Furtheron an in vitro contracture test and an ATP-test was performed and the Serum CPK and the isoenzyme in serum and muscle examined. The following conclusions were taken: 1) Although unspecific pattern (in 5 cases more than 12% polyphasic potentials were found, one of them showing a reduction of the mean duration of 30%) the electromyography is a good but not absolutely certain indicator of subclinical myopathic signs in Malignant Hyperthermia. 2) The exclusion of M.H. risk is not possible using a single test. 3) The diagnostic value of not invasive methods, as the electromyography is of special importance. Nicht eindeutig geklärt wurde bisher vor allem der Wert der routinemäßigen Blutspiegel-Bestimmungen. Die bisherigen Untersuchungen, denen zufolge die erreichte Therapieverbesserung auf eine Bestimmung der Blutspiegel zurückzuflihren war, wurden ohne Kontrollgruppe durchgeflihrt. Es liegen außerdem Untersuchungen vor, bei denen die Bestimmung der Blutspiegel keine Therapieverbesserung erbrachte. Überlegungen zur gezielten Indikationsstellung ergeben sich aus dem Zwang, die hohen Kosten mit in die Diskussion einzubeziehen. Hauptindikationen zur Blutspiegel-Bestimmung sind Therapieresistenz aus unklarer Ursache einschließlich des Verdachts auf unregelmäßige Einnahme sowie die Abklärung von Intoxikationen. Im einzelnen ergeben sich zur Zeit folgende Indikationen zur Blutspiegel-Bestimmung der Antiepileptika: I. Verbesserung der Dosierungsrichtlinien, 2. Verdacht auf unregelmäßige Einnahme, 3. Therapieresistenz, 4. Einstellung des Blutspiegels nach einem therapeutischen Bereich zum Schutz vor Unteroder Oberdosierung, 5. Dosisreduktion bei Anfallsfreiheit , 6. Dosierungskontrolle bei zusätzlichen Erkrankungen, 7. Dosierungskontrolle in der Schwangerschaft, 8. Intoxikationen, vor allem bei Kombinationstherapie, 9. Abklärung von unspezifischen Beschwerden, 10. Erfassung von Interaktionen, I I. The following beta-globulins: transferrin, haemopexin, beta-1 A-globulin, beta-1 E-globulin, beta-2-glycoproteid and beta-lipoproteid were determined immunologically quantitatively in the CSF and partly quantitatively in the serum, and their behaviour was compared with that of the beta and tau fraction in CSF electrophoresis. It was found that changes in the fractions in CSF electrophoresis agreed only slightly with those of the quantitatively determined globulins, although these globulins represent 70--80% of the beta and tau fraction. Increases in the number of beta-1 A and beta-1 E-globulins are more significantly marked in intracranial haemorrhages than with other diseases. In all other respects, increases or decreases in the number of beta-globulins do not appear to be typical of any particular disease pattern determined by electrophoresis or quantitatively. {"title":"[The Kleine-Levin-Critchley-syndrome. In contrast to the first descriptions we characterise this syndrome by the Trias: periodic hypersomnia, vegetative disturbances (especially of food intake) and psychopathologic symptoms. Especially young men in their second decade of life suffer from this syndrome. As far as women are concerned the disease is found more seldom but the authors are sure that it exists. A spontaneous remission of the periodically proceeding disease often occurs in the third decade of life. In analogy to the respective literature a retardation of the EEG during the course of the disease going along with otherwise inconspicuous neurology was observed by the authors. Polygraphic-EEG studies are seldom. The cause of the syndrome is still open to question. Our observation show that a primarily disposition-linked and a secondarily acquisition-linked form can be assumed. Good effects are put down to amphetamines used for therapy. The various per formance clusters differed not only strongly in the height and shape of their average test profiles, but additionally also through configurations of charac teristics which differed in their favourable or unfav ourable nature, approximately in the sense of the correlational findings. The discussion deals with the deficiency of susceptibility to interference, which is typical of schizophrenia, and also with slowing-down, intelligence, motivation and rehabilitative consequen ces. The differential diagnosis of the last group is described. Die rasch zunehmenden Erfahrungen in den letzten Jahren haben gezeigt, daß die Verletzungen jedoch bunt und manigfaltig gestaltet sind. Dementsprechend müssen auch die operativen Maßnahmen den Verletzungen angepaßt sein. Von operativen Konsequenzen ausgehend, ist eine Einteilung der Verletzungen im oberen oberhalb C 3 und im unteren bei und unterhalb von C 3 Bereich vorzunehmen. Bei Verletzungen im oberen HWSBereich ist eine der Verletzungsart angepaßte Dorsalfusion angezeigt. Hingegen muß bei Verletzungen von und unterhalb C 3 stets ein ventraler Zugang gewählt werden. Bei Achsenknickungen und bei reinen Luxationen ohne eine Fraktur der beteiligten Wirbelkörper oder der Wirbelgelenke genügt in der Regel eine einfache Ventralfusion, vorausgesetzt, daß die Patienten gehfahig un d kooperativ sind. Bei paraplegisch gelähmten, gehunfahigen oder unruhigen Patienten, aber auf jeden Fall bei Luxationsfrakturen, bei Kompressionsund schweren Stückbrüchen muß stets eine zusätzliche innere Fixierung angebracht werden. It is well known that tricyclic neuroleptics and anti-depressants lower the rate of salivary excretion and lead to hyposalivation and a dry mouth. The following methods were studied on their effect to compensate hyposalivation: Distigminbromid, sweets, Glandosane, a new synthetic saliva, and water which was gustatory adopted. Salivary flow was measured according to Matzker (7). Subjective feelings of thirst and mouth dryness were recorded by selfconstructed tests. Only Glandosane and Distigminbromid compensated mouth dryness. The recorded amount of salivary flow was within the range of normal controls. There were no significant correlations between the subjective test answers and the objective measurement of salivary rate. Improvement of depression by application of Glandosane could not be observed.
