{"title":"[Treatment of oral lichen planus-a review].","authors":"Khava Abdusalamova, Farzan Solimani, Margitta Worm","doi":"10.1007/s00105-025-05540-x","DOIUrl":"10.1007/s00105-025-05540-x","url":null,"abstract":"<p><p>Mucosal lichen planus (MLP) is a chronic, recurrent, inflammatory T‑cell disorder that can affect mucous membranes and, together with cutaneous lichen planus (CLP) and lichen planopilaris (LPP), belongs to the variants of lichen planus (LP) diseases. MLP can affect both the oral mucosa (oral lichen planus, OLP) and the genital mucosa. Patients with OLP can have varying degrees of severity. In particular, patients affected by an erosive/ulcerative form usually pose a therapeutic challenge because 1) there are currently no US Food and Drug Administration(FDA)/European Medicines Agency (EMA)-approved drugs and 2) the disease is often treatment resistant. Recently published studies on immunopathogenesis describe a predominant role of interferon-γ-induced inflammation, but many aspects of the disease are still unknown. Accordingly, primarily symptomatic therapies are currently available. In addition to topical glucocorticoids, retinoids in topical and systemic form and systemic glucocorticoids are established first-line therapies. A wide range of second- and third-line therapies show that no standard treatment that there is still o standard treatment. Among the new therapies, Janus kinase inhibitors and monoclonal antibodies should be emphasized, as they could expand the therapeutic spectrum for OLP in the future. However, prospective, placebo-controlled studies are needed for this in the future. Alternative and complementary treatments such as herbal therapies, phototherapy, platelet-rich plasma (PRP) and injectable platelet-rich fibrin (i-PRF) are also described in the literature. Using dynamic optical coherence tomography (D-OCT) and line-field confocal optical coherence tomography (LC-OCT), a significant reduction in vessel density and diameter was noninvasively observed. Due to the known range of reactions that can be mediated by the messenger substance histamine, it is postulated that food histamine can trigger the same reactions. Results of studies with double-blind, placebo-controlled oral provocation tests do not confirm this, but rather show that orally administered histamine does not trigger reproducible reactions and that symptoms often occur after placebo, indicating a strong nocebo effect. Without reproducibility, however, the definition for an adverse reaction to food is not fulfilled. As many sufferers are severely affected by their self-diagnosis due to a massive restriction of their food choice, quality of life, and social interaction, allergy societies in German-speaking countries have published a guideline which describes a pragmatic diagnostic and therapeutic approach. The primary aim is to alleviate symptoms by improving digestive function and to expand the choice of foods, rather than to exclude the suspected diagnosis. {"title":"[Vegan beware! The relinquishing of animal products requires adequate protein, vitamin and mineral substitutes from plant-based foods. Legumes play an important role in this context. At the same time there are an increasing number of case reports of allergic reactions after consumption of legumes.</p><p><strong>Aim: </strong>The aim of this study was to summarize the current state of knowledge regarding the allergenic potential and the relevant allergens of legumes in the context of a vegan diet.</p><p><strong>Material/methods: </strong>A comprehensive literature search from the last 25 years on vegan diets and the allergens in legumes was conducted.</p><p><strong>Results: </strong>Peanuts and soybeans are among the well-known legumes with a high allergenic potential; however, there are also other legumes, such as lupins, lentils and peas, which can trigger severe allergic reactions. Exercise-induced anaphylaxis has also been described following the consumption of legumes.</p><p><strong>Conclusion: </strong>The increased consumption of legumes in the context of a vegan diet is bringing a wider range of plant-based foods into focus, which may have potentially allergenic properties. An increase in allergic reactions to legumes is to be expected. Histological examination showed pale amorphous nodular deposits in the dermis. On radiological examination, calcifications could be ruled out. Laboratory tests showed marked hyperuricemia and chronic renal insufficiency. We diagnosed miliary gout (MG) and initiated uric acid-reducing therapy with allopurinol 100 mg/day. First described in 2007, MG presenting with multiple whitish-yellowish papules is a rare intradermal variant of classic gout that is characterized by subcutaneous deposits.
