Berkoz","doi":"10.4183/AEB.2019.165","DOIUrl":"https://doi.org/10.4183/AEB.2019.165","url":null,"abstract":"Context\u0000The inhibition of adipocyte differentiation has a significant role on the prevention of obesity and obesity-associated complications.\u0000\u0000\u0000Objective\u0000In this study, we aimed to detect whether hyperoside is able to inhibit the conversion of pre-adiposits into mature adiposits.\u0000\u0000\u0000Design and Methods\u00003T3-L1 pre-adipocytes were stimulated so as to differentiate into mature adipocytes. Hyperoside in non-cytotoxic concentrations (1, 2, 5, and 10 µM) were separately applied to differentiated 3T3-L1 cells. Oil red O staining was performed and triacylglycerol contents were measured. Furthermore, gene and protein expressions of transcription factors, adipogenic genes and adipokines were examined in order to investigate the effect of hyperoside on adipocyte differentiation.\u0000\u0000\u0000Results\u0000Hyperoside in high concentrations significantly suppressed the adipogenic process by inhibiting the expression of transcription factors and adipogenic genes and reducing lipid accumulation in adipocytes (p<0.05). Low doses of hyperoside are able to inhibit adipogenesis, but higher doses are needed to reduce fat accumulation in mature adipocytes. The disease is characterized by the loss of adipose tissue from the face, upper limbs, and the upper trunk. Fat loss usually starts in childhood and early adulthood, and the disease is more common in females. Some patients may exhibit an accumulation of fat in the lower part of the body. Metabolic abnormalities secondary to insulin resistance may develop in patients with acquired partial lipodystrophy; however, they are relatively less frequent compared to other subtypes of lipodystrophy. Then, capacitated sperms and oocytes were placed together in fertilization medium in four groups in the presence of different concentrations of HSP60 (10, 50 and 100 ng/mL) and in the absence of HSP60. After calculation of the fertilization rate, zygotes were transformed into the other medium for development and the cleavage rate was monitored to blastocyst stage.\u0000\u0000\u0000Results\u0000There was not a significant difference in the rate of fertilization between 10 ng/mL HSP60 group and the control group. The rate of fertilization and two-cell embryo development decreased significantly (P≤0.05) in 100 ng/mL HSP60 compared to other experimental and control groups. Further, the rate of two-cell embryo development increased significantly (P≤0.05) in 10 ng/mL HSP60 compared to other experimental and control groups.\u0000\u0000\u0000Conclusions\u0000The present study demonstrated that HSP60 in low dose had a positive effect on two-cell embryo development, however it did not have any significant effect on the fertilization rate. Mureșan, Ionela Pascanu","doi":"10.4183/AEB.2019.225","DOIUrl":"https://doi.org/10.4183/AEB.2019.225","url":null,"abstract":"Context\u0000Permanent hypocalcemia is a rare but significant complication of thyroid surgery.\u0000\u0000\u0000Objective\u0000The aim of this study was to identify predictive factors of hypocalcemia and hypoparathyroidism after thyroidectomy.\u0000\u0000\u0000Design\u0000Study included 134 total patients submitted to thyroidectomy from two endocrine units (January 2015 - August 2016).\u0000\u0000\u0000Methods\u0000We measured total serum calcium (sCa) and intact PTH (iPTH) on postoperative day one and 1 month after surgery.\u0000\u0000\u0000Results\u0000118 patients were women with F/M ratio of 7.3/1 and a mean age of 51.8 years. 64 patients were included in group A (iPTH <12 pg/mL) and 70 patients in group B (iPTH >12 pg/mL). sCa and hypocalcemia symptoms were correlated with iPTH, measured 24 hours after surgery. The cut-off value was for sCa 8.05 mg/dL with a sensitivity of 85.29% and a specificity of 88.0% and for iPTH 11.2 pg/mL, with a sensitivity of 82.3% and a specificity of 71.0%. Sellar xanthogranulomas (SXG) result from secondary hemorrhage, infarction, inflammation or necrosis upon existing craniopharyngioma (CP), Rathkès cleft cyst (RCC) or pituitary adenoma (PA), or represent a stage in xanthomatous hypophysitis evolution. \"Pure SXG\" are independent of a preexisting lesion. A 70 year old male patient, laryngeal cancer survivor, presented with central diabetes insipidus (CDI). MRI revealed an intra-suprasellar mass of uncertain origin. Transsphenoidal surgery resulted in an efficient lesion resection with maximal pituitary sparing. Pathological report has confirmed SXG without conclusive identification of preexisting sellar lesion. Age at presentation and gender were atypical for SXG. The most frequent presenting signs of SXG were absent. Most SXG are initially misdiagnosed as CP, RCC or PA. Preoperative clinical and radiological uncertainty may impact operative planning. Differentiating from CP is crucial, due to divergent operative target goals and prognosis. Intraoperative frozen section analysis could guide surgical extensiveness. Close collaboration must include endocrinologist, neuroradiologist, neurosurgeon and pathologist. Quantity and quality of provided tissue are essential for avoiding bias in pathohistological analysis of cystic or heterogenous lesions. Awareness is needed of new pathological entities in the sellar-parasellar region. Current literature regarding the risk of thyroid cancer in incidentalomas found in patients with non-thyroid cancer is limited.\u0000\u0000\u0000Objective\u0000The aim of the present study was to investigate the frequency of thyroid malignancy in thyroid incidentalomas detected in patients with non-thyroid cancer.\u0000\u0000\u0000Design\u0000Case control study.\u0000\u0000\u0000Subjects and Methods\u0000The database of 287 thyroid nodules from 161 patients with a history of non-thyroid cancer followed between 2008 and 2014 were retrospectively evaluated.\u0000\u0000\u0000Results\u0000From 287 thyroid nodules, 69.7 % had a benign final cytology. Thyroid cancer detected in one nodule while follicular neoplasia detected in 4 nodules, atypia of unknown significance (AUS) detected in 10 nodules, Hurthle cell neoplasia detected in 5 nodules and suspicious for malignancy detected in 6 nodules according to fine needle aspiration biopsy results. Metastasis of the non-thyroid cancer to the thyroid gland was detected in 4 nodules. Twenty seven nodules from 15 patients were removed with surgery. There were 3 malignant nodules found after surgery (1 papillary, 1 follicular and 1 medullary cancer). In addition to these three thyroid cancers, two patients with benign nodules had co-incidental thyroid cancer detected after surgery. During the years preceding the diagnosis and hormonal replacement therapy (HRT) they had often isolated themselves as they felt different from their peers. Ovarian failure and infertility, not the body height, were the major problems for most of the women. Infertility had affected the women very deeply and many felt depressed because of this. Adolescent behaviour, a feeling of chronic inferiority or a feeling of grief were different ways of coping with the situation. Median age at sexual debut was 19.5 years. Painful intercourse related to vaginal constriction and sore membranes was commonly reported. Most of the women had stopped HRT because of side-effects. Many of the problems experienced by the women could have been avoided if proper HRT had been administered in due time and on a long-term basis.
