La prise en charge antalgique est complexe du fait des multiples mécanismes physiopathologiques de la douleur liée à la maladie. Une approche multimodale pluridisciplinaire s’impose alors, afin d’adapter au mieux la stratégie thérapeutique à chaque patiente. L’hormonothérapie continue permet d’obtenir une aménorrhée bénéfique. Du point de vue des traitements médicamenteux, ce sont ceux utilisés couramment en douleur chronique. L’intérêt de la toxine botulinique et du cannabis sont en cours d’évaluation. Néanmoins, il ne faut pas se limiter d’un traitement médicamenteux : afin de redonner de la mobilité aux tissus « figés » par la douleur, la rééducation périnéale est indispensable à l’antalgie. Enfin, les approche psychocorporelles s’intègrent parfaitement dans la prise en charge d’une maladie chronique qui impacte tous les aspects de la vie des patientes.</p></div><div><p>Endometriosis is a chronic hormone-dependent inflammatory disease that affects women from adolescence to menopause. Analgesic management is complex due to the multiple pathophysiological mechanisms of pain associated with the disease. Therefore, a multidisciplinary multimodal approach is necessary in order to adapt the therapeutic strategy to each patient. Continuous hormone therapy achieves beneficial amenorrhea. From the point of view of drug treatments, these are the ones commonly used in chronic pain. The benefits of botulinum toxin and cannabis are currently being assessed. Nevertheless, it was not limited to drug treatment: in order to restore mobility to the tissues “frozen” by pain, perineal rehabilitation is essential for analgesia. Ce n’est que plus récemment que des mécanismes impliquant une stimulation périphérique locale et résultant en une réponse centrale sous forme de stimulation du système opioïde endogène a été identifiée. Ces effets impliquent aussi d’autres neuromédiateurs (Dopamine) ainsi que le système nerveux autonome (activation parasympathique). Dans ces conditions, ces thérapeutiques dénuées de toute toxicité pourraient être proposées dans le cadre de la prise en charge de douleurs nociplastiques. Après la présentation des phases « empiriques » et les résultats expérimentaux et cliniques validant la mécanistique des expositions aux OMM, les raisons d’une potentielle indication relevant des douleurs nociplastiques seront discutées.</p></div><div><p>The analgesic effects of millimeter waves (MW) were demonstrated and used clinically on various conditions from the 1970s to the 1990s. It is only more recently that mechanisms involving local peripheral stimulation and resulting in a central response in the form of stimulation of the endogenous opioid system have been identified. These effects also involve other neuromediators (Dopamine) as well as the autonomic nervous system (parasympathetic activation). Under these conditions, these therapeutics, devoid of any toxicity, could be proposed in the management of nociplastic pain.
