While insulin therapy remains the standard of care, advancements in immunotherapy present promising alternatives aimed at halting disease progression and reducing insulin dependence. This brief review highlights key immunomodulatory therapies, including teplizumab, which has demonstrated the ability to delay the onset of T1D, and rituximab, known for preserving β-cell function, though its effects tend to be transient. Emerging treatments, such as stem-cell therapies, also show potential. However, significant challenges remain, including high costs, long-term safety concerns, and the need for personalized care. {"title":"Short-term outcome of metabolic surgery versus intensive medical therapy among obese type 2 diabetics: a quasi-experimental study.","authors":"Zahra Raza, Muhammad Burhan Ul Haq, Tashfeen Imtiaz, Muhammad Adnan Khokhar, Aatif Inam","doi":"10.47391/JPMA.10425","DOIUrl":"10.47391/JPMA.10425","url":null,"abstract":"<p><strong>Objective: </strong>To compare the short-term outcomes of metabolic surgery and intensive medical therapy in the management of diabetes mellitus among obese persons with type 2 diabetes mellitus.</p><p><strong>Methods: </strong>The quasi-experimental study was conducted at the Pakistan Institute of Medical Sciences, Islamabad, Pakistan, from March 2018 to February 2019, and comprised obese type 2 diabetes mellitus patients of either gender aged 20-65 years. Those undergoing metabolic and bariatric surgery were assigned group A, while those undergoing intensive medical therapy formed control group B. All patients were followed up for 6 months to monitor their metabolic profile, including body mass index, glycosylated haemoglobin and fasting plasma glucose. Data was analysed using SPSS 20.</p><p><strong>Results: </strong>Of the 72 patients, 36(50%) were in group A; 23(63.9%) females and 13(36.1%) males with mean age 41.2 ± 10.2 years. The remaining 36(50%) patients were in group B; 22(61.1%) males and 14(38.9%) females with mean age 42.5 ± 10.6 years (p>0.05). At 6 months, mean body mass index, glycosylated haemoglobin and fasting plasma glucose values were significantly lower in group A compared to group B (p<0.001). {"title":"Potential hip fracture early death predictors - a retrospective analysis from prospective trauma registry.","authors":"Tashfeen Ahmad, Zehra Abdul Muhammad","doi":"10.47391/JPMA.20102","DOIUrl":"10.47391/JPMA.20102","url":null,"abstract":"<p><strong>Objective: </strong>To explore the blood pressure, fracture anatomical site, and age as potential predictors of death within 3 months after proximal femoral fracture management.</p><p><strong>Methods: </strong>The retrospective study was conducted from June 2015 and March 2020 at the Aga Khan University Hospital, Karachi and comprised orthopaedic trauma registry data of isolated proximal femur fracture patients managed between June 2015 and March 2020. Data of those who expired within 3 months of the management was pooled in group A, while data of those who survived >3 months post-treatment formed group B. Blood pressure at the time of hospital admission was considered normal at 90-120mmHg systolic and 60-80mmHg diastolic. Data was analysed using SPSS version 19.0.</p><p><strong>Results: </strong>Of the 242 patients, 9(3.7%) were in group A; 5(55.5%) males and 4(44.4%) females with median age 78 years (interquartile range: 7 years). There were 233(96.3%) patients in group B; 123(52.7%) females and 110(47.2%) males with median age 70 years (interquartile range: 17 years). In 12 months post-treatment, 16(6.6%) patients expired; 9(56.25%) in group A, and 7(43.75%) in group B. Conducted on a national scale in Pakistan, the current study was planned to establish a consensus among orthodontic professionals regarding critical risks involved in such a discussion. The crosssectional survey involving 132 orthodontists across Pakistan led to the emergence of 10 key risks, including relapse, treatment duration and gingivitis. However, the study's sole focus on orthodontists' perspectives limited its insights, overlooking invaluable input from patients' experiences. Nevertheless, the consensus underscored the significance of addressing these risks to ensure informed consent and patient autonomy. Mechanistically, it operates through diverse pathways, including modulation of endothelial nitric oxide synthase, antiapoptotic properties, and regulation of pyruvate dehydrogenase phosphorylation to boost cellular energy production and mitochondrial function. Furthermore, its interactions with cellular receptors, signalling cascades and metabolic regulators unveil its prowess in mitigating cardiovascular risks and enhancing glycaemic control. To transition adropin from bench to bedside, rigorous human trials are imperative, encompassing dose optimisation, long-term safety assessments, and exploration of genetic influences. Inclusive research targetting diverse populations, especially high-risk cohorts, is advocated to unravel its full therapeutic potential. This association stems from a complex interplay of biological and psychosocial factors, including the accumulation of uraemic toxins such as middle molecules, chronic inflammation, and the psychological burden of disease progression. Initiation of dialysis has been observed to alleviate depressive symptoms through multiple mechanisms: removal of uraemic toxins, improvement in physical health, and reduction in psychosocial stressors. Individuals with autoimmune conditions are highly susceptible to pulmonary embolism. This case involves a 39-year-old female with ITP who developed thromboembolisms. A comprehensive investigation into prothrombotic risk factors, including obesity, smoking, family history of thromboembolism, and ITP-related interventions such as prolonged corticosteroid use or splenectomy, revealed insignificant results, except for the patient's history of receiving corticosteroid therapy. Despite the disparate pathogenesis of thromboembolism (TE) and ITP, recent events of TE in ITP patients have been observed.
