{"title":"Enablers& barriers to online and e-learning for health professionals in Pakistan: a formative research.","authors":"Zahra Saleem Ladhani, Amira Mushtaq Khan, Zulfiqar Ahmed Bhutta","doi":"10.47391/JPMA.20453","DOIUrl":"https://doi.org/10.47391/JPMA.20453","url":null,"abstract":"<p><strong>Objective: </strong>To design a continuous professional development programme pertaining to integrated nutrition, health, and Water, Sanitation and Hygiene interventions for health professionals (HP).</p><p><strong>Methods: </strong>The cross-sectional study was conducted from January 1 to April 30, 2024, in Lahore, Pakistan, after approval from the ethics review committee of the Institute of Clinical Psychology, University of Management and Technology, Lahore. The sample was raised from among students of either gender aged 18-26 years studying at various public and private universities in Lahore. Data was collected using the Attachment Styles Scale, Dating Violence Scale, Self-Criticism Scale and the Psychosocial and Emotional Reactions Scale. Data was analysed using SPSS 25.</p><p><strong>Results: </strong>Of the 1,768 publications identified, 41(2.3%) were analysed in detail. Of the 15 key informants interviewed, 7(46.6%) were HPs, while 4(26.7%) each were police-makers and facilitators or designers of online modules. Under the two broad categories of \"enablers\" and \"barriers\", five subthemes emerged for each. The subthemes related to enablers included personal, content and design, quality of facilitators, peer connectivity, and social networking. The subthemes related to barriers were structural, cultural, curricular and content, human resources, and financial.</p><p><strong>Conclusion: </strong>In Pakistan, the corona virus disease-2019 pandemic brought about a forced digital transformation for health professions students. Blended learning emerged as an effective e-learning model. {"title":"Acute effects of caffeine on intraocular pressure.","authors":"Irum Riaz, Muhammad Fayaz, Fatima Zahid, Sadia Noureen, Hakim Anjum Nadeem","doi":"10.47391/JPMA.20294","DOIUrl":"https://doi.org/10.47391/JPMA.20294","url":null,"abstract":"<p><strong>Objectives: </strong>To examine the acute effects of caffeine on the intraocular pressure among healthy young individuals.</p><p><strong>Methods: </strong>The quasi-experimental study was conducted from January to May 2023 at the Superior University, Lahore, Pakistan, while data was collected from Al-Khair Trust Eye Hospital, Lahore. The sample comprised subjects aged 18- 27 years who had healthy eyes and were non-habitual consumers of coffee. Blood pressure and intraocular pressure were measured at baseline and then again at 30, 60 and 90 minutes after coffee ingestion. Individuals were given freshly produced instant coffee with caffeine. After an interval of two days, the same group was given instant decaffeinated coffee. Data was analysed using SPSS 25.</p><p><strong>Results: </strong>Of the 73 subjects, 46(63%) were males and 27(37%) were females. The overall mean age was 22.52±2.08 years (range: 19-27 years). Caffeine consumption raised intraocular pressure (p<0.001) and blood pressure (p=0.007) significantly compared to the baseline. These subgroup haplotypes include Benin, Arab-Indian, Senegal, Bantu, Algeria, Cameroon, Central African Republic, Saudi and various other varieties. The current study was planned to determine the formation periods and network linkage relationships of genetic variations linked to haemoglobin S in the Denizli area and other geographical regions. The haplotype data was processed using Network software to calculate possible links between haplotypes and their formation dates. The results uphold the possibility that the haemoglobin S mutation may have come to the area through migratory pathways. According to analysis, the Benin haplotype historically formed first, followed by the Senegal and Arab-Indian haplotypes. The sample comprised patients diagnosed with obstructive nephropathy secondary to stones admitted to the Department of Urology and Renal Transplant, Lahore General Hospital, Lahore, Pakistan, between January 1, 2018, and December 31, 2021. Data was analysed using SPSS 20. Of the 76 patients with a mean age of 50.06±13.42 years, 56(73.7%) were males and 20(26.3%) were females. The most frequent clinical cause was bilateral ureteric obstruction in 39 (51.3%) patients. The most common emergency procedure performed was unilateral percutaneous nephrostomy. It took an average of 10.8±2.3 days for patients to have a nadir value of creatinine. {"title":"Clinical profile, management, and outcome of electric burn injuries at a tertiary care hospital.","authors":"Bushra Akram, Farrukh Aslam Khalid, Usman Ishaq, Sidra Shumshir Ali, Muhammad Younas Mehrose, Yawar Sajjad","doi":"10.47391/JPMA.11447","DOIUrl":"https://doi.org/10.47391/JPMA.11447","url":null,"abstract":"<p><strong>Objective: </strong>To determine the clinical profile, management and outcomes of electric burn injury cases in a tertiary care setting.</p><p><strong>Methods: </strong>The prospective, cross-sectional study was conducted at the Jinnah Burns and Reconstructive Surgery Centre, Lahore, Pakistan, from August 2021 to July 2022, and comprised patients admitted with electric burns. Data collected included demographic variables, total burn surface area, associated injuries, entry and exit points, areas/regions involved, number of blood transfusions, number and type of amputations, total stay in hospital, surgical procedures, and clinical outcomes. The patients were followed up for six months after discharge from hospital. Data was analysed using SPSS 21.</p><p><strong>Results: </strong>Of the 883 patients, 91(10.3%) had electric burns; 70(76.9%) males and 21(23.1%) females. There were 44(48.3%) patients aged 21-40 years and 37(40.6%) were aged >40 years. Total body surface area was >20% in 62(68.2%) patients. Amputation was required in 25(27.5%) cases. The duration of hospital stay was up to 15 days in 50(54.9%) cases. Despite its rarity, mesenteric cyst should be considered in patients with abdominal masses, especially those with a history of trauma. Diagnostic imaging, such as ultrasound and computerised tomography scan, aids in characterisation and surgical planning. Surgical excision remains the mainstay of treatment, offering favourable outcomes and reducing the risk of recurrence and malignancy. We report the case of a 15-year-old male with progressively enlarging abdominal swelling following blunt abdominal trauma, who was ultimately diagnosed with a chylous mesenteric cyst. The originator semaglutide, for example, is a biologically active compound, but later versions are chemically synthesized generics. Tirzepatide is a synthetic polypeptide. These molecules are marketed in different strengths and delivery devices across the world. The word 'pharmaco-similar' can be used to describe both biosimilar and generic versions of originator peptidebased medications. This communication suggests a useful framework which assists in sharing information and reaching an informed decision regarding choice of anti-obesity medication.
