Marcel *","doi":"10.1016/S0035-2977(73)80038-0","DOIUrl":"10.1016/S0035-2977(73)80038-0","url":null,"abstract":"<div><p>L'endotoxine agit sur l'hémostase à de nombreux niveaux : elle entraîne des perturbations endothéliales, un « releaseet une agrégagtion plaquettaire, une synthèse accrue de fibrinogène et de facteur VIII, enfin un blocage de l'épuration hépatique des P.D.F. (produits de dégradation du fibrinogène) et de certains facteurs activés. De ceci résulte un tableau de C.I.V. (coagulation intravasculaire) dominé par la chute des plaquettes et du facteur V, l'apparition de P.D.F. et de monomères de fibrine, l'allongement du temps de thrombine et de reptilase et la positivité du test à la protamine.</p><p>L'activation des systèmes de la coagulation et du facteur XII en particulier s'associe à une activation du système kallikréine-kinines, qui joue sans doute un rôle majeur dans la génèse du choc.</p><p>L'héparinothérapie n'est guère indiquée que s'il est indispensable de corriger rapidement le déficit en fibrinogène et/ou en plaquettes par un apport de fractions exogènes. Elle est peut-être utile très précocément, avant la constitution d'une insuffisance rénale aiguë. Elle peut être dangereuse en cas de plaie opératoire. La situation peut également être aggravée par les antifibrinolytiques.</p></div><div><p>Endotoxin acts on hemostasis at many levels : under its influence endothelial cells enter the circulation, platelets release and aggregate, synthesis of factor VIII and of fibrinogen is increased, hepatic clearance of F.D.P. (fibrinogen degradation products) and of certain activated factors is decreased. This leads to D.I.C. (disseminated intravascular coagulation) with drop of platelets and of factor V, presence of F.D.P. and fibrin monomer, lenghtening of thrombin and reptilase times, and positive protamin test.</p><p>Activation of the coagulation system and particularily of factor XII induces an activation of the kallikrein-kinin system, which most likely plays an essential part in the genesis of shock.</p><p>Heparin must be used if it is necessary to correct fibrinogen and/or platelet levels by the use of exogenous fractions. It may also be useful at a very early stage, to prevent renal capillary thrombosis. However heparin can be dangerous in surgical patients. Lemaire","doi":"10.1016/S0035-2977(73)80031-8","DOIUrl":"10.1016/S0035-2977(73)80031-8","url":null,"abstract":"<div><p>Les auteurs ont recherché l'antigène Milan chez des malades atteints ou non d'hépatite virale, de diverses hépatopathies et enfin de maladies générales.</p><p>Ils ont travaillé pendant la période juillet-août 1971 avec un sérum contenant l'anticorps de Milan puis avec des sérums provenant de sujets ayant réagi avec l'anticorps originel.</p><p>Ne voulant considérer que la première période, ils concluent à la non spécificité de cet antigène qui n'a pas, pour l'hépatite virale A, la valeur qu'a l'antigène Australie pour l'hépatite B. Ils considèrent que d'après le contexte clinique il est difficile de dire à quoi correspond ce système précipitant non constitutionnel.</p></div><div><p>The authors studied the Milan antigen in patients with or without viral hepatitis, with various hepatopathies and lastly with general illnesses.</p><p>From July to August 1971, they worked with Milan antibody serum, then with sera from patients having reacted with the original antibody. Taking into consideration the first period only, they concluded that :</p><ul><li><span>--</span><span><p>this antigen called Milan is not specific for viral hepatitis A as it is present without it. Thus is doesn't have the same value as Australia antigen (with which it is sometimes associated);</p></span></li><li><span>--</span><span><p>it seems to be a non constitutional precipitating system. It is possible that it's an abnormal lipoprotein which appears during cholestasis. However, no correlation was found with the bilirubin, cholesterol and lipid levels.
