Asthenia is a subjective, static feeling of weakness, not necessarily related to effort. We propose a simple algorithm which can help primary care physicians in evaluation and counselling of persons presenting with asthenia. (Table 1). {"title":"Comparative effectiveness of silodosin vs. tamsulosin in treating acute urinary retention in benign prostatic hyperplasia patients.","authors":"Hassaan Ul Hassan, Tanzeel-Ur-Rahman Al Gazder, Nawaz Khan, Abbas Jafri, Asad Shahzad Hassan, Altaf Hashmi","doi":"10.47391/JPMA.20668","DOIUrl":"https://doi.org/10.47391/JPMA.20668","url":null,"abstract":"<p><strong>Objective: </strong>To evaluate the effectiveness of silodosin and tamsulosin in patients with acute urine retention caused by benign prostatic hyperplasia.</p><p><strong>Methods: </strong>The prospective cohort study was conducted at the Sindh Institute of Urology and Transplantation, Karachi, from July to December 2022, and comprised male benign prostatic hyperplasia patients with acute urine retention. The patients were divided into group A receiving 0.4mg tamsulosin and group B receiving 8mg silodosin. The catheter was withdrawn after seven days. Trial without catheterisation success was measured by patients' postvoid residue. All patients were reassessed for post-void residue at 4 weeks after a successful trial without catheterisation. Data was analysed using SPSS 21.</p><p><strong>Results: </strong>Of the 130 male subjects with mean age 64.79±9.71 years, mean prostate volume 46.22±6.82ml and mean post-void residual 96±13ml on day 7 and 89.73±14.38ml on day 28, 65(50%) were in each of the two groups. Demographic information, comorbid conditions, selected haemogram parameters and ratios, acute-phase reactants, and biochemical markers were recorded along with outcomes. Data was analysed using SPSS 20.</p><p><strong>Results: </strong>Of the 164 patients, 91(55.5%) were males and 73(44.5%) were females. Of them 101(61.6%) died; 52(71.2%) females and 49(53.8%) males with mean age 73.98+/-11.73 years. The remaining 63(38.4%) were discharged; 42(46.2%) males and 21(28.8%) females with mean age 67.69+/-13.05 years (p<0.05). Progressive muscular weakness and an irreversible loss of alpha motor neurons in the spinal cord are the hallmarks of SMA and are associated with increased sensitivity to opioids and muscle relaxants. SMA is classified clinically into four categories according to its severity and age of onset, which poses a challenge for the anaesthesiologist. Although general anaesthesia can be given, it is preferable to avoid muscle relaxation. We are reporting the anaesthetic management of a patient with type 3 SMA, diagnosed with a case of bullous lung disease. The procedure planned was thoracoscopy and pleurodesis/pleurectomy under general anaesthesia at the Aga Khan University Hospital, Karachi, in August 2022. This review explores the role of graph network analysis in predicting cognitive function and overall survival (OS) in glioma patients, focussing on studies that applied graph theory metrics to evaluate cognitive and survival outcomes in glioma patients. Various studies showed that graph network alterations in glioma patients were associated with cognitive decline and worse OS. Small-world network properties were disrupted, with reductions in global efficiency and clustering coefficients correlating with neurocognitive deficits. Network hubs, which are critical for brain integration, were significantly affected, further contributing to functional impairments. {"title":"Premature atrial contractions and their associations with cryptogenic strokes.","authors":"Muhammad Kamran Khan, Maheen Fayaz, Raja Farhat Shoaib, Waseem Tariq Malik, Laiba Fayaz, Saeedullah Shah","doi":"10.47391/JPMA.11075","DOIUrl":"https://doi.org/10.47391/JPMA.11075","url":null,"abstract":"<p><strong>Objective: </strong>To assess the association between premature atrial contractions and cryptogenic stroke while comparing the prevalence of premature atrial contractions among cryptogenic and non-cryptogenic stroke patients.</p><p><strong>Methods: </strong>The retrospective, cross-sectional study was conducted at the Shifa International Hospital, Islamabad, Pakistan, and comprised data from January 2020 to April 2021 of patients diagnosed with ischemic stroke on magnetic resonance imaging of the brain, and who underwent investigation and treatment according to the national stroke guidelines. Patients with cryptogenic stroke were placed in group A, and those with noncryptogenic stroke in group B. Data on demographic information, stroke type, risk factors, and cardiac assessment was collected from the institutional electronic medical records. Data was analysed using SPSS 25.</p><p><strong>Results: </strong>Of the 118 patients, 44(37.3%) were in group A; 35(79.5%) males and 9(20.5%) females with mean age 64.09±12.28 years. There were 74(62.7%) patients in group B; 57(77%) males and 17(23%) females with mean age 65.56±12.12 years. Premature atrial contractions were detected in 43(97.7%) group A patients compared to 1 patient (2.3%) in group B (p<0.05), and demonstrated significant association with atrial fibrillation, dyslipidaemia, mitral stenosis and left ventricular systolic dysfunction (p<0.05) in group A, and with atrial fibrillation, previous myocardial infarction, left atrium dilatation, wall akinesia, left carotid artery stenosis, and left ventricular systolic dysfunction in group B (p<0.05).</p><p><strong>Conclusions: </strong>Prolonged cardiac monitoring pf stroke patients is critical to any effort to identify occult atrial fibrillation. {"title":"Retrospective analysis of ventilation strategies for blunt pulmonary trauma patients: comparing the effectiveness of synchronized intermittent mandatory ventilation, continuous positive airway pressure, and pressure-controlled ventilation.","authors":"Lanlan Wu, Yanhua Hu, Yanwen Lu","doi":"10.47391/JPMA.21036","DOIUrl":"https://doi.org/10.47391/JPMA.21036","url":null,"abstract":"<p><strong>Objective: </strong>To compare the efficacy of various ventilation approaches to the treatment of patient with patients with severe blunt chest trauma.</p><p><strong>Methods: </strong>The retrospective study was conducted at the Cardiorespiratory Syndrome Ward of the First People's Hospital of Linping District, Hangzhou, China, and comprised data of patients with blunt pulmonary trauma admitted from December 2019 to February 2023. The patients were divided into three groups based on the ventilation support mode. Group 1 received synchronised intermittent mandatory ventilation + positive endexpiratory pressure, Group 2 received continuous positive airway pressure + pressure support ventilation, and Group 3 recieved pressure-controlled ventilation. Clinical curative effect, respiratory mode parameters, and blood gas analysis indicators of the groups were compared before and after the treatment. Data was analysd using SPSS 20.</p><p><strong>Results: </strong>Of the 227 patients, 71(31.3%) were in Group A; 62(87.3%) males and 9(12.7%) females with mean age 69.6±5.9 years. There were 74(32.6%) patients in Group 2; 59(79.7%) males and 15(20.3%) females with mean age 71.2±6.2 years. In Group 3, there were 82(36.1%) patients; 61(74.4%) males and 21(25.6%) females with mean age 69.0±6.0 years (p>0.05). Most patients 146(64.32%) recovered with no significant differences in treatment effect across the groups (p>0.05).
