Leng","doi":"10.3760/CMA.J.ISSN.1007-631X.2020.02.001","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1007-631X.2020.02.001","url":null,"abstract":"Objective \u0000To investigate the effect of Roux-en-Y hilar-jejunum anastomosis in surgical resection of hilar cholangiocarcinoma. \u0000 \u0000 \u0000Methods \u0000The clinical data of 31 patients with hilar cholangiocarcinoma admitted from Feb 2009 to Feb 2017 who underwent Roux-en-Y hilar-jejunum anastomosis after resection were retrospectively analyzed. \u0000 \u0000 \u0000Results \u0000According to Bismuth-Corlette classification there were 6 cases of IIIa, 6 cases of IIIb and 19 cases of type IV. These patients all successfully underwent Roux-en-Y hilar-jejunum anastomosis, with intraoperative blood loss of 50-4000 ml (on average of 1146±1082 ml). The average operation time was (346±118) min, and the average hilar-jejunum anastomosis time was (35±13) min. The average postoperative hospital stay was (20±11) days. There were 25 cases of R0 resection(80.6%). The postoperative complication rate was 25.8% (8/31). The overall median survival time was 21 months, and the 1-, 3-, and 5-year survival rates were 68.8%, 14.6%, and 3.6%, respectively. The receiver operating characteristic (ROC) curve was drawn to determine the optimal cut-off value of FAR, and the prognostic factors were evaluated by Kaplan-Meier method, univariate and multivariate analysis. \u0000 \u0000 \u0000Results \u0000The optimal cut-off value of preoperative FAR for postoperative overall survival was 0.08. FAR was significantly associated with preoperative total bilirubin(TBil)levels, tumor differentiation, T stage, TNM stage, resection margin status, and preoperative CA199 levels (all P<0.05). There were 18 cases having AFP≥20ng/ml, 9 cases were with carbohydrate antigen 199 (CA199)≥37U/ml, and 5 cases with carcinoembryonic antigen (CEA)≥5ng/ml. The 1 -, 3 -, and 5-year postoperative survival rates were 68.8%, 34.4%, and 17.4%. Multivariate analysis showed that serum CA199≥37U/ ml(χ2=5.687, P=0.019) was an independent risk factor for patients′ survival. \u0000 \u0000 \u0000Conclusion \u0000Most cHCC-CC is found in association with chronic viral hepatitis. Among the 30 liver cancer tissues, FNDC5 was weakly positive in 5, positive in 19, and strongly positive in 6 cases. There were 8 cases of intrahepatic cholangiocytic lymphoepithelioma-like carcinoma, 4 cases of hepatocytic lymphoepithelioma-like carcinoma, and 1 case of mixed hepatocytic and cholangiocytic lymphoepithelioma-like carcinoma . All patients received partial hepatectomy and postoperative comprehensive treatment. The patients were followed from 6 months to 7 years. Only one patient died, and the other patients were all in a tumor-free state. \u0000 \u0000 \u0000Conclusion \u0000Primary hepatic lymphoepithelioma-like carcinoma is a rare liver cancer. It is confirmed mainly by pathological examination and immunohistochemistry. Leng, Jiye Zhu","doi":"10.3760/CMA.J.ISSN.1007-631X.2020.02.003","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1007-631X.2020.02.003","url":null,"abstract":"Objective \u0000To summarize the experience of diagnosis and treatment of rare type of hepatic benign space occupying lesions. \u0000 \u0000 \u0000Methods \u0000The clinical data of 113 patients with rare type of hepatic benign space occupying lesions confirmed by surgery and pathology from Jan 2009 to Dec 2018 were retrospectively analyzed. \u0000 \u0000 \u0000Results \u0000There were 51 males and 62 females , age ranging from 12 to 83 years, with an average of 44.3 years. 91.2% of the 113 cases were single lesions and 8.8% were multiple lesions. Surgical methods included hepatectomy in 98 cases, ablation therapy in 12 cases and hepatectomy combined with ablation in 3 cases. There were 21 types of pathology in 113 patients. The top five types were focal nodular hyperplasia (30 cases), hepatocellular adenoma (16 cases), dysplasia nodules (14 cases), perivascular epithelioid cell tumors (12 cases), and mucinous cystic neoplasms (11 cases), accounting for 73.5% cases. All the patients were alive in the follow-up period ranging from 6 to 120 months. \u0000 \u0000 \u0000Conclusion \u0000Preoperative diagnosis of rare benign space-occupying lesions of the liver is very difficult. Preoperative MRI is helpful for diagnosis. Conservative treatment or follow-up observation can be considered for the type malignancy have never been reported. Short-term venous recanalization rate scores and long-term proximal valvular function of left femoral vein were compared. The survival curve was drawn using kaplan-meier method, and incidence of PTS was compared with the Log rank test. \u0000 \u0000 \u0000Result \u0000The short-term venous recanalization rate scores of the patients in the comprehensive treatment group were higher than scores of the patients in the traditional treatment group(t=9.872, P<0.001). 16 patients developed PTS. The incidence of PTS in the comprehensive treatment group was lower than that in the traditional treatment group(χ2=7.146, P=0.008). Han, Yimin Wang, Yingwei Xue","doi":"10.3760/CMA.J.ISSN.1007-631X.2020.01.005","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1007-631X.2020.01.005","url":null,"abstract":"Objective \u0000To study the relationship between preoperative serum ALT and clinicopathological factors in patients with gastric cancer after radical gastrectomy. \u0000 \u0000 \u0000Methods \u0000At the Department of Gastrointestinal Surgery, Tumor Hospital of Harbin Medical University from Jan 2008 to Dec 2010, 491 patients were grouped according to ROC curve cut-off point of serum ALT. The relationship between ALT and clinical factors was analyzed, and single-factor and multi-factor survival analysis was performed. \u0000 \u0000 \u0000Results \u0000There were 201 patients with ALT ≤ 20.05 U/L, and 290 patients with ALT >20.05 U/L. Serum ALT was associated with age(χ2=11.231, P 20.05U/L were 83.5%, 51.4%, 42.1% and 66.2%, 27.4%, 15.7%. There was significant difference between the two groups (χ2=41.711, P<0.001). Deng, Mancai Wang, Xiaodong Xu, Youcheng Zhang","doi":"10.3760/CMA.J.ISSN.1007-631X.2020.01.006","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1007-631X.2020.01.006","url":null,"abstract":"Objective \u0000To investigate the predictive value of metastatic lymph node ratio (MLNR) combined with preoperative serum carcinoembryonic antigen (CEA) in the detection of distant metastasis of stage Ⅱ-Ⅲ colon cancer after radical resection. \u0000 \u0000 \u0000Methods \u0000A retrospective study was performed on the clinical data of 325 patients with stage Ⅱ-Ⅲ colon cancer undergoing radical resection in the Second Hospital of Lanzhou University from Jan 2010 to Jan 2015. \u0000 \u0000 \u0000Results \u0000MLNR was correlated with the maximum diameter of tumors, the degree of differentiation of tumors, clinical TNM stage and distant metastasis (P<0.05), serum CEA was correlated with the maximum diameter of tumors, clinical TNM stage, nerve or vascular invasion and distant metastasis (P<0.05).
