Les résultats attendus après PTG sont une diminution de la douleur, une meilleure fonction et une amélioration de la qualité de vie avec un résultat durable dans le temps. Cependant, 10 à 34 % des patients restent insatisfaits après leur intervention. La cause principale de ce résultat imparfait est, dans la majorité des cas, une douleur persistante. Les principales causes de douleur persistantes après PTG sont : les causes périarticulaires, les douleurs projetées, les douleurs neuropathiques, les descellements, l’infection et les instabilités et anomalies de positionnement des implants. Il existe des facteurs de risque de persistance voire de chronicisation (> 3 mois) de cette douleur (douleur sévère préopératoire, anxiété, catastrophisme, douleur chronique préexistante, prise chronique d’opioïdes, facteurs génétiques, etc.). Un patient qui présente une douleur persistante après son intervention doit être exploré de façon structurée et logique. Pour cela l’examen clinique est essentiel et va orienter la demande d’examens complémentaires. Il convient dans un premier temps de rechercher une cause intrinsèque : échec mécanique, infectieux ou allergique de l’implant. Ensuite, il est nécessaire de chercher les causes extrinsèques : conflits périarticulaires, douleurs projetées (hanche et rachis). Une fois toutes ces causes éliminées, il faudra rechercher une douleur neuropathique ou un syndrome douloureux régional complexe. Une douleur neuropathique peut être associée à une autre cause telle que l’échec mécanique. Une fois le diagnostic établi, il sera possible de proposer un traitement optimal.</div></div><div><div>Osteoarthritis is a disease that is on the rise, leading to a significant increase in the number of total knee replacements (TKRs). The expected results of TKR are less pain, better function and improved quality of life, with results that last over time. However, between 10% and 34% of patients remain unsatisfied after their procedure. The main cause of this unsatisfactory outcome is, in the majority of cases, persistent pain. The main causes of persistent pain after TKR are: periarticular causes, projected pain, neuropathic pain, loosening, infection and instability, and abnormal positioning of implants. There are risk factors for persistence or even chronicity (> 3 months) of this pain (severe preoperative pain, anxiety, catastrophizing, pre-existing chronic pain, chronic opioid use, genetic factors, etc.). A patient presenting with persistent postoperative pain must be examined in a structured and logical manner. The clinical examination is essential and will guide the ordering of further tests. The first thing to look for is an intrinsic cause: mechanical, infectious or allergic failure of the implant. Dans ce contexte, les interventions non médicamenteuses sont fréquemment plébiscitées pour la prévention et le traitement de la douleur. Les soignants doivent faire évoluer leurs compétences dans ce domaine et croire encore plus à l’intérêt de ses différents moyens. Il serait d’ailleurs pertinent de développer dans ce cadre, une science de « l’accompagnement de proximité » qui sollicite des interventions simples, discutées dans ce travail.</div></div><div><div>For 20 years, healthcare professionals have been thinking about how to better support patients in preventing pain during treatment. In this context, non-pharmacological interventions are frequently favored for the prevention and treatment of pain. Caregivers must develop their skills in this area and believe even more in the benefits of its different means.
