The current study was planned to determine whether or not rituximab increases the risk and severity of coronavirus diseases-2019 infection in pemphigus vulgaris patients. The retrospective study was conducted at the Lady Reading Hospital, Peshawar, Pakistan, and comprised data from the dermatology unit of 34 diagnosed cases of pemphigus vulgaris who received rituximab treatment from March 2020 to March 2022. The mean age of the patients was 36.58±12.44 years. Majority of patients 28(82.3%) received rituximab as a second-line therapy, while 6(17.64%) were given it as a first-line therapy. Only 1(2.9%) patient developed mild covid pneumonia, and did not need in-patient care or faced oxygen dependence. There was a significant weak association between coronavirus diseases-2019 positivity with age (p=0.000). It is a combination of an enlarging vascular lesion, thrombocytopenia, microangiopathic haemolytic anaemia, and hypofibrinogemia. The case of a nine-month-old child who presented with a tuft haemangioma leading to the clinical features of KMP is presented. The study took place between June and July 2022 at the Indus hospital in Karachi. Healthcare workers who had previously screened negative to prevaccination Severe Acute Respiratory Syndrome Coronavirus 2 nucleocapsid antibodies were tested for post-vaccination Severe Acute Respiratory Syndrome Coronavirus 2 anti-spike immunoglobulin G levels using a quantitative assay. The test was also performed on the stored pre-vaccination samples of the subjects collected up to 18 months previously. Antibody levels in subjects without infection, with infection and with booster administration were compared. The median postvaccination anti-spike immunoglobulin G in infected only, infected with routine vaccination and infected with booster values were 1,725.6 IU/mL (interquartile range: 684.80- 4,708.9 IU/mL), 2,067.15 IU/mL (interquartile range: 705.33-4,670.4 IU/mL) and 6,139.15 IU/mL (interquartile range: 2,426.05-10,623.40 IU/mL). There was a doubling of antibody titers, from 1,744 IU/mL to 3,829 IU/mL, in those who received a booster versus routine vaccination (p>0.05). {"title":"A comparative analysis of the efficacy of intrauterine misoprostol in conjunction with oxytocin versus oxytocin monotherapy for the prophylaxis of primary postpartum haemorrhage.","authors":"Afnan Rizwan, Muhammad Hassaan Farooq, Uzma Nayyer, Asma Irfan, Mahwish Farooq, Maryam Siddique","doi":"10.47391/JPMA.11080","DOIUrl":"https://doi.org/10.47391/JPMA.11080","url":null,"abstract":"<p><strong>Objectives: </strong>To determine the effectiveness of intrauterine misoprostol along with oxytocin versus oxytocin monotherapy for the prevention of primary postpartum haemorrhage.</p><p><strong>Methods: </strong>The quasi-randomised controlled trial took place at the Akbar Niazi Teaching Hospital, Islamabad, Pakistan, from February to August 2021, and comprised women who underwent lower segment caesarean section and at fullterm pregnancy who had risk factors for experiencing postpartum haemorrhage. The subjects were randomised into experimental group A and control group B. Group A subjects received both misoprostol and oxytocin, while those in group B received oxytocin only. Data was analysed using SPSS 26.</p><p><strong>Results: </strong>Of the 120 subjects, 60(50%) were in group A with mean age 28.72±5.82 years and mean gestational age 38.02±1.1 weeks. The remaining 60(50%) subjects were in group B with mean age 29.47±4.72 years and mean gestational age 37.50±1.5 weeks. Group A subjects had fewer changes in haemoglobin levels post-operation than those in group B (p<0.05). Medical history, family history and ophthalmological examination results of each patient were reviewed, and the causes and frequency of occurrence of visual impairment and blindness were determined. Data was analysed using SPSS 21.</p><p><strong>Results: </strong>Of the 433 patients, 269(62.1%) were female and 164(37.9%) were male. The overall mean age was 38.52±21.90 years (range; 5-90 years) with a median age of 37 years (IQR: 19-58). Refractive error 271(62.6%), cataract 69(15.9%) and diabetic retinopathy 37(8.5%) were the major causes of visual impairment, followed by amblyopia 15(3.5), corneal opacity 12(2.8), and glaucoma 8(1.8). It is very uncommon for uterus, ovaries and fallopian tubes to be present in hernial sac altogether. Since strangulation and necrosis of these organs may result in serious illness and infertility, early detection and surgery are essential. The most useful technique for diagnosing and differentiating infant inguinal hernias is ultrasound. We sought to illustrate surgical features of a two months old female infant, delivered by caesarean section, presented to the Paediatric Surgery Emergency Department at Mayo Hospital Lahore on 8th March 2024 with irreducible mass in left groin. The diagnosis was made via ultrasound, which revealed an indirect incarcerated inguinal hernia containing viable uterus, ovaries and fallopian tubes. Surgery was performed through an inguinal approach, where the uterus, fallopian tubes and ovaries were found within the hernial sac. High ligation and additional repair of internal inguinal ring were done after reducing the contents in the peritoneal cavity. {"title":"Role of different methods of contraception and time of menstrual cycle on dry socket.","authors":"Sidra Abid, Javeria Khan, Uzma Hameed, Mohsina Marufi, Amynah Charania Sheikh","doi":"10.47391/JPMA.11164","DOIUrl":"https://doi.org/10.47391/JPMA.11164","url":null,"abstract":"<p><strong>Objectives: </strong>To investigate the potential association between the use of contraceptives and the occurrence of dry socket following tooth extraction.</p><p><strong>Methods: </strong>The cross-sectional study was conducted at the Dow International Dental College, Karachi, from April to October 2023, and comprised female patients aged 18-45 years who underwent posterior maxillary and mandibular tooth extractions. All the extractions were conducted under local anaesthesia by the same surgeon. After extraction, the patients were given preventive anti-inflammatories and prophylactic antibiotics. Extraction success was the dependent variable, while contraceptive use was the independent variable, and the association between the two was evaluated. Data was analysed using SPSS 21.</p><p><strong>Results: </strong>Of the 193 female subjects, female patients aged 18-45 years 68(35.2%) were in the follicular phase of their menstrual cycle, 106(54.9%) were in the luteal phase, and 19(9.8%) were in the ovulation phase. Regarding contraceptive methods, 133(68.9%) used oral contraceptive pills, followed by 28(14.5%) using combined oral contraceptive pills. The development of dry socket was observed in 61(31.6%) cases. A significant positive correlation was found between oral contraceptive use and an increased risk of dry socket (p<0.05).
