This article combines insights from recent research and guidelines to offer a thorough framework for risk assessment. Patients are classified into low, moderate, and high-risk categories based on individual, environmental, and health-related factors. Management approaches are organized into four main areas: spiritual support, non-drug interventions, medication adjustments, and modifications to kidney replacement therapy (KRT). Evidence indicates that stable chronic kidney disease (CKD) patients, especially those in stages 1-3, and kidney transplant recipients with stable graft function can fast safely with careful supervision. Conversely, patients with advanced CKD, those on dialysis, and individuals with significant additional health issues are advised against fasting due to possible complications. Essential recommendations include optimizing hydration, customizing medication plans, conducting regular monitoring, and making dietary adjustments. Lumbar traction is an effective conservative treatment for DLBP, shown to decrease pain, improve functional disability, and positively affect disc height and herniation index. While typically applied in the supine position, lumbar traction can also be administered prone. However, many practitioners are unaware of this alternative approach. Studies have demonstrated that prone lumbar traction can effectively alleviate pain and disability in persons with DLBP, lumbar disc herniation, and radiculopathy. Some research suggests it may be more effective than supine traction for certain subgroups. Despite these promising findings, additional research is needed to further explore the clinical and biomechanical effects of prone traction. Additionally, it is important to identify which patient subgroups are more likely to benefit from prone versus supine traction. {"title":"Thoracic spinal thrust manipulation for shoulder impingement syndrome: A meta-analysis of pain relief and functional outcomes.","authors":"Bazal Bukhari, Sarwat Ali, Saba Afzal","doi":"10.47391/JPMA.11608","DOIUrl":"10.47391/JPMA.11608","url":null,"abstract":"<p><strong>Objective: </strong>To evaluate the effectiveness of thoracic spine thrust manipulation in managing shoulder impingement syndrome.</p><p><strong>Methods: </strong>The meta-analysis was conducted from April 25 to November 5, 2023, after approval from the ethics review board of the University of Lahore, Lahore, Pakistan, and comprised a comprehensive search on multiple databases, including Cochrane Central Register of Controlled Trials PubMed, PEDro and MEDLINE. Randomised controlled trials with patients of either gender aged 19-44 years who had been diagnosed with shoulder impingement syndrome were included. Data extraction was independently performed by two researchers, and meta-analysis was conducted using RevMan 5.2.</p><p><strong>Results: </strong>Of the 14 studies comprising 749 participants, 8(57.14%) studies comprising 410(54.73%) participants involved quantitative analysis. The fixed-effect model revealed minimal heterogeneity (I2=0%) and a standardised mean difference of -0.03 (95% confidence interval: -0.22-0.16), indicating no significant improvement in clinical outcomes following thoracic spinal thrust manipulation.</p><p><strong>Conclusions: </strong>The meta-analysis provided a more complex view of the efficacy of thoracic spinal thrust manipulation in treating shoulder impingement syndrome. Some researchers suggested that treatment by manipulation positively affected immediate changes in range of motion and pain, while others revealed no significant clinical changes. {"title":"Effects of physical therapy on improving disc height index, postural stability, pain and function in persons with discogenic low back pain.","authors":"Muhammad Osama, Aamer Naeem, Sameen Tariq, Sumera Mushtaq, Furqan Ahmed Siddiqi, Sabah Afridi","doi":"10.47391/JPMA.20384","DOIUrl":"10.47391/JPMA.20384","url":null,"abstract":"<p><strong>Objective: </strong>To determine if physical therapy is capable of improving disc height index, pain, functional disability, lumbar range of motion, postural stability and gait in persons with discogenic low back pain.