In just over 300 pages, Dan Stein covers the most controversial debates in psychiatry, from a point of view that is neither clinical nor psychological, but philosophical. As Stein emphasizes in his editorial, the goal is to mediate between traditional and critical accounts of psychiatry to offer a more balanced view of the discipline. In pursuing this goal, Stein does not shun from big questions, but faces them head-on, from the relationship between mind and body to how to pursue happiness and meaning in life. The discussion is not merely an overview and quick assessment of the most influential positions in these long-standing debates, but a passionate, in-depth defense of a new, potentially transforming approach that overcomes sterile polarization and is genuinely pluralistic about methodology. An example of the integrative approach is offered in chapter 7, where Stein identifies two conceptual frameworks ‘‘guiding the future of psychiatry’’ (page 181). For clinical neuroscience progress will be made by finding well-established biomarkers linked to specific psychiatric conditions – a task that has been the main focus of psychiatry for decades. In contrast, for global mental health, psychiatry will advance by identifying social determinants of psychiatric conditions, and ‘‘by developing interventions that are feasible and acceptable around the world’’ (page 182). Stein proposes that psychiatry will make better progress by adopting an integrative approach. This requires recognizing that psychiatry has a ‘‘range of gaps,’’ those identified by clinical neuroscience and those identified by global mental health – and all the gaps need to be filled. The picture Stein sketches is inspiring, leading to a more personalized and comprehensive notion of public health, but his proposal also faces significant challenges. There are two tensions involved. A first tension emerges in the characterization of the scope of psychiatry, which is instrumental to a detection of the future directions for the discipline. Traditionally, clinical psychiatry deals with mental disorders associated with putative biological dysfunctions. But even some research programs within biological psychiatry call for a substantial reframing of such a scope. For instance, projects in this area, such as the Research Domain Criteria developed by the U.S. National Institute of Mental Health, are not based on the search for biomarkers for specific mental disorders, as noted elsewhere. Advocates of global mental health also propose that we reframe our understanding of the scope of psychiatry, emphasizing instead the role of social and environmental aspects. Some warn against underdiagnosis and undertreatment, stretching the notion of health to the point that it almost overlaps with that of agency, and issues such as poverty and thriving become legitimate health issues – as in holistic models such as Nordenfelt’s. The present study aimed to determine the prevalence of underage drinking and its associated factors among adolescents in Brazil.</p><p><strong>Methods: </strong>The 3rd National Survey on Drug Use by the Brazilian Population (III Levantamento Nacional sobre o Uso de Drogas pela População Brasileira) is a nationwide, multi-stage, probability-sample household survey. Herein, youth between the ages of 12-17 years were included. Lifetime and 12-month alcohol use prevalence were estimated. Factors associated with 12-month alcohol use were evaluated through multivariate analysis considering survey weights and design.</p><p><strong>Results: </strong>Overall, 628 youth were interviewed. Estimated lifetime and 12-month alcohol use were 34.3% (standard error [SE] = 1.9) and 22.2% (SE = 1.7), respectively. He published six books, one postmortem, the most famous being Cerebri anatome: cui accessit nervorum descriptio et usus (1664), as well as two further comprehensive books on the brain and nervous system: Pathologiae cerebri, et nervosi generis specimen. In quo agitur de morbis convulsivis, et de scorbuto (1667), and De anima brutorum quae homine vitalis ac sensitiva est (1672). Willis enrolled in the University of Oxford on March 3, 1637, received his Bachelor of Arts (chemistry) on June 12, 1639, and a Master of Arts on June 18, 1642. Known as a royalist, Willis participated in the Earl of Dover’s auxiliary regiment against the Parliamentarians. In 1646, his loyalty was rewarded with a Bachelor of Medicine after only 6 months of study. However, he had difficulty establishing a practice and spent the next decade in conjunction with the experimental philosophical club in Oxford, working with chemical theories. Willis was a founding member of the Royal Society of London, helping transform medicine into a science. His research fused the study of the brain, nerves, and behavior into a unified field known as neurology. In Willis’ physiological model, the nerves carried stimuli from the periphery to the center and from there to the muscles. The nerves occupied a prominent position in his view, which he described as the doctrine of nerves or neurology. In his book Cerebri anatome, a fundamental work of neuroscience influenced by the cultural and political context of Baroque Europe and mid-17th century England, he emphasized that anatomy could ‘‘unlock the secret places of Man’s mind and look into the living and breathing Chapel of the Deity.’’ Through his knowledge of alchemy, iatrochemistry, and his experiments with blood and urine, he reshaped the concept of animal spirits in physiology and neurophysiology. His scientific methods, allied with his religious convictions, led him to view the brain as a harmonious system designed by God. Willis believed that human and animal anatomy shared similarities but differed regarding higher functions. He believed that man had an immortal soul in addition to that which was shared with the animals. Following Galenic methods, he used what he learned in animal dissections in autopsies of deceased patients, describing parts of the body and determining their function. Social distancing measures, school clo-sures, elevated stress in the home environment, eco-nomic recession, and increased screen time may have impacted the development of youth and contributed to a worsening of their emotion regulation (ER) skills. 1 Thus, there is a growing need for evidence-based practices and strategies to guide clinicians in dealing with emerging demands.Although life is slowly returning to normal, it is important to carefully consider last year’s toll on mental health and its possible short- and long-term ramifications; evidence has been accumulating about long-term COVID-19 symptoms in youth, such as fatigue and brain fog. In addition, increasing social anxiety, which been fueled by the large amount of time in home schooling and away from peers, is one of the burdens that have been overwhelming families, schools, and clinicians alike. 