{"title":"Actualités sur le bilan d’urgence en toxicologie","authors":"Laurence Labat","doi":"10.1016/S1773-035X(25)76469-2","DOIUrl":"10.1016/S1773-035X(25)76469-2","url":null,"abstract":"<div><h3>Résumé</h3><div>Le bilan toxicologique d’urgence réalisé en milieu hospitalier est essentiel à la prise en charge des patients intoxiqués arrivant dans les services d’accueil. Seuls les biologistes ayant quelques compétences fondamentales qui seront rappelées telles que la toxicocinétique, les principes des phases d’intoxication, des toxiques à intervalles libres, des notions d’anamnèse ainsi qu’une connaissance des principaux antidotes et des techniques d’épuration, pourront assurer la validation de ces bilans d’urgence pour accompagner les cliniciens dans la prise en charge de leurs patients. La qualité du prélèvement est essentielle en biologie et en particulier en toxicologie avec, comme matrices les plus couramment utilisées, le sang et les urines. Le biologiste devra également être équipé d’appareils et de méthodes adaptés à l’urgence avec en général un automate de chimie permettant d’utiliser des tests d’identification ou de dosages rapides par méthodes immunochimiques ou enzymatiques. On citera principalement le dépistage des drogues dans les urines, le dosage de l’éthanol, du paracétamol ou du GHB, les recherches de classes comme les benzodiazépines dans le plasma ou le sérum. Ces méthodes restent toutefois limitées en termes de sensibilité et spécificité et leurs résultats rendus rapidement nécessitent une confirmation et des dosages par méthodes chromatographiques couplées à de la spectrométrie de masse. Enfin, on rappellera l’intérêt des screenings plus larges ciblés et non ciblés, notamment pour l’identification des nouvelles drogues de synthèse.</div></div><div><h3>Abstract</h3><div>The emergency toxicology tests carried out in hospital is essential for the management of intoxicated patients arriving in emergency units. Only biologists who are familiar with a few basic concepts, such as toxicokinetics, intoxication phases, free interval toxicants, anamnesis, the main antidotes and purification techniques, will be able to validate these emergency assessments and assist clinicians in the management of their patients. The quality of sampling is essential in biology, particularly in toxicology, with blood and urine being the most commonly used matrices. The biologist must also be equipped with equipment and methods suitable for emergency use, generally including an automated chemistry system that can be used for rapid identification tests or assays using immunochemical or enzymatic methods. These include urine drug screening, ethanol, paracetamol or GHB, and tests for classes such as benzodiazepines in plasma or serum. These methods are still limited in terms of sensitivity and specificity, and their rapid results require confirmation and assays using chromatographic methods coupled with mass spectrometry. Cet article synthétise les travaux récents menés dans le contexte de l’arrêt de la commercialisation de plusieurs techniques et présente les recommandations du CNR de la toxoplasmose pour la confirmation de la présence d’IgM spécifiques ainsi que pour le diagnostic sérologique de la toxoplasmose congénitale chez l’enfant. Chez l’adulte, il est recommandé d’associer une technique très sensible (Platelia Toxo IgM Bio-rad ou Architect/Alinity Toxo IgM Abbott) et une technique très spécifique (VIDAS Toxo IgM Biomérieux ou Liaison Toxo IgM Diasorin) pour la détection des IgM. Par ailleurs, l’immunoblot Toxo II IgM (LDBIO Diagnostics) apparaît désormais comme la technique de choix pour la confirmation de la spécificité de ces anticorps. Pour le diagnostic sérologique de toxoplasmose congénitale chez l’enfant, les profils immunologiques comparés mère/enfant, la détection d’IgM spécifiques et le suivi de la cinétique des taux d’IgG sont des approches complémentaires. La technique Platelia Toxo IgM est une bonne alternative à la technique Toxo-ISAGA IgM dans ce contexte.</div></div><div><h3>Abstract</h3><div>Serological tests take an important place in the biological diagnosis of toxoplasmosis in pregnant women and children suspected of congenital toxoplasmosis. This article summarises recent work carried out in the context of the discontinuation of several techniques and presents the recommendations of the French Toxoplasmosis Reference Center for confirming the presence of specific IgM and for the serological diagnosis of congenital toxoplasmosis in children. In adults, the combination of a highly sensitive technique (Platelia Toxo IgM Bio-rad or Architect/Alinity Toxo IgM Abbott) and a highly specific technique (VIDAS Toxo IgM Biomérieux or Liaison Toxo IgM Diasorin) is recommanded for the detection of IgM. The Toxo II IgM immunoblot (LDBIO Diagnostics) now appears to be the best technique for confirming the specificity of these antibodies. For serological diagnosis of congenital toxoplasmosis in children, comparative immunological profiles of mother and child, detection of specific IgM antibodies and monitoring of IgG levels are complementary approaches.