</p><p><strong>Methods: </strong>A quasi-experimental study was conducted at the Foundation University College of Physical Therapy, Foundation University, Islamabad, and Radiology Department, Fauji Foundation Hospital, Rawalpindi, Pakistan, from May 2022 to June 2023, and comprised patients of either gender having postero-lateral disc herniation and discogenic low back pain. Pain was assessed using the visual analogue scale, disc height index was measured via magnetic resonance imaging, postural stability was evaluated using the Biodex balance system, lumbar range of motion was measured via inclinometer, and functional disability was assessed using the Oswestry disability index. Treatment protocol spanned 2 weeks and consisted of 5 supervised sessions per week. The protocol included interferential therapy combined with heating pack, Mckenzie's extension bias protocol, manual lumbar traction and rotational mobilisation. Data was collected at baseline and postintervention. Data was analysed using SPSS 21.</p><p><strong>Results: </strong>Of the 22 patients, 18(81.8%) were males and 4(18.2%) were females. The overall mean age was 40.13±10.08 years, mean weight was 70.54±11.38kg and mean height was 160.58±13.52 cm. {"title":"Evaluation of pulmonary function tests and vitamin D level in pre and postmenopausal women.","authors":"Saima Zareen, Faraz Ahmed Bokhari, Sibgha Zulfiqar","doi":"10.47391/JPMA.11432","DOIUrl":"10.47391/JPMA.11432","url":null,"abstract":"<p><strong>Objectives: </strong>To assess the increased risk of vitamin D deficiency in postmenopausal women, and its correlation with pulmonary function decline in both premenopausal and postmenopausal women.</p><p><strong>Methods: </strong>The cross-sectional, comparative study was conducted at the Physiology Department, Federal Postgraduate Medical Institute, Shaikh Zayed Hospital, Lahore, Pakistan, from May 2019 to May 2020. Healthy premenopausal women having regular menstrual cycles aged at least 40 years were placed in group 1, while healthy postmenopausal women having no menstruation for the preceding 12 months with maximum age 55 years were placed in group 2. Serum vitamin D levels were determined using enzyme-linked immunosorbent assay, and pulmonary functions by spirometry. Groups 1 and 2 were divided into hypovitaminosis and vitamin D-sufficient subgroups 1A, 1B, 2A and 2B. Data was analysed using SPSS 21.</p><p><strong>Results: </strong>Of the 120 female subjects, 60(50%) were in each group. Mean age in years of group 1(premenopausal group) was 46.03±2.35. Mean age in years of group 2 (postmenopausal group) was 52.72±1.43. Mean body mass index was 23.0± 2.59kg/m2 in group 1 compared to 24.14±2.76kg/m2 in group 2. Mean serum vitamin D level was 42.91±16.94ng/ml in group 1, and 27.42±6.70ng/ml in group 2 (p=0.001). The percentage predicted value of age for forced expiratory volume in 1 second, forced vital capacity, and the ratio between the two in group 1 were 82.95±2.75%, 83.10±2.65% and 0.998±0.003% respectively. In group 2, the corresponding values were 79.48±1.20%, 79.12±1.40% and 1.004±0.010% (p<0.05). The patients were subjected to percutaneous nephrostomy, and they were monitored for 15 days after the surgery. Complications post-procedure were noted using the Modified Clavien Classification System. Data was analysed using SPSS 26.</p><p><strong>Results: </strong>Of the 90 patients with mean age 50.19±14.70 years, 53(58.9%) were males and 37(41.1%) were females. Grade I complications were observed in 37(41.1%) cases, grade II 8(8.9%), grade IIIa 11(12.2%), and grade IVa in 1 (1.1%) case. No complications were observed in 33(36.7%) cases. {"title":"A comparative of multiple rib fractures with embracing fixators and riblocking titanium plates.","authors":"Wu Wen, Yong Qiao","doi":"10.47391/JPMA.20047","DOIUrl":"10.47391/JPMA.20047","url":null,"abstract":"<p><strong>Objective: </strong>To compare the clinical efficacy and safety between embracing fixator and rib-locking titanium plate in the treatment of multiple rib fractures.</p><p><strong>Methods: </strong>The retrospective study was conducted at the Sichuan Province Orthopaedics Hospital, China, and comprised clinical data from January 2020 to December 2022 of patients with multiple rib fractures. The patients were divided into the embracing fixator group A and rib-locking titanium plate group B. Data was analysed using SPSS 22.</p><p><strong>Results: </strong>Of the 100 patients, 50(50%) were in each of the two groups. There were 35(70%) males and 15(30%) females in group A with mean age 51.38±10.62 years (range: 25-62 years) compared to 33(66%) males and 17(34%) females with mean age 51.48±10.93 years (range: 27-65 years) (p>0.05). Surgical duration and incisional length were shorter, and the blood loss was smaller in group B compared to group A (p<0.05). Three months after treatment, the visual analogue scale VAS) score of group B was lower than group A (p<0.05). All markers of pulmonary function significantly improved both groups (p<0.05) compared to baseline values, but group B patients showed more and better improvement than group A (p<0.05).