2 ER strategies enable individuals to reason with and about emotions, regulating their intensity and frequency. These strategies affect one’s awareness of and ability to express emotions, as well as to handle interpersonal relationships wisely. Young people with higher levels of psychological distress show heightened difficulties in reading non-verbal cues, which translates into difficulties in understanding the emotions of oneself and others, leading into social, functional, and academic difficulties. Thus, appraising a patient’s ER skills is an essential part of a comprehensive mental health assessment. New technologies can measure psychomotor activity objectively and continuously, but evidence on the topic is scarce. Our aim is to systematically review the existing evidence about eHealth tools for assessing psychomotor activity in patients diagnosed with schizophrenia.</p><p><strong>Method: </strong>We performed a systematic search of the PubMed and Embase databases and identified 15 relevant articles on eHealth tools for assessing psychomotor activity in schizophrenia.</p><p><strong>Results: </strong>eHealth devices accurately assessed psychomotor activity and were well accepted. Abnormalities in psychomotor activity helped differentiate between different subtypes of schizophrenia. Abnormal increases in psychomotor activity were correlated with acute presentations, while lower activity was associated with relapses, deterioration, and negative symptoms.</p><p><strong>Conclusion: </strong>Actigraphy is still the preferred eHealth device in research settings, but mobile applications have great potential. Due to the coronavirus disease 2019 (COVID-19) pandemic, our minds have been conditioned to associate these symptoms with a rather severe form of the disease. Although such symptoms could indicate a case of COVID-19, they are also common in panic attacks (PAs) (i.e., the hallmark of panic disorder [PD]), in which respiratory symptoms occur along with other panic symptoms that are mainly physical in nature. Experimental evidence has supported the hypothesis, unique in the realm of mental disorders, that subclinical alterations of basic physical functioning, mainly the respiratory system, may be involved in the pathogenesis of PAs. Patients with PD were thought to have a hyperactive suffocation alarm, which results in a specific behavioral and respiratory hypersensitivity to hypercapnia. Different laboratory challenges that induce hypercapnia (e.g., the Read’s rebreathing technique, the prolonged inhalation of 5% or 7% of CO2-enriched air, and the double inhalation of a 35% CO2 and 65% O2 gas mixture) resulted in higher rates of PAs and respiratory-response abnormalities in PD patients than controls. Moreover, these patients are hypersensitive to various other laboratory respiratory challenges, such as breath-holding, hyperventilation, and a hypoxic challenge test. They suffer from irregular breathing patterns, impaired diaphragmatic breathing with reduced vital capacity, chronic hyperventilation, and a common sensation of difficulty in breathing during activities of daily living. Finally, PD has remarkable associations with asthma or chronic obstructive pulmonary disease. PD patients also have imbalanced autonomic regulation, reduced heart rate variability, poorer cardiovascular fitness, and higher variability of electrocardiographic QT interval and cardiac repolarization. Although the source of these features is still unclear, patients with PD experience, from a clinical point of view, a significant burden of respiratory and physical symptoms or discomfort during PAs, as well as in certain environmental situations. Furthermore, the fear of suffocation is one of their primary troubles. Overall, they seem to exhibit greater physical and emotional difficulties in coping with somatic sensations and internal bodily changes than individuals suffering from other anxiety disorders. Given these peculiar features, PD patients are likely to have distinct difficulty in dealing with a disease like COVID-19, since it may be severe and life-threatening and, most importantly, it may cause various respiratory symptoms, which could lead to respiratory failure. Hence, when PD patients develop COVID-19, a complex clinical picture might emerge that deserves consideration from clinicians involved in managing COVID-19 patients. As a secondary objective, a new instrument, Medical Student Stress Factor Scale (MSSF), was developed based on these stressors.</p><p><strong>Methods: </strong>The MSSF was applied to 431 medical students. The Depression, Anxiety and Stress Scale was administered to investigate the relationship between these stressors and the students' mental health.</p><p><strong>Results: </strong>The five greatest stressors were the extensive content, lack of time to study, sleep deprivation, excessive self-pressure for good grades, and lack of leisure time. The MSSF showed good psychometric characteristics, resulting in a 28-item scale with five subscales. The Relationships and Health subscales were associated with depressive symptoms; Relationships, Health, and Learning Environment/Academic Performance were associated with anxiety symptoms, while Health, and Learning Environment/Academic Performance were associated with stress symptoms.</p><p><strong>Conclusions: </strong>Our study presents important data about the most important stressors that affect medical students' lives and cause mental health issues. Our aims were to 1) determine, in a representative sample of public preschool 4- to 5-year old children in Brazil, the prevalence of internalizing and externalizing disorders and socioemotional development delays; and 2) to identify modifiable risk factors associated with mental, behavioral, or developmental disorders (MBDD), such as microsystem (i.e., parent-child relationship), mesosystem (social support), and macrosystem contextual factors (neighborhood disadvantage). Methods: A random sample of public preschool children was recruited in the city of Embu das Artes (São Paulo metropolitan area) (n=1,292 from 30 public preschools). Six-month prevalence of MBDD was measured using the Child Behavior Checklist (CBCL) and the Ages and Stages Questionnaires: Social-Emotional (ASQ:SE). Results: Six-month prevalence estimates were 25.4% for internalizing disorders, 12.1% for externalizing disorders, and 30.3% for socioemotional development delays. MBDD prevalence estimates were higher in families with stressful relationships and parental depression or anxiety, and in families with lower social capital. Conclusion: At least 25% of preschool children living in an urban area in Brazil presented a mental health disorder. These mental disorder were associated with modifiable factors such as stressful family relationships and lower social capital. Psychiatric diagnoses were determined through the Structured Clinical Interview for the DSM-IV. Symptom severity was assessed with standardized rating scales.</p><p><strong>Results: </strong>Post-treatment levels of anxiety symptoms were significantly lower than baseline.
