked to answer this question, and also to rank the criteria they considered most relevant in refusing admission to an ICU. For most doctors, dementia per se does not preclude admission in ICU. Functional reserve and degree of disability were the most important factors, followed by cognitive impairment and comorbidities. Family expectations and age were the least important variables when considering admission.

Adequate information is essential for good adherence of the prescribed drugs and for participating in decisions about treatment. The main objective of this study to assess the knowledge on prescribed Artemether-lumefantrine (AL) among outpatients and caretakers attending Temeke hospital. \u0000 \u0000Methods: This was a cross sectional study involving 224 outpatients and caregivers who were attending at Temeke Hospital and received (AL) drugs. \u0000 \u0000Result:Only 105 (46.9%) understood the Interval of hours required to take AL pills; their knowledge did not vary with sex. However those with university/college education (74.1%) were more likely to be knowledgeable as compared to those with primary or lower education (45%). Fifty eight percent of participants did not understand the number of days required to complete AL dosage, while only 114 (50.9%) understood the instructions written on the AL package. When asked about the consequences associated with incomplete AL dosage, 176 (78.6%) did not understand the risks of not completing AL dosage. More than half (58.5%) of participants were not satisfied with the information given by health workers about risks and benefits of the prescribed AL. The Majority (84.8%) of participants did not discuss with care provider about managing the prescribed AL. In this regard 198 (89.2%) of participants did not get an opportunity to ask any question about their prescribed drug. \u0000 \u0000Conclusion: Finally the prescribers must increase the time to spend with patients so as to enable them to understand the instruction provided because is the patients fundamental as articulated in the Universal Declaration of bioethics and Human Right (UDBHR).
